(WSII) for the Wealthsimple Self-Directed First Home Savings Account and request that WSII files an election to register the qualifying arrangement as a FHSA under the provisions of the Income Tax Act (Canada). I certify that the information contained in this application is true and correct and I have read the terms and conditions set out in the Declaration of Trust and I agree to be bound by them, as amended from time to time. If I am participating in a Group FHSA, I authorize and appoint the Group Sponsor in Part 1 as my Agent to act on my behalf for the purposes of administering the Group FHSA, and investing the FHSA assets.
