Health Risk Assessment. An assessment that is completed for all members through direct contact with the member, guardian, or adult caregiver.
Health Risk Assessment. An assessment tool which identifies primary, acute, long-term services and supports, and Behavioral Health and functional needs.
Health Risk Assessment. The Contractor shall complete a health risk assessment within ninety (90) days after enrollment for Enrollees stratified as high or moderate risk, except as follows:
2.6.3.1 For those Enrollees receiving HCBS Waiver Services or residing in NFs as of the Effective Enrollment Date, the health risk assessment relating to those Covered Services must be face-to-face and completed within the 180-day transition period. For all other Enrollees newly determined to be eligible for HCBS Services or transitioning to NFs, the health risk assessment must be face-to-face and completed within ninety (90) days after enrollment.
Health Risk Assessment. The Employer will offer an annual Health Risk Assessment to all bargaining unit employees and their spouses.
Health Risk Assessment. The Employer will offer an annual Health Risk Assessment to all bargaining unit employees and their spouses. Eligible employees shall receive a reduction in the required employee contribution, as identified in e) below, by doing the following:
1) The covered employee, and eligible spouse, completes an annual Health Risk Assessment (HRA) by the date established by the Personnel Department of the preceding year. It is understood that the requirement for the eligible spouse is met by the non-employee spouse completing the questionnaire portion of the HRA.
i) Newly hired employees, and eligible spouses, shall complete the HRA prior to their eligibility date in order to receive said discount.
2) The covered employee, and eligible spouse, agrees to have their HRA results shared with their primary care physician by signing the appropriate release with the party administering the HRA.
Health Risk Assessment. The purpose is to avoid harm to people due to Health Hazard by carrying out Health Risk Assessment (HRA) and implementing the control and recovery measures specified. For contractors, the inclusion of identified health hazards and controls in JHAs is sufficient – a specific HRA will not be required if the health hazards and controls are identified in JHAs. - Prime Contractor and subcontractor activities. - All Health Hazards associated with work.
Health Risk Assessment. Contractor shall use its best efforts to complete a health risk assessment for:
5.13.5.1 All Level 3 (high‐risk) Enrollees: The assessment will be conducted, in‐person or over the phone, and an IPoC will be developed within ninety (90) days after enrollment. For any Child in Level 3, the health risk assessment will be applied as per Attachment XXII.
5.13.5.2 Enrollees receiving HCBS Waiver Services or residing in NFs as of their Effective Enrollment Date with Contractor: The health risk assessment must be face‐to‐face and completed within ninety (90) days after the Effective Enrollment Date.
5.13.5.3 Enrollees receiving HCBS Waiver services or residing in NFs as of the Effective Enrollment Date, who were enrolled in another MCO, but are transitioning to Contractor’s Health Plan: The health risk assessment relating to those Covered Services must be face‐to‐ face and completed within the first 90‐days after the Effective Enrollment Date.
5.13.5.4 Enrollees transitioning to NFs: The health risk assessment relating to those Covered Services must be face‐to‐face and completed within the first ninety (90) days after the Effective Enrollment Date.
5.13.5.5 Enrollees deemed newly eligible for HCBS Waiver Services: The health risk assessment must be face‐to‐face and completed within fifteen (15) days after Contractor is notified that the Enrollee is determined eligible for HCBS waiver services.
Health Risk Assessment. The Contractor shall have a risk assessment and make its best efforts to complete a health risk assessment, by following the procedures outlined in this section, within ninety (90) days after the Effective Enrollment Date for Enrollees stratified as high or moderate risk, except as follows:
Health Risk Assessment. Employees (and their spouses, if applicable) are expected to participate in an annual health risk assessment with his/her health care provider. The health risk assessment includes height, weight, pulse and tests for the following outlined on the Health Risk Assessment form: Fasting Glucose Hemogram Lipid Panel The Health Risk Assessment form will be available in the Human Resources Department.
Health Risk Assessment. 2.7.2.1. The Contractor shall provide each Enrollee with a timely HRA (see Appendix M) completed by the Contractor’s Care Management team. All HRA tools must be approved by DMAS prior to use and conducted in a location that meets the needs of the Enrollee. HRAs will encompass social, functional, medical, behavioral, cognitive, LTSS, wellness and prevention domains, as well as the Enrollees’ strengths and goals, need for any specialists and the plan for Care Management and coordination. Each element of the HRA, including a description of the LTSS and other covered services to be provided until the next POC review, will be reflected in the Individualized POC, and the ICT will ensure that all relevant aspects of the Enrollee’s care are addressed in a fully integrated manner on an ongoing basis.
2.7.2.2. Relevant and comprehensive data sources, including the Enrollee, providers, family/caregivers, etc., shall be used by the Contractor. Results of the assessment will be used to confirm the appropriate stratification level for the Enrollee and as the basis for developing the POC.
2.7.2.3. During Demonstration Year 1 (which encompasses both CY 2014 and CY 2015), the Contractor must complete a HRA for all Enrollees meeting any of the following criteria no later than sixty (60) days from the individual’s Enrollment date:
2.7.2.3.1. Individuals enrolled in the EDCD Waiver;
2.7.2.3.2. Individuals with intellectual/developmental disabilities;
2.7.2.3.3. Individuals with cognitive or memory problems (e.g., dementia or traumatic brain injury);
2.7.2.3.4. Individuals with physical or sensory disabilities;
2.7.2.3.5. Individuals residing in nursing facilities;
2.7.2.3.6. Individuals with serious and persistent mental illnesses;
2.7.2.3.7. Individuals with end stage renal disease; and,
2.7.2.3.8. Individuals with complex or multiple chronic conditions.
2.7.2.4. During the first year of the Demonstration, the Contractor must complete a HRA within ninety (90) days of Enrollment for all other Enrollees.
2.7.2.5. The Contractor must conduct HRAs for individuals enrolled in the EDCD Waiver and for Enrollees residing in nursing facilities face-to-face. The Contractor must incorporate pertinent information from the MDS into HRAs for individuals residing in nursing facilities.
2.7.2.6. During subsequent years of the Demonstration, the Contractor must complete HRAs within thirty (30) days of Enrollment for individuals enrolled in the EDCD Waiver and within sixty (60) days of Enroll...
