Preparation of Assistance Plan. As soon as possible after assignment, the Consulting Teacher shall review the referring packet for the Referred Teacher/Volunteer Teacher and the principal together or separately, as appropriate, to review the employee’s performance and recommendations for improvement. The Consulting Teacher will then prepare a written Individual Assistance Plan aimed at remedying the deficiencies which were cited in the Referred Teacher’s unsatisfactory evaluation or meeting the goals set forth in the Volunteer Teacher’s application. For the Referred Teacher, it will include performance goals which are aligned with pupil learning and consistent with California Standards for the Teaching Profession and a projected date for completion which will ordinarily be at the end of the current school year, The Plan will be submitted to the PAR Panel for final development and approval.

The Consulting Teacher will then prepare an Assistance Plan on forms generated by the panel, which will include goals and objectives for improvement and a projected date for completion, which will ordinarily be at the end of the current school year. The Plan will be submitted to the Panel for final development and approval.

  • Plan of Assistance a. If an educator receives an “Unsatisfactory” rating on any component on the summative evaluation, a written Notification of Unsatisfactory Performance will be provided to the educator which will include areas requiring improvement.

  • Employee Assistance Plan The Board shall make available to eligible employees and their eligible family members, at no cost, an Employee Assistance Plan (EAP). Employee participation in and/or referral to the EAP shall be voluntary and confidential, except as to any disclosures required by applicable state law. All personal treatment records generated as a result of an eligible individual’s utilization of the EAP shall be maintained by the service provider and shall not be shared with the Board unless otherwise authorized by the eligible employee or the covered dependent, or by operation of applicable law. The contact person for the EAP services to be made available under this Agreement shall not be employed by the Board of Education.

  • EDUCATIONAL ASSISTANCE PROGRAM The District’s Educational Assistance Program will be continued during the life of this Agreement. Employees who wish to enroll in job-related and promotion oriented courses, which will improve their job knowledge, and performance will be compensated by the District for expenses as follows:

  • Employee Assistance Program (EAP) Section 1. The Employer agrees to provide to the Union the statistical and program evaluation information provided to management concerning Employee Assistance Program(s).

  • Employee and Family Assistance Plan The CODC PRO Care Plan is an industry-funded employee and family assistance plan for employees and their eligible family members according to the participation of sponsoring organizations and employers as well as Plan eligibility rules. Employees must be enrolled in the Plan by their employer to become eligible for Plan benefits, subject to the Plan eligibility rules. An individual employee cannot self-enroll in the Plan.
