Spousal Buy Back. Employees who are participants in the Genesee County Self-Funded Medical Plan, and who also have medical/hospitalization coverage through a spouse’s non-county plan, may participate in the Medical Spousal Buy Back Program upon submitting sufficient evidence of dual coverage to the Personnel Office, and upon signing an appropriate release form. In lieu of the employee’s participation in the County’s Self-Funded Medical Plan, the employee will receive an annual cash settlement of TWO thousand five hundred dollars ($2500). The cash settlement shall be paid in two equal annual installments, one $1250 during the month of July, and one $1250 during the month of December. Any employee wishing to participate in this program must initiate the process by requesting the release form from the Personnel Office, completing and returning same to the Personnel Office. The employee’s participation will commence on the first of the month following the receipt and approval by the Personnel Office. An employee hired on or after September 11, 1996 who has Medical/ Hospitalization coverage through a spouse’s medical plan or employment other than with the County will not be eligible to participate in the Genesee County Self-Funded Medical Plan and must participate in the Spousal Buy Back Program. (Refer to Memorandum of Agreement dated 12/1/2000) The County’s current practice with respect to hospitalization coverage for spouses both of whom are employed by the County shall continue for employees hired on or before December 31, 1980.

The cash settlement shall be paid in two equal annual installments, one during the month of July, and one during the month of December upon certification of the Highway Superintendent or the Supervisor of Buildings and Grounds to that effect. Any employee wishing to participate in this program, must initiate the process by requesting the release form from the Personnel Office, completing and returning same to the Personnel Office. The employee’s participation will commence on the first of the month following the receipt and approval by the Personnel Office. The annual cash settlement will be prorated effective the month following the ratification of the 1999-2002 Agreement by both parties (April 29, 1999).

  • Certification Regarding Debarment, Suspension, Ineligibility and Voluntary Exclusion - Lower Tier Covered Transactions (a) The prospective lower tier participant certifies, by submission of this proposal, that neither it nor its principals is presently debarred, suspended, proposed for debarment, declared ineligible, or voluntarily excluded from participation in this transaction by any federal department or agency.

  • Utilization of Sick Leave with Pay Temporary employees who have earned sick leave credits in their temporary appointment shall be eligible for sick leave for any period of absence from employment due to any of the following reasons: • illness; • bodily injury; • disability resulting from pregnancy; • necessity for medical or dental care; • if the employee is a victim of domestic violence, harassment, sexual assault, or stalking; or the parent or guardian of a minor child or dependent who is a victim of domestic violence, harassment, sexual assault or stalking, pursuant to ORS 659A.270 through 659A.290; • attendance at an employee assistance program; • exposure to contagious disease; • for the emergency repair of personal assistive devices which are medically necessary for the employee to perform assigned duties; • attendance upon members of the employee’s or the employee’s spouse’s immediate family, or the equivalent of each for domestic partners, (parent, wife, husband, children, brother, sister, grandmother, grandfather, grandchild, or another member of the immediate household) where the employee’s presence is required because of illness or death; • parental leave. The employee has the duty to insure that they make other arrangements, within a reasonable period of time, for the attendance upon children or other persons in the employee’s care. Certification of an attending physician or practitioner may be required by the Agency to support the employee’s claim for sick leave if the employee is absent in excess of seven (7) days, or if the Agency has evidence that the employee is abusing sick leave privileges. The Agency may also require such certificate from an employee to determine whether the employee should be allowed to return to work where the Agency has reason to believe that the employee’s return to work would be a health hazard to either the employee or to others. (See Section 4 for FMLA and OFLA.)

  • PRICING OF After Hours Coefficient What is your after hours coefficient for the RS Means Price Book for work performed after normal working hours? Remember that this is a ceiling price proposed. You can discount to any TIPS Member customer a lower coefficient than your proposed contract coefficient, but not higher. This is one of three pricing questions that are required for consideration for award on this solicitation. Please consider your answer carefully. An explanation of the TIPS scoring of pricing titled "Pricing Coefficient Instruction" is included in the attachments for your information. The below is an EXAMPLE of how the pricing model works (It is not intended to influence your proposed coefficient, you should propose a coefficient that you determine is reasonable for your business for the life of the contract): The most common after hours coefficient is time and a half of the RS Means Unit Price Book prices. To illustrate this coefficient, if your regular hours coefficient is .95, your after hours coefficient would be 1.45.

  • Certification Regarding Debarment, Suspension, Ineligibility and Voluntary Exclusion First Tier Participants:

  • Resume Self-Certification Form When submitting a response to an RFQ the Contractor shall submit with its response a completed and signed Resume Self-Certification Form (Contract Exhibit F) to the Customer for each proposed Staff member identified in the RFQ response.

  • Loading on Annual Leave During a period of annual leave an employee will receive a loading calculated on the rate of wage prescribed by subclause 7.1.3. The loading shall be as follows:

  • SICK LEAVE WITH PAY LIMITATION 175. An employee who is absent because of disability leave and who is receiving disability indemnity payments may request that the amount of disability indemnity payment be supplemented with salary to be charged against the employee's sick leave with pay credits so as to equal the amount the employee would have earned for a regular work schedule. If the employee wishes to exercise this option, the employee must submit a signed statement to the employee's department no later than thirty (30) days following the employee's release from disability leave.

  • Traditional IRA-to-Xxxx XXX Conversions If you convert to a Xxxx XXX, the amount of the conversion from your Traditional IRA to your Xxxx XXX will be treated as a distribution for income tax purposes, and is includible in your gross income (except for any nondeductible contributions).   • Non-Paid Status During Treatment After Positive Test The employee will be in a non-pay status during any absence for evaluation or treatment, while participating in a rehabilitation program.

  • Enrolled Nurse (With Notation Pay point 1 (a) Pay point 1 refers to the pay point to which an enrolled nurse (EN) has been appointed. (b) An employee will be appointed based on training and experience including: having satisfactorily completed a hospital based course of training in nursing of not more than 12 months duration leading to enrolment as an EN; or having satisfactorily completed a course of training of 12 months duration in a specified branch of nursing leading to enrolment on a register or roll maintained by a state/territory nurses registration board; and having practical experience of up to but not more than 12 months in the provision of nursing care and/or services, and, the undertaking of in-service training, subject to its provision by the employing agency, from time to time.
