Privacy and Communications. 6.12.1. The Licensor and London Accommodation Office cannot guarantee the security of data which you send to us by email. Please do not send us payment details using email.

The Licensor and London Accommodation Office (and our appointed third party providers) collect certain personal information (e.g. name, address, email address) about the Student ("Personal Information"). 6.12.3. The Student is responsible for ensuring the email address and other contact details provided to the Licensor and/or London Accommodation Office are correct. We will not be responsible if the Student does not receive communications from us because the details supplied are incorrect or in the event of telecommunication system failures outside our control.
6.12.4. The Licensor and/or London Accommodation Office communicate with the Student by email and by posting notices on Scape Website. The Student agrees to receive communications from us electronically in this way and that electronic communications will satisfy any legal requirement for communications be in writing.

To the extent the Licensee Data is subject to Europe’s General Data Protection Regulation, the United Kingdom’s Data Protection Xxx 0000, or Singapore’s Personal Data Protection Xxx 0000 (together, the “Data Protection Laws”), Illumina’s Data Processing Addendum available at xxx.xxxxxxx.xxx/xxx is hereby entered into between the Parties and is incorporated by reference into these Terms.

Eastlink may send you e-mails containing offers that Eastlink believes may be of interest to you and you hereby consent to receiving such offers. You also acknowledge, agree and consent to Eastlink (i) providing the above information and data to its third party suppliers to support our provision of the Service, and (ii) using relevant usage, performance or technical portions of such information and data on an anonymized basis
(1) to improve activation flow, (2) as part of trends or reports published by Eastlink or its third party suppliers, and/or (3) for marketing and sales purposes.

Holistic Health is a Covered Entity for purposes of the federal privacy and security law more commonly known as HIPAA (i.e., the Health Insurance and Portability Act of 1996, as amended, and its implementing regulations). As such, Holistic Wellness must comply with the applicable provision of HIPAA in conducting its operations and in its communications with its patients. Patient acknowledges communication by email, video chat, facsimile and text messaging offers great convenience, portability, and efficient exchange of medical information. Even with precautions, these communications have some risk of loss of privacy, and Holistic Wellness cannot guarantee the security or confidentiality of such communications. Any such communications may become a part of your medical record. It is recommended that Patient not use electronic media for the communication of sensitive or personal information, but rather make these communications by phone or in person. Regardless as to whether Patient elects to the Holistic Wellness secure email communication methods (Elation Passport), Patient understands and agrees that E-mail is not an appropriate means of communication regarding emergency or other time-sensitive issues or for inquiries regarding sensitive information. In the event of an emergency, or a situation in which the member could reasonably expect to develop into an emergency, Member shall call 911 or the nearest emergency room, and follow the directions of emergency personnel. If Patient does not receive a response to an e-mail message within two days, Patient agrees to use another means of communication to contact Holistic Wellness. Neither Holistic Wellness, nor its providers will be liable to Patient for any loss, cost, injury, or expense caused by, or resulting from, a delay in responding to Patient as a result of technical failures or other failures outside of the control of Holistic Wellness, including, but not limited to,

Privacy and Communications. 6.1. I understand that any protected health information will be safeguarded by the Practice’s commitment to current medical ethics and standards, and I have reviewed and understand the Practice’s Notice of Privacy Practices for Protected Health Information.
6.2. I understand that any and all methods of correspondence may be used by the Doctor and the Practice to generate information for the member’s medical records.
Privacy and Communications. You acknowledge that communications with Physician using e−mail, facsimile, video chat, text messaging, and cell phone are not guaranteed to be secure or confidential methods of communication. Practice will make an effort to secure all communications via passwords and other protective means and these will be discussed in an annually updated Health Insurance Portability and Accountability Act (HIPAA) "Risk Assessment." Practice will make an effort to promote the utilization of the most secure methods of communication, such as software platforms with data encryption, HIPAA familiarity, a willingness to sign HIPAA Business Associate Agreements. This may mean that conversations over certain communication platforms are highlighted as preferable based on higher levels of data encryption, but many communication platforms, including email, may be made available to Patient. If Patient initiates a conversation in which Patient discloses "Protected Health Information (PHI)" on one or more of these communication platforms then Patient has authorized Practice to communicate with Patient regarding PHI in the same format.
