Other Supportive Services. C.3.22.1 In addition to case management services, the Provider shall provide directly, or partner with other agencies, organizations or community-based providers, to ensure households have access to additional services onsite and in the community to support the successful transition of households to housing within 90 days or less. Services offered may vary by site depending on the physical layout of each site and other factors unique to the STFH building, community location, or partnerships established between the Provider and other providers, organizations or agencies.

If the household’s assessment score falls within the PSH eligibility range, the assessor must contact the Office of Supportive Housing’s RRH Program Monitor to determine next steps. Program Description In coordination with the County’s Department of Employment and Benefits Services (DEBS), the contractor shall process financial assistance requests as specified by the applicable SSA program manager.   • Supportive Services 2.1. Case Management Access Shelter Providers are required to have case management available to participants on site. Participation within case management is voluntary to program participants, however all participants must be offered case management and must be engaged on an ongoing basis to encourage participation. Shelter Providers should recognize that it may take multiple contacts before a participant is ready to engage. Shelter Providers must ensure case management services are participant-centered to individual needs. Programs must provide space for the provision of case management that works to create as much privacy and confidentiality as possible.

Nothing in this Agreement shall be deemed to shift to you or to diminish the obligations of any agent of the Fund or any other person not a party to this Agreement which is obligated to provide services to the Fund.

  • Transition Services The Purchasers will provide to the Sellers termination assistance as reasonably requested in order to provide an orderly transition following the termination of the Agreement (or any portion thereof), and the Sellers will provide to the Purchasers reasonable cooperation and assistance in connection therewith. In connection with this transition assistance, the Purchasers and Sellers will reasonably cooperate in the transition of the Services from the Purchasers to any Replacement Provider.
