Preventative. Maintenance Visits are provided in order to inspect, clean, calibrate, and update the Product, and are not intended to be a repair visit.

Additional Benefits (reimbursement at 70% up to $1000/calendar year) This program provides for a denture replacement after you have been enrolled under this program for one year.
Preventative. (a) Prophylaxis Scaling and Polishing –once every six (6) months for children and once every nine (9) months for members and spouses.

Coverage III - After deductible, 50% paid by insurance, 50% by patient. Crowns and build-ups for crowns; First placement of inlays and bridges; First placement of partial or full dentures. Coverage IV - Orthodontia, 50% paid by insurance, 50% paid by patient. Maximum Contract Year Benefit - The maximum amount which the plan will pay is $1,250 per person per Calendar Year. Orthodontia lifetime maximum is $1,000 per person. Employees shall pay 10% of the total cost for medical and dental insurance coverage. Employees will also have the option of dropping medical insurance coverage and receiving two months of company contributions towards the premium, rounded to the nearest $10. If an employee dies, medical and dental insurance coverage will be continued for the employee's spouse and dependent children for up to six months under the terms of the Medical and Dental Insurance Extension Policy HR 1.26, effective June 1, 2000. Supplemental Group Term Life Insurance Employees will have the option of purchasing supplemental group term life insurance equal to 1x, 2x or 3x their base pay (hourly wage times 2080), and pay the premiums through payroll deduction. The first $100,000 coverage will be issued without any evidence of insurability if the employee signs up for coverage when initially eligible. Evidence of insurability may be required by the insurance company:

Preventative. Flea and tick preventative and heartworm prevention is required year round. Flea and tick collars are not allowed. Treatment will be applied and a minimum $30 fee will be charged to the owner if any pet is found to have fleas or ticks.

Preventative maintenance visits include 1 lb. of freon as needed due to occasional loss during maintenance. If freon leak is noted, a leak check will be required or future visits will not receive courtesy freon.

Every day, and every time the mold is pulled from production or put back into production • Inspection: Every 20,000 cycles (or every 10 days of production) • Maintenance: Every 100,000 cycles (or every 10 production runs) • Major Maintenance: Every 250,000 cycles (or half the anticipated life time volume) Preventative Measures • The parting surfaces, core, and cavity should be gently cleaned with a mild, clean solvent and soft, clean towels to remove any buildup from vented gases, greases, and other resins that accumulate. Recommended at Mold Start-up, and end of production. • Before the mold is removed from the press the mold should be returned to room temperature to avoid condensation which will cause rust. • All water lines should be drained and blown free of all residual water to avoid build up of rust due to standing water. • The parting surfaces, core, and cavity should be gently cleaned with a mild, clean solvent and soft, clean towels to remove unwanted contamination. • The ejector system should be moved fully forward, then spray both mold halves with light rust preventive lubricant (Slide Mold Shield #42910P). Retract ejector system and close the mold. • Check and assure all bolts, plates, etc. are in place and tight. • Apply a thin film of new grease to moving components where applicable such as slides, lifters ejector pins, ejector guide, guide pins etc. • Do not grease Selflube aluminum bronze/graphite components such as bushings, wear plates or gibs. • Be sure all safety components are in place and in working condition before installing and cycling the mold. Including but not limited to, limit switches, mold protection cages, installation components (Lift Ring), shipping straps, warning plaques etc.
Preventative recall oral hygiene instruction limited to once every 5 months, but no more than twice in any 12 month period, initial oral hygiene instruction limited to once every 24 months; • prophylaxis including light scaling or polishing limited to once every 5 months but no more than twice in any 12 month period; • topical fluoride applications to the teeth of all persons covered once every 5 months but nor more than twice in any 12 month period, excluding home application fluoride; • space maintainers for dependent children under 16 years of age for premature loss of baby teeth; • interproximal discing for dependent children under 16 years of age only; • pit and fissure sealants are covered for dependent children under the age of 16 years only, limited to one application on any one tooth.
