of Benefits. Notwithstanding anything in this Agreement, the Trust's Employee and Trust Manager Incentive Share Plan (the "Plan"), any agreement entered into under the Plan, or under any retirement, pension, profit sharing or other similar plan, upon the occurrence of a Change in Control, all deferred or unvested portions of any award made to Executive under any of the foregoing plans and agreements shall automatically become fully vested in Executive and shall be in effect and redeemable by or payable to Executive, or Executive's designated beneficiary or estate, on the same conditions (other than vesting) as would have applied had the Change in Control not occurred. All unvested awards under the Plan shall immediately vest upon the Change in Control and the Executive shall have the right to exercise any vested awards during the balance of the awards' term.
of Benefits. The Participant's Account Balance attributable to his or her Deferral Accounts, and Notional Investment Adjustments thereto, will always be 100% vested. Subject to Section 3.7, credits to each Participant's Company Matching Accounts, and Notional Investment Adjustments thereto, and credits to each Participant's Company Discretionary Accounts, and Notional Investment Adjustments thereto, will be vested in accordance with the provisions set forth in the Adoption Agreement.
of Benefits. If the cost of any proposed dental treatment is expected to exceed Manulife Financial suggeststhat you submit a detailedtreatment plan, available from your dentist, before the treatment begins. You can then be advised of the amount you are entitled to receive under this benefit.

When a claimant requires a course of dental treatment expected to cost in excess of three hundred dollars the dentist's treatment plan including estimates of the charges must be with the Company before treatments commence, otherwise no payment will be made to such claimant. If you or your dependent are disabled at the time of termination of your employment, charges as a result of such disability will continue to be paid up to days, provided the benefit remains in force. Claim Forms are available your employer. This form must be completed in and submitted with the original bills within days after the end of the calendar year in which the claim was incurred. Note: To ensure prompt claims service, any receipts should include: your name or your dependent’s name receiving the service or the date and the type of each service or treatment the charge for each date the prescription numbers for prescribed drugs and medicine the name of the drug or the medicine How to make an out-of province/country claim: There are special rules for claiming the costs of emergency treatment outside of your home province or Canada. For all medical expenses, you must contact the Travel Assistance provider at the time of the emergency. This will enable the Assistance provider to co-ordinate payment directly with the hospital and/or medical provider involved. In addition, with your approval the Travel Assistance provider will co-ordinate payment with your Provincial Health Care plan. If a medical provider or hospital bills you directly, send the bill along with your claim form to the Travel Assistance provider. Note: If your spouse has insurance with another carrier, please also refer to the Coordination of Benefits section for claim submission information. If the Group Insurance Policy terminates, all claims must be submitted within days of the cancellation. Otherwise the claim will be denied.
of Benefits. Where in addition to being a member of the Scheme the Member is also a member of an approved scheme (the voluntary scheme) which provides additional benefits to supplement those provided by the Scheme and to which no contributions are made by any employer of his, the provisions of the paragraph that follows shall apply in relation to any augmentation of the benefits provided for him by the Scheme after he has ceased to participate in it. Any provisions in the Scheme imposing a limit on the amount of a benefit provided for the Member shall have effect (notwithstanding anything in them to the contrary) as if they provided for the limit to be reduced by the amount of any like benefit provided for the Member by the voluntary scheme.
of Benefits. An instructor granted a statutory leave shall continue to participate in those benefit plans [other than the sick leave credit plan but including the life insurance plan; extended health plan and dental plan) which the instructor already enjoys unless the instructor elects not to do so. The Board shall continue to pay its share of the cost of such benefits during such statutory but not during any extensions of such leaves. Such instructor shall assume the total cost of these benefit plans for any period of leave that exceeds the above mentioned periods if the instructor elects to remain covered under such benefit plans.
