Notification of Pregnancy. As soon as an employee realizes she is pregnant, she shall submit an IOC through the chain of command and a written statement from her physician, including the following:

Notification of Pregnancy. (i) A female Medical Radiation Technologist or Technician is required to notify the Employer of her pregnancy immediately upon becoming aware of pregnancy.

FROM YOU You are encouraged to notify Human Resources (HR) and your manager about your pregnancy, as soon as possible, but no later than the qualifying week (which is the 15th week before the Expected Week of Childbirth). This will allow you to establish what financial support you are entitled to and enable a risk assessment to be carried out to protect the health, safety and welfare of you and your unborn child. Solihull MBC also requires the following information and documents, otherwise your SMP (Statutory Maternity Pay) will be affected:- ▪ An original MATB1 form. ▪ A completed MS1 form (found in Appendix ONE) at least 28 days before you wish to start your maternity leave. You will notice that the MS1 form asks about your choice of option with regards to the occupational maternity pay scheme. This is a scheme in which Solihull MBC pays an additional 12 weeks at half pay on top of the statutory scheme. Section 5 of this booklet will explain this further and outline any conditions that apply. Please send your MS1 and original MATB1 directly to the HR Shared Service team or via your line manager, who will forward it on to us. If you have chosen a maternity leave start date, but then you wish to change your mind, please let us know at least 28 days before the revised start date or 28 days before the original date that you chose, whichever of these two dates is the earlier.
Notification of Pregnancy. (a) An employee who is pregnant shall be granted seventeen (17) consecutive weeks maternity leave without pay commencing at any time during the seventeen (17) week period immediately preceding the expected date of delivery, provided that the employee gives the Employer written notice at least four (4) weeks before the day on which the employee expects to commence her leave. All other issues of notice or extension of the period of maternity leave shall be according to the Employment Standards Act.
Notification of Pregnancy. A faculty member who is pregnant shall notify the appropriate department head, Division Xxxx, or Vice President in writing of the desire to take leave, and shall give such notice as soon as possible. The faculty member shall provide the College with the attending physician's approval of the length of time said faculty member may remain at work. The faculty member may, with the attending physician's approval, continue in active employment as late as said approval indicates.
Notification of Pregnancy. (a) In accordance with Duty of Care obligations and Health and Safety considerations, a pregnant member is to notify their Officer in Charge as soon as practicable of their pregnancy. Where duties pose a risk to the member or unborn child, modification of duties or a temporary transfer may occur in accordance with the policy developed in consultation between the Commissioner of Police and the NTPA.

(i) An employee who becomes pregnant shall notify the Employer of her pregnancy at least 15 weeks prior to the expected date of termination of her pregnancy and, subject to section (ii) of this Clause, shall, eleven (11) weeks before the expected date of the termination of her pregnancy be granted leave without pay for a period ending not later than twenty-six (26) weeks after the date of the termination of her pregnancy.
Notification of Pregnancy. In view of the potential risk of flying during pregnancy, a Cabin Crew employee is expected to notify the Cabin Crew Manager of the pregnancy without delay as soon as the employee becomes aware of the pregnancy. This is to enable easyJet to ensure, to the extent reasonably possible, that all health and safety risks arising from work are avoided. Cabin crew will be removed from the roster for flight services once the pregnancy notification has been received.
