If applicable, the file shall include: Service Period Begin Date; Service Period End Date; Provider Unique Identification Number; Provider Name; Provider Street Address; Provider Telephone Number; Provider City; State; Zip; Provider e-mail addresses (if available centrally in electronic format); Medicaid payment made by DHS/OHA for each Adult Xxxxxx Care resident, to include separately the total service rate and the DHS/OHA-paid portion.

List and Information. The Public Authority shall provide the Union on a monthly basis a list of each Independent Provider in the unit represented by the Union. This list will include name, home address, date of hire, telephone number, date of birth, date of termination and hours worked in a designated payroll period. The list shall be provided electronically or on floppy disks in an agreed upon format. The Union shall have the right to mail materials to the home address of Independent Providers. For Independent Providers whose telephone number is also that of a Consumer, the Union agrees that calls will not be made to the Provider when the Consumer is receiving care and also agrees to terminate calls if the Consumer objects to calls to the home phone number. The Public Authority will include official Union notices in mailings to the bargaining unit from the Public Authority if the Union provides such notices to the Public Authority five (5) working days prior to the mailing date, and if the Union reimburses the mailing costs to the Public Authority in the event that the Union documents increase mailing costs. The Union shall be allowed use of reasonable space on available bulletin boards within the offices of the Public Authority for communications having to do with official organization business, such as times and places of meetings provided such use does not interfere with the needs of the Public Authority. Union Stewards and Union Representatives shall be allowed to attend and disseminate information at orientations and Public Authority Training sessions. The Public Authority shall provide the Union with 10 days notice prior to any such orientation or training.

As permitted by law, the Public Authority shall, on a monthly basis, between the first (1st) and fifteenth (15th) of each month, provide the SEIU ULTCW a list of all current providers including name, address, telephone numbers, social security number, hours authorized and hours worked. The list is to be provided in an agreed-upon electronic format.

The final data system shall be developed by SPD payment unit staff and SEIU information technology staff.

At such time as the information is made available to the Public Authority, the Public Authority shall arrange for delivery or provide the Union a list of all current Providers, including name, and if available, hours worked in a designated payroll period. The list shall be provided on floppy disks in an agreed upon format. The Public Authority will update this list to the Union on a monthly basis upon request. The Union shall defend, indemnify, save, protect and hold harmless the Public Authority, its Board, directors, officers and providers from any and all claims, costs and liabilities for any damages and/or injury arising from disclosure to the Union of IHSS provider names, social security numbers, addresses and phone numbers. The Public Authority’s right to be defended, indemnified, saved, protected and held harmless hereunder shall be unaffected by the concurrent negligence of the Public Authority or any other person, and this right shall survive the expiration date of this MOU.
