License Information. Contractor agrees that all facilities and staff including, but not limited to, all professional and paraprofessional staff used to provide services will maintain throughout the term of this Contract, such qualifications, licenses and/or permits as are required by state or local law. Contractor shall provide County a list of all licensed persons who may be providing services under this Contract. The list shall include the name, title, professional degree, license number, and NPI number.
License Information a. b. Licensing Restrictions
1) Contractor certifies that none of its subcontractors or agents has ever had a professional license, registration or certification revoked, limited, restricted, suspended, placed on probation/conditional status, or had other disciplinary action taken against them by a licensing or certification board. Contractor staff must also attest to the above.
2) Contractor certifies that none of its subcontractors or agents have ever had professional privileges or membership revoked, cancelled or denied. Contractor staff must also attest to the above.
3) Contractor certifies that any of its subcontractors or agents who have ever been convicted of a felony have reported the circumstances of the conviction to the contractor and the contractor has determined that the conviction will not affect the individual’s ability to perform any of the contracted functions. Contractor staff must also attest to the above.
License Information. Any Software licenses (including Subscription Licenses) granted by Avaya to Customer under this SLSA shall be in accordance with the then-current terms and conditions set forth in the EULA.
License Information. The FCC Licenses and PUC Authorizations held or managed by the Restricted Companies that are material to the Mobile Communications Business of the Restricted Companies are valid and in full force and effect without conditions except for such conditions as are generally applicable to holders of FCC Licenses and such Authorizations. No event (including the pendency of any petition to deny, informal objection, petition to revoke, or other complaint, investigation or proceeding before the FCC or any PUC) has occurred and is continuing which could reasonably be expected to (i) result in the imposition of a material forfeiture or the revocation, termination or adverse modification of any FCC License or PUC Authorization held or managed by a Restricted Company and material to the Mobile Communications Business of the Restricted Companies or (ii) materially and adversely affect any rights of the Restricted Companies thereunder that are material to the Mobile Communications Business of the Restricted Companies. None of the Restricted Companies has any reason to believe or has any knowledge that the FCC Licenses and PUC Authorizations held or managed by the Restricted Companies that are material to the Mobile Communications Business of the Restricted Companies will not be renewed in the ordinary course. The Restricted Companies (taking into account the Commitments hereunder and the other financial sources reflected or assumed in the business plan of NCI set forth in the Information Memorandum) have sufficient time, materials, equipment, contract rights and other required resources to complete, in a timely fashion and in full, construction of each SMR System they currently operate or currently plan to operate in compliance with all applicable technical standards and construction requirements and deadlines. The current ownership and operation by each of the Restricted Companies of its Mobile Communications Business comply with the Communications Act of 1934, as amended, and all rules, regulations and policies of the FCC, any PUC and of any other Governmental Authority, except for such non-compliance that could not, individually or in the aggregate, reasonably be expected to result in a Material Adverse Effect.
License Information. Any Software licenses (including Software licenses sold on a subscription basis) that Avaya grants to the Customer under this SLSA shall be in accordance with the then- current terms and conditions set forth in the Avaya Global Software License Terms, found at xxxx://xxxxxxx.xxxxx.xxx/LicenseInfo or a successor site (sometimes referred to herein as the “XXXX”). Avaya may release new versions of the Avaya Global Software License Terms, which will replace the previously applicable version prospectively.
License Information. To the extent requested by the Administrative Agent, Part 2 of SCHEDULE 7.1(w) hereto sets forth, as of the date hereof (or as of the dates specified on such Schedule with respect to particular information), a true and complete list of the following information for each FCC License or PUC Authorization issued to the Borrower or any Subsidiary:
(1) for each FCC License issued to the Borrower or any Subsidiary, the call sign, the name of the licensee, the transmitter location, the type of service, the frequency or frequencies authorized, the site name or location and the expiration dates; and
(2) for each PUC Authorization issued to the Borrower or any Subsidiary, the geographic area covered by such Authorization, the services that may be provided thereunder and the expiration date, if any.

(i) for all FCC Licenses (including all SMR Licenses), the name of the licensee, the name of the seller(s), the call sign, the transmitter location (by site coordinates and city), the category or type of service
License Information. Schedule 2.24 sets forth a true and complete list of the following information for each FCC License issued to or operated by Chadmoore or its Subsidiaries (including all FCC Licenses subject to a Chadmoore Management Agreement):
(i) for all FCC Licenses (including all SMR Licenses), the name of the licensee, the name of the seller(s), the call sign, the transmitter location (by site coordinates and city), the category or type of service (e.g., paging, SMR, etc.), the frequency or frequencies authorized, and operating entity;
(ii) in the case of SMR Licenses, the number of channels authorized, the number of channels constructed, whether the license is for a conventional or trunked SMR System, the applicable loading date, whether the SMR License is subject to a Finders Preference, whether the SMR License is operating under Special Temporary Operating Authority ("STA") and the applicable STA expiration date, and whether the SMR License is managed by Chadmoore pursuant to a Chadmoore Management Agreement or by any other Persons pursuant to a Third-Party Management Agreement;
(iii) each holder of any such FCC License that is neither wholly-owned by Chadmoore nor owned entirely by unaffiliated Persons and managed by Chadmoore; and
(iv) for all FCC Licenses (including SMR Licenses), whether such FCC Licenses are subject to rights of first refusal, options and other such rights or obligations in existence on the Signing Date, including, without limitation, entitlements to acquire additional ownership interests, which may affect the ownership interests of Chadmoore.

If Licensee does not have previously agreed to license terms in effect for the Program, the Globlue Program License Agreement (GTL-ARKHOS-TI2C-120116) applies. Program Name: ARKHOS TLOxp i2 Connector Program Number: GTL-ARKHOS-TI2C-120116 As described in the Program License Agreement ("PLA") and this License Information, Globlue grants Licensee a limited right to use the Program. This right is limited to the level of Authorized Use, a Value Unit License ("VUL"), or other specified level of use, acquired by Licensee as evidenced in the Proof of Entitlement. Licensee's use may also be limited to a specified machine, or only as a Supporting Program, or subject to other restrictions. As Licensee has not paid for all of the economic value of the Program, no other use is permitted without written authorization from Globlue and TransUnion, at their sole discretion. Additional ARKHOS Intelligence Platform components may be available to Licensee subject to the payment of additional fees or under different or supplementary terms. Globlue reserves the right to determine whether to make such additional rights available to Licensee. If the Program is designated as for "Deployment", the Program can be used for production, but may not be used to develop applications that utilize the Program. Licensee is not authorized to use any part of the Program for development purposes without the appropriate development entitlements.
