Supportive Living Facility definition

Supportive Living Facility or “SLF” shall mean a residential setting that meets the requirements of Subpart B of 89 Illinois Administrative Code 146.

Supportive Living Facility or “SLF” shall mean a residential setting that meets the requirements of Subpart B of 89 Illinois Administrative Code 146, or the Comprehensive Care in Residential Settings Program.
Supportive Living Facility. (SLF) means a residential apartment-style (assisted living) setting in Illinois that is (i) certified by the Department to provide or coordinate flexible Personal Care services, twenty-four (24) hour supervision and assistance (scheduled and unscheduled), activities, and health related services with a service program and physical environment designed to minimize the need for Residents to move within or from the setting to accommodate changing needs and preferences; (ii) has an organizational mission, service programs and physical environment designed to maximize Residents’ dignity, autonomy, privacy and independence; (iii) encourages family and community involvement; and, (iv) administered by HFS under the Supportive Living Program HCBS Waiver.
Supportive Living Facility means a licenced facility providing permanent accommodation to four (4) or more adults in which the operator provides or arranges for services related to safety and security of the residents, and provides at least one meal a day or housekeeping services. Residents in a supportive living setting can range from seniors who require support services due to age, chronic conditions and frailty to young adults with mental health or physical disabilities;
Supportive Living Facility means an assisted living facility, continuing care facility, convalescent center, or nursing home. Supportive living facility does not include an adult family home, community residential facility, hospital, or secure facility.
