Towing Services. If a Member’s vehicle cannot be started or driven safely, eAutorescue may provide tow services up to 10 miles destination as described below. There is a limit of one tow per breakdown incident. Tow services include tows to the nearest repair facility. Members will be responsible for the payment of additional costs and expenses of tows that are in excess of the 10 miles, specified above. Extras will be charged at the then-current rates for the service location and must be paid at the time of service. eAutorescue roadside technicians may perform winching/extrication services on a Member’s vehicle, but only if the vehicle can be safely reached from a reasonable distance (as determined in the discretion of the technician) from a passable, normally traveled road or established thoroughfare, and serviced with the equipment ordinarily used for Emergency Road Services. The roadside technician will assess whether a vehicle will be eligible for winching/extrication. If winching/extrication requires special equipment, more than one truck or additional personnel, the Member may be required to pay for such associated costs. eAutorescue service technicians are available to provide mobile vehicle diagnostics and perform minor repair services for Member vehicles as determined in the discretion of the technician. If the technician is unable to repair the vehicle, Member may elect to have the vehicle towed (subject to the towing terms and conditions, above).

Towing Services. Towing services shall include tow truck, a qualified driver and all fuel, maintenance, repairs, permits, insurance as outlined in the Illinois vehicle code, 625 ILCS 5/12-606 and any other items of expense or equipment necessary to render complete towing service. The towing service shall be licensed by the Illinois Commerce Commission (I.C.C.), and all tow truck drivers shall have a current license issued by the Secretary of State. Towing service shall comply with all Federal, State, I.C.C., and local legal requirements and policies. The towing service will have in operation at all times a minimum of two (2) heavy-duty wreckers each equipped with a 10,000-pound power winch, crane and boom; and one (1) flatbed type heavy-duty wrecker with a 10,000-pound capacity. However, a towing service who is unable to provide the flatbed type wrecker may execute a statement indicating that such a wrecker is available to be brought to a scene. Such statement shall be in writing and indicate the name and location of the alternate towing agency which will provide the flatbed wrecker. If a written contract is executed between the primary towing service and the alternate towing agency, a copy of said contract will be provided to the CITY. This statement will be incorporated as part of the agreement upon acceptance by the CITY. Should the alternate towing agency be used, all tows will be made to the primary towing facility. The towing service shall maintain and keep the equipment in good operating condition and appropriately licensed and inspected by the state. Evidence of license and last inspection shall be submitted to the CITY prior to execution of Agreement. Agency must show proof of ownership and/or contractual agreement acceptance to the city covering the term of the Agreement. The towing service agrees to maintain communication with CITY on a 24-hour availability basis. All trucks shall be equipped with a two-way radio on a commercial frequency for the sole purpose of communicating with the towing services base station and shall be appropriately licensed by the F.C.C. Service under this Agreement is to be rendered only upon request of the CITY. The towing service, upon notification by the police department shall immediately send the requested number of tow trucks to the designated location and at the direction of the police officer in charge at the scene shall remove the wrecked/abandoned/disabled vehicle or vehicles from the scene directly to the towing service’s sto...

  • Counseling Services SUD therapy/counseling is a relationship between people that works in part because of clearly defined rights and responsibilities held by each person. As a client in the SUD counseling process, you have certain rights and responsibilities that are important for you to understand. There are also legal limitations to those rights that you should be aware of. I, as your SUD counselor, have corresponding responsibilities to you. These rights and responsibilities are described in the following sections. SUD therapy/counseling has both benefits and risks. Risks may include experiencing uncomfortable feelings, such as sadness, guilt, anxiety, anger, frustration, loneliness and helplessness, because the process of therapy often requires discussing the unpleasant aspects of your life. However, therapy has been shown to have benefits for individuals who undertake it. Therapy often leads to a significant reduction in feelings of distress, increased satisfaction in interpersonal relationships, greater personal awareness and insight, increased skills for managing stress and resolutions to specific problems. But, there are no guarantees about what will happen. Therapy requires a very active effort on your part. In order to be most successful, you will want to put into practice things we discuss outside of sessions. The first 2-3 sessions will involve a comprehensive evaluation of your needs. By the end of the evaluation, I will be able to offer you some initial impressions of what our work might include. At that point, we will discuss your treatment goals and create an initial treatment plan. You should evaluate this information and make your own assessment about whether you feel comfortable working with me. If you have questions about my procedures, we should discuss them whenever they arise. If your doubts persist, I will be happy to help you set up a meeting with another SUD professional for a second opinion. PROFESSIONAL FEES The standard fee for a 50-minute individual session is $125.00 and the standard fee for a 90 minute couples session is $175.00 You are responsible for paying the full amount at the time of your session, unless prior arrangements have been made. Payment must be made by cash, or credit card. If you refuse to pay your debt, I reserve the right to use an attorney or collection agency to secure payment. APPOINTMENTS Appointments will ordinarily be 45-60 minutes in duration, once per week at a time we agree on, although some sessions may be more, or less frequent as needed. The time scheduled for your appointment is assigned to you and you alone. If you need to cancel or reschedule a session, I ask that you provide me with 24-hours notice. If you miss a session without canceling, or cancel with less than 24-hour notice, my policy is to collect the full session fee of $125.00/individual or $175.00/couples. It is important to note that insurance companies do not provide reimbursement for cancelled sessions; thus, you will be responsible for 100% of the full rate. If it is possible, I will try to find another time to reschedule the appointment. In addition, you are responsible for coming to your session on time; if you are late, your appointment will still need to end at the scheduled time. Finally, I have the right to terminate treatment after 2 missed appointments or habitual tardiness. Thank you for understanding. INSURANCE I am not a participating provider for any insurance plan. If you would like, I will supply you with a receipt of payment for services, which you can submit to your insurance company for reimbursement. PROFESSIONAL RECORDS I am required to keep appropriate records of the SUD therapy/counseling services that I provide. Your records are maintained in a secured, encrypted, HIPAA compliant web-based system. I keep brief records noting that you were here, your reasons for seeking SUD therapy/counseling, the goals and progress we set for treatment, your diagnosis, topics we discussed, your medical, social, and treatment history, records I receive from other providers, copies of records I send to others, and your billing records. Except in unusual circumstances that involve danger to yourself, you have the right to a copy of your file. Because these are professional records, they may be misinterpreted and / or upsetting to untrained readers. For this reason, I recommend that you initially review them with me, or have them forwarded to another mental health professional to discuss the contents. You also have the right to request that a copy of your file be made available to any other health care provider at your written request.

  • Dining Services The Contract is for a space in a University Housing & Dining Services (“UHDS”) facility and not for a particular room or type of housing. By signing this Contract, you agree to accept your residence assignment, and understand this assignment may change. Once you receive a key to your assigned residence or move personal belongings into your room (whichever comes first) you are considered to have taken occupancy (“Occupancy”) and will incur charges. You also agree to familiarize yourself and comply with all University policies governing occupancy, including those set forth in this Contract and in the UHDS publication Student Policy and Information Guide, to be considerate of other residents and to respect the rights of others at all times.   • Ordering Services a) By submitting an Order to Megaport: 1.
