All operators are required to have district email accounts and check them regularly throughout the school year. The Transportation Department will contact operators by email, phone, and/or school messenger alerting them of an upcoming field trip. The operator has until close of business of the next business day to accept or decline the trip with the Transportation Department. If the operator does not reply, the Transportation Department will send the operator a reminder notification. The operator has until close of business the next business day to respond to the reminder notification, accepting or declining the field trip. If the operator does not respond by the suspense for the reminder notification, the Transportation Department will decline the field trip on behalf of the said operator and move the operator to the bottom of the order of merit list and subsequently offer the trip to the next operator on the board order. Once the next operator completes the trip, Transportation will move that operator to the bottom of the order of merit list.

Methods of Communication. All notices and other communications permitted or required to be given under this Agreement shall be in writing, delivered and effective as follows to the address specified in this Agreement or at any other address subsequently provided by a notice in compliance with this notice provision: (a) delivered in person, effective upon personal hand delivery, (b) effective on business day after being sent by certified mail, postage pre-paid return receipt requested with written confirmation of receipt, (c) effective one business day after being sent by reliable nationally recognized overnight courier with written confirmation of receipt, (d) on written confirmation of facsimile transmission, or (e) by email or other electronic communication method such as posting on a designated internet portal, effective after the time sent (as recorded on the device from which the sender sent the email) or posted.

Communications by their nature cannot be guaranteed to be secure or confidential. If you initiate a conversation in which you disclose protected health information on any of these Communication platforms, then you authorize Practice to communicate with you regarding all protected health information in the same format. Communications technology and platforms are wholly outside of our control. Therefore, Practice and our providers shall not be liable to you, or anyone, for any cost, damage, expense, injury, or other loss relating to Communications, malfunctions, or delays in response. Your signature on this Agreement attests that you have read, understand, and agree to our Informed Consent for Telehealth and that you have been given a copy of such or opted to use a digital copy.
