Required Medical Examinations. A ll employees shall file with the Business Office at the beginning of service a written report of a medical examination by a physician. The report shall be on the form provided by the school. Upon receipt of the required report, a copy of the doctor’s bill, and a copy of the insurance statement (if the employee is eligible for insurance) the B usiness Office shall reimburse the employee for the cost of the physical that is not covered by insurance up to $100.00. In the event that the Board requests a physical other than the beginning of service physical, for the purpose of determining an employee's ability to perform assigned duties, the Board shall pay the cost of this physical. It is understood that the Board may designate the physician who will conduct this extra required physical examination.

Required Medical Examinations. 1. The Board agrees to provide any and all physical or health examinations the Board may require free of expense to all Employees except for X-rays. If, in the course of the premium year, the Rider J benefits for an Employee and the Employee's family have been exhausted, the charge for any additionally required X-ray of the Employee will be reimbursed by the Board. In the event that an Employee's Rider J benefits are exhausted in the course of the premium year, and the Board has required X-ray paid for by Rider J, the Board shall reimburse the Employee for the cost of the X-rays which were required by the Board. 2. If the Employee chooses to decline the services of any licensed practitioner designated by the Board, he/she may select any licensed practitioner, approved by the Board, in which case the cost of such physical or health examination shall be borne by the Employee.

Notwithstanding the foregoing, in addition to the field examinations permitted above (A) during any calendar year when Aggregate Availability is at any time less than the greater of (x) an amount equal to 20% of the Loan Cap Minimum then in effect and (y) an amount equal to 20% of the Loan Cap then in effect, one additional field examination shall be permitted at the discretion of the Administrative Agent or the Required Lenders per calendar year and (B) if an Event of Default has occurred and is continuing, there shall be no limitation on the number or frequency of field examinations. For purposes of this Section 5.12, it is understood and agreed that a single field examination may be conducted at multiple relevant sites and involve one or more relevant Loan Parties and their assets. All of the foregoing field examinations shall be at the sole expense of the Loan Parties. Notwithstanding the foregoing, the Lenders shall be permitted to conduct one additional field examination per calendar year at their own cost and expense.

  • Annual Physical Examination A permanent employee shall be granted up to one day per year with pay for the purpose of a comprehensive physical examination provided that the verification of such an examination is submitted to the District.
