Responsibility and Supervision. 5.1 Age and Account Set -Up. You represent that the Service will be installed and Used solely in Your residential, commercial, retail, or other business location. You represent that You are at least 18 years of age and have the legal authority to order the Service(s). You agree that You are responsible for obtaining installation services for the Leased Equipment from a ConnectAnza installer and for verifying and maintaining the account, options, settings and other parameters under which the Service is Used, including (without limitation) all related passwords and User identification information.

Responsibility and Supervision. It is the understanding of the LEA’s within the SELPA that the existing SELPA Participant Agreement between the District of Residence Board of Education and the LACOE Board of Education grants the authority for students enrolled in LACOE classes to be the responsibility of LACOE staff and administration from the time they enter the school until they exit the program. It is also expected through this agreement that the host site principal and/or designee shall collaborate as defined through LACOE Procedures for Collaboration/Communication with Host Site School (Appendix A) with the LACOE Administrative staff to support management of programs and services of all LACOE programs on host sites during the period in which these programs are in operation on the host site. Governance structures within the SELPA allow for discussions and management of issues at the Program Specialist, Director and Superintendent Council levels in order to provide a continuum of placement options for students with moderate to severe disabilities in our region. It is the expectation that LACOE Area Administrator and PAU Administrators will communicate with the students district of residence and host site administrator/designee or staff while students are enrolled in LACOE programs and/or receiving services. LACOE shall comply with all state and local laws as well as host site campus specific policies and procedures regarding visitors to/on school campuses. LACOE shall be responsible for the purchase and provision of supplies and assessment tools necessary to implement the provision of LACOE programs and services on host site campuses. Facilities agreements exist between host sites and LACOE outlining specific needs relating to custodial, maintenance, modifications access, fire, health, sanitation and building standards and safety; fire warning systems, zoning and occupancy capacity. Issues relating to facility and facility modification needs of host site classrooms and should be referred to host site business offices and LACOE facilities if specific issues arise. Information relating to these agreements is available through the host site business office and at LACOE. According to the SELPA allocation plan, ADA for students enrolled in LACOE special day classes shall be transferred to LACOE. Additionally, the SELPA Local Plan & facilities plan formula address issues surrounding host site funding reimbursements.
Responsibility and Supervision. The Licensee must remain at the Facility at all times during the Rental period and must ensure that all children under the age of 18 are supervised by parents or guardians at all times. It is unacceptable to leave children alone in a room or to allow them to play on or near the stairs. Any Licensee who has left children unattended and/or any damage that has occurred by unattended children will be subject to forfeiting their Security Deposit.
