Schedule Requests. If an employee works a sixth (6th) consecutive day or six (6) out of seven (7) days because of the employee's own request or as a consequence of a request for change in schedule by another employee, then that employee shall not receive one and one-half (1 1/2) his or her hourly rate of pay for that sixth (6th) consecutive day or six (6) out of seven (7) days.

Schedule Requests. Schedule requests must be made at least three (3) weeks prior to the development of the schedule or according to individual unit/department practice. When an employee requests scheduled time off that reduces hours worked in a pay period less than the FTE of record, ET shall be used. Such requests will not be unreasonably denied. When a request for time off is of an emergency nature, ET shall be used if available. Otherwise the employee will take leave without pay. Such requests will not be unreasonably denied.
Schedule Requests. Management and staff of each unit will establish equitable, specific guidelines for making and granting schedule requests to work or not to work. Such guidelines will be voted on and approved by a simple majority of the bargaining unit members on that unit. Schedule requests must be submitted at least two (2) weeks in advance of posting. When conflicts arise in the application of unit guidelines and no resolution can be reached regarding scheduling, bargaining unit seniority shall prevail.   • Telephone Requests You agree (for joint accounts, all joint owners agree jointly and severally) that funds in any of your accounts with us can be transferred upon the telephone request of any signer on the account. In addition, you understand and agree that we may, from time to time, request and review credit reports and other information about you prepared by consumer reporting agencies or others.

  • Service Requests In support of services outlined in this Agreement, the Service Provider will respond to service related incidents and/or requests submitted by the Customer within the following time frames:  0-8 hours (during business hours) for issues classified as High priority.  Within 48 hours for issues classified as Medium priority.  Within 5 working days for issues classified as Low priority. Remote assistance will be provided in-line with the above timescales dependent on the priority of the support request.

  • DATA REQUESTS Upon the written request of the District, the State Auditor’s Office, the Appraisal District, or the Comptroller during the term of this Agreement, the Applicant, the District or any other entity on behalf of the District shall provide the requesting party with all information reasonably necessary for the requesting party to determine whether the Applicant is in compliance with its rights, obligations or responsibilities, including, but not limited to, any employment obligations which may arise under this Agreement.

  • PRODUCT AND PRICING CHANGE REQUESTS Supplier may request Equipment, Product, or Service changes, additions, or deletions at any time. All requests must be made in writing by submitting a signed Sourcewell Price and Product Change Request Form to the assigned Sourcewell Supplier Development Administrator. This approved form is available from the assigned Sourcewell Supplier Development Administrator. At a minimum, the request must: • Identify the applicable Sourcewell contract number; • Clearly specify the requested change; • Provide sufficient detail to justify the requested change; • Individually list all Equipment, Products, or Services affected by the requested change, along with the requested change (e.g., addition, deletion, price change); and • Include a complete restatement of pricing documentation in Microsoft Excel with the effective date of the modified pricing, or product addition or deletion. The new pricing restatement must include all Equipment, Products, and Services offered, even for those items where pricing remains unchanged. A fully executed Sourcewell Price and Product Change Request Form will become an amendment to this Contract and will be incorporated by reference.

  • Change Requests 4.1.1. Either party can request changes to the Service.

  • Access Requests (a) Each party agrees to reasonably cooperate with the other in connection with requests to access or amend Personal Data.

  • Employee-Requested Schedule Changes Overtime-eligible employees’ workweeks and work schedules may be changed at the employee’s request and with the Employer’s approval, provided the Employer’s business and customer service needs are met and no overtime expense is incurred.

  • Owner-Requested Changes If the Owner requests changes in the Contract Documents that would materially affect the completion of the Work by lengthening the critical path of the Overall Project Schedule, the Design Professional shall determine the appropriate number of days and thereby extend the Material Completion and Occupancy Date. The Contractor expressly agrees that the Contractor’s sole monetary remedy for such extensions of Contract Time shall be calculated at the daily rate established for Time Dependent Overhead Costs in the Contract.

  • Leave Requests All requests for leaves of absence must be in writing.

  • Special Requests Additional Services will be considered by request and billed separately: -Waxing -Shampooing of rugs and carpets -Additional Cleaning of areas or additional cleaning services not quoted in the Master Agreement’s Schedule, Building Areas and Janitorial Standards, and/or “Scope of Work” per agency.
