Agency Insurance. The Agency shall obtain, maintain, and keep in force throughout the term of this Agreement: (a) general liability and property damage (with an extended endorsement) insurance covering the use of the Agency Facilities in connection with the School’s Education Program and the Fieldwork Experiences, and (b) if applicable, professional liability insurance, or any other form of insurance necessary to provide liability coverage for the services to be provided under this Agreement, all issued by one or more insurance companies authorized to do business in the State of Hawai`i, each coverage with minimum limits of at least One Million Dollars ($1,000,000.00) arising out of each occurrence and at least Three Million Dollars ($3,000,000.00) in the aggregate. The liabilities to be covered by the insurance described hereunder may be covered through a self-insurance program.
Agency Insurance. The Agency shall have a clear risk management and personnel policies and procedures in place for student/s. It is recommended that the Agency carry volunteer accident and liability coverage.
Agency Insurance. Agency shall procure and maintain in force during the term of this Agreement, at its sole cost and expense, insurance in amounts that are reasonably necessary to protect it against liability arising from any and all negligent acts or incidents caused by its employees. Coverage under such professional and commercial general liability insurance shall be not less than one million dollars ($1,000,000) for each occurrence and three million dollars ($3,000,000) in the aggregate. Such coverage is to be obtained from a carrier rated A or better by AM Best or a qualified program of self-insurance. Agency shall also maintain and provide evidence of workers’ compensation and disability coverage for its employees as required by law. Agency shall provide University with evidence of the insurance coverage required by this paragraph upon request. Agency shall promptly notify University of any cancellation, reduction, or other material change in the amount or scope of any coverage required hereunder.

If requested, Agency will supply a Certificate of Coverage. Agency agrees to keep Xxxxxx Xxxxx University advised of any changes in this policy.

AGENCY will maintain at its sole expense a valid policy of insurance covering professional liability arising from the acts or omissions of AGENCY, its agents, and its employees in the amount of $1,000,000. AGENCY will forward a copy of its professional liability policy to PROVIDER, upon request, and will give prompt written notice of any material change in AGENCY coverage.

In the event that the Student is injured or becomes ill during the course of the experiential learning program, the Agency shall provide medical assistance and first aid to the Student as appropriate. The Agency shall be in compliance with all Federal and State regulations where the Student is involved. Please see “Contract” for Agency obligations specific to this experiential learning program. University Obligations and Responsibilities.

AGENCY will forward a copy of its professional liability policy to PROVIDER upon request and will give prompt written notice of any material change in AGENCY coverage. Quality Assurance. AGENCY shall develop, maintain and conduct an ongoing comprehensive assessment to evaluate the quality and appropriateness of music therapy services. This will be completed by chart review, interview and survey of patients served. PROVIDER shall cooperate with the AGENCY conduction of Quality Assurance and will facilitate the administration of such program in relation to purchased services.
