Service Delivery Sites. Most trainings will be offered at the subcontractors’ training centers; however, trainings for individual agencies may be delivered at the provider’s site. County trainings will be delivered primarily at county facilities or at other public areas convenient to staff members. Other venues may be used as necessary to accommodate larger audiences or special needs. When necessary and/or appropriate due to extenuating issues or safety concerns, trainings may be held virtually.
Service Delivery Sites. Service locations shall be determined and mutually approved by the Contractor and HCSA. Contractors shall collaborate with the County to adjust service locations within the geographic area in order to move with the service population. In addition to providing direct outreach and clinical services at encampments and other locations in the Contractor’s service area, the Contractor shall provide services at the following brick-and-mortar clinic: Main Street Village (MSV) Clinic – 0000 Xxxx Xxxxxx, Xxxxxxx, XX 00000 or any TCHC Primary Medical Care Clinic location
Service Delivery Sites. 7.1 Contractor must have a facility located in LAC for any case where a client needs to be seen at Contractor's facility and for DHSP to conduct monitoring reviews.
7.2 Contractor must request approval from DHSP, in writing, a minimum 30 days before terminating services at such location and/or before commencing services at any other location. Contractor must obtain prior written approval from DHSP before commencing services at a new or difference facility location.
7.3 A Memorandum of Understanding (MOU) is required for facility sites on locations or properties not owned or leased by Contractor with the landlord who owns or leases such location or property. Contractor will submit the MOU to DHSP for approval at least 30 days prior to implementation.
Service Delivery Sites. Narcotic Replacement Therapy services will be provided only in Provider’s Drug Medi-Cal certified sites that have been approved by County and DHCS.
a. The County and DHCS have approved Aegis Merced and Aegis Fresno sites.
b. Provider shall notify county and request approval of a new site as may be required in the event a Mariposa County resident and beneficiary with Mariposa County of responsibility Medi-Cal appears for service at another location. Provider will provide documentation required to set up the new site and receive DHCS approval so that the Medi-Cal claims can be successfully submitted from the new site.

Service Delivery Sites. 8.1 CONTRACTOR’S service delivery sites shall be located within the SPA for which the YDS Contract is awarded.
8.2 CONTRACTOR shall ensure that the service delivery sites are easily accessible (e.g. public transportation, disabled youth, etc.) to targeted Youth.
8.3 CONTRACTOR shall be responsible for securing the appropriate space on the campuses of "public or private” educational institutions accredited by any U.S. Regional Institutional Accrediting Association, recognized by the U.S. Department of Education OR other appropriate locations that facilitate emancipation preparedness training, to conduct life skills training classes and hands-on workshops. The appropriate space (e.g. Service Delivery Sites) shall be within the SPA for which the YDS Contract is awarded (see Section 8.0, Service Delivery Sites). A written request for alternate space may be presented to the County Program Manager by the Contractor and may be used upon written approval by County Program Manager.
8.4 Services shall be provided at the service delivery sites listed on SOW Exhibit A-5, Service Delivery Sites. All service delivery sites listed on SOW Exhibit A-5 shall be fully operational within 30 days of the commencement of the Contract.
8.5 CONTRACTOR shall request approval from County Program Manager in writing at least thirty (30) days before terminating services at any of the location(s) listed on SOW Exhibit A-5, Service Delivery Sites, and/or before commencing services at any other location(s) not previously approved in writing by the County Program Manager.
Service Delivery Sites. Contractor shall make services available at all Los Angeles Health Agencies, which includes DHS, DMH, and DPH directly operated and contracted agency sites located within Los Angeles County’s service areas (Service Areas 1 through 8).
Service Delivery Sites. 1. The Quality Assurance & Planning Coordinators for Xxxxxx Community College and Las Positas Community College will determine and provide the appropriate classroom and childcare sites.
2. Childcare room(s) will be in close proximity to the RFA Pre-Approval Training class sessions.
