Report of Absence and Licensed. Medical Practitioner statements will be handled confidentially by all employees who handle them. If medical attention is required, a certificate stating the nature of the illness from a licensed medical practitioner shall be required to justify the use of sick leave. An illness which exceeds three (3) days requires a licensed medical practitioner’s statement. Falsification of either a written, signed statement or a licensed medical practitioner’s statement shall be grounds for disciplinary action up to and including dismissal. Any abuse of sick leave can be just cause for disciplinary action independent of any denial of paid status for sick time away from the job. The Employer may request a Licensed Medical Practitioner statement from an employee and/or take disciplinary action where there is an indication of abuse of sick leave or excessive use. If the Employer requires a second opinion, the cost of such examination shall be paid for by the County. The Employer can require medical verification and/or take disciplinary action if excessive use of sick leave, or abuse of sick leave is suspected. (Examples of abuse may include, but are not limited to):

  • Effect of Absence Where any leave of absence without pay exceeds thirty (30) continuous calendar days, the following shall apply:

  • Notification of Absence To be entitled to payment in accordance with this clause the employee shall meet the following criteria;

  • Reporting Absences (a) Employees are responsible to report to work on time on each scheduled work day.

  • Reporting Absence Staff who cannot report to work because of sickness or other reasons are expected to telephone within fifteen (15) minutes of their normal starting time, to advise their supervisor of the expected time of their return to work. Staff members who commence work at 16:00 hours or later will make every effort to inform their supervisor(s) of their pending absences as early in the day as possible, and no later than 12:00 hours for the 16:00 hour or 18:00 hour shifts, or 15:00 hours for the midnight shift. However, employees failing to provide notice as stipulated in this Article through circumstances beyond their control shall not be deemed to have violated any of the terms of this Agreement. Staff should inform their supervisor of the reason for their absence. In the event of illness, exact medical reasons need not be given.

  • Authorization of Services a. The Contractor and its subcontractors shall have in place, and follow, written authorization policies and procedures.

  • Public Posting of Approved Users’ Research Use Statement The PI agrees that information about themselves and the approved research use will be posted publicly on the dbGaP website. The information includes the PI’s name and Requester, project name, Research Use Statement, and a Non-Technical Summary of the Research Use Statement. In addition, and if applicable, this information may include the Cloud Computing Use Statement and name of the CSP or PCS. Citations of publications resulting from the use of controlled-access datasets obtained through this DAR may also be posted on the dbGaP website.

  • Selection of Subcontractors, Procurement of Materials and Leasing of Equipment The contractor shall not discriminate on the grounds of race, color, religion, sex, national origin, age or disability in the selection and retention of subcontractors, including procurement of materials and leases of equipment. The contractor shall take all necessary and reasonable steps to ensure nondiscrimination in the administration of this contract.

  • Procurement of Recovered Materials (1) In the performance of this contract, the Contractor shall make maximum use of products containing recovered materials that are EPA-designated items unless the product cannot be acquired
