Tracking, Reporting. With respect to all Equipment purchased hereunder, Company shall provide sales documentation reports ("Reports") in a mutually agreeable hard copy and electronic-Excel format, ("Sales Documentation Format"), within thirty (30) calendar days following the end of each calendar quarter for the Term of this Agreement and as may be reasonably requested by the Saint Luke’s requestor. Company shall further report the following on the form/document requested by Saint Luke’s: the Company’s product/equipment tracking number and SKU, GLN, UPN, UNSPSC, and GTIN. Such Reports will include information on all Equipment purchased by each Facility individually and in the aggregate hereunder and all information reasonably requested by Saint Luke’s. Company shall provide such Reports to the applicable Saint Luke’s requestor.

Tracking, Reporting. By 9:00 a.m. Central Daylight Time each Monday-Friday during the Program Period, USAA Bank will provide TrueCar with a report setting forth the information necessary for TrueCar to match all Subvention Sales to Discounted USAA Auto Loans occurring the prior day (“Loan Data Report”). Such report will be provided in a mutually agreed upon, secure format. By 6:00 a.m. Central Daylight Time the following day (i.e., Tuesday-Saturday), TrueCar will provide USAA Bank with a report identifying each Subvention Sale associated with a Discounted USAA Auto Loan for management and accounting purposes. In addition, by 6:00 a.m. Central Daylight Time Monday-Friday, TrueCar will provide the member number, loan number and discount rate so the rate adjustments can be systematically created. Additionally, USAA Bank agrees to work in good faith with TrueCar to continue to develop tools for, and provide full-file access to, loan and insurance data on a timely basis, with the goal of achieving real-time access for reporting purposes. Exceptions to this process will be handled manually by USAA Bank and TrueCar. Information provided by USAA pursuant to this Section 2(B)(4) of this Exhibit A shall be used to match Leads to USAA Auto Loans and vehicles purchased from Dealers in the USAA Auto Program to verify Subvention Sales for the purposes of administering the Loan Subvention Program, and for no other purpose. Such information shall be subject to the Confidentiality and Data Security obligations in the Agreement.
Tracking, Reporting. Contractor will track and report all required services and activities under this Agreement in the manner and frequency directed by federal, state, or ACWDB policy.
a. Data Entry: Contractor will use the management information systems (MIS) designated by ACWDB to track and report activities and services provided under this Agreement. This may require dual entry of data into multiple MIS. Contractor will enter required service and activity data into ACWDB designated MIS in an accurate, complete and timely manner. At a minimum, CalJOBS data entry for WIOA program services must incl agency code and activity code and must be entered/updated every 30 days.
Tracking, Reporting. A form (see sample in Exhibit A-3) has been developed for use by the Group Leader to track the following information: Prevention and Early Intervention (PEI) Services.
1. PEI Related Mental Health services as authorized by the Multi-Provider Treatment Team referred from Mendocino County Behavioral Health and Recovery Services:
a) Mental Health Services -$2.61 per minute.
b) Case Management Services - $2.02 per minute c) Rehabilitation Services - $2.61 per minute
Tracking, Reporting. TrueCar will keep record of the member visits to the OVPP, number of instances member contact information is submitted as well as the number of sales. These metrics will be provided to USAA on a monthly basis.
Tracking, Reporting. TrueCar will maintain record of claims that were received, reasons for fulfillment, not fulfilled, reasons for denial, as well as the outcomes to be reviewed by USAA on a monthly basis. This reporting shall include Member complaint escalations and final resolutions. In addition, TrueCar shall provide reporting on the payments made to Members. Such reporting will be provided in a mutually agreed upon format. Such reporting will be provided to USAA on a monthly basis.
Tracking, Reporting. Provides real-time reporting of Precision eMail deliverability and tracking of user interaction per Precision eMail message.
