Responsibilities of the Agency a. The Agency shall periodically provide the Department’s Contract Manager, or designee, with a schedule of work to be accomplished under the terms of this Contract. Deviation from the established schedule shall be reported to, and coordinated with, the Department. b. If required, the Agency shall obtain licenses or permits for the work to be performed. The Agency shall provide supervision and guidance for projects that require a permit or which require technical assistance to complete the project. c. The Agency shall ensure that all projects utilizing inmates are authorized projects of the municipality, city, county, governmental Agency, or non-profit organization and that private contractors employed by the Agency do not use inmates as any part of their labor force. d. The Agency shall retain ownership of any vehicles or equipment provided by the Agency for the work squad(s). The Agency shall maintain its own inventory of transportation, tools, and equipment belonging to the Agency. e. The Agency shall provide vehicles for transportation of the work squad(s) and is responsible for the maintenance of said vehicle.
Responsibilities of the Agency. 3.1 The Agency shall contribute local funds equivalent to at least fifty percent (50%) of the Grant Funds towards the Project. 3.2 The Agency may contract with local non-profits to complete the Project or elements of the Project; provided, that the Port shall not, under any circumstance, disburse the Grant Funds to any of the Agency's contractors or subcontractors. 3.3 The Agency shall complete the Project by November 30, 2020.

Responsibilities of the Agency. A. On-site supervision should include a minimum of one hour per week face-to-face supervision, including consultation about the intern's actual workload. B. Interns are neither to replace staff nor to render services except as identified for educational value. C. The agency will arrange for a student orientation program for the purpose of familiarizing the student with the agency's philosophy, policies, and procedures. D. The agency will make available those educational experiences mutually agreed upon by the parties during the term of this agreement. Further, they will provide an opportunity for the intern to participate in the routine professional activities. These should include: administration, use of educational, occupational, and personal-social information; consultation with staff; referral of clients; and staff meetings. E. The student must complete 360 clock hours on-site. F. The designated site supervisor should: 1. Inform the intern about the policies and procedures governing the site. 2. Participate in the development of the intern's objectives for the Internship. 3. Arrange for professional working space and facilities for the intern's use. 4. Provide a minimum of one hour per week supervising the intern's work, including consultation about the intern. 5. Make provisions for the intern to attend agency's area and regional meetings, conferences, workshops, etc., which the site supervisor normally attends, if appropriate. 6. Contact the on-campus Internship Coordinator as needed. 7. Sign and date weekly logs completed by the Intern. 8. Complete the Midterm and Final Evaluations of the intern's performance on forms provided by the student after completion of half and all of the internship hours, respectively. All students assigned by University shall be required to complete Agency’s training (if required by Agency) and adhere to Agency’s rules, regulations, policies and procedures. Failure to do so shall be grounds for immediate removal from Agency’s premises and participation in the program at the Agency. The Agency may resolve any situation in favor of the patients’ welfare. Agency reserves the right to immediately remove any student deemed to be in violation of a facility policy, procedure, rule or regulation or who is determined to be a danger to the health or well-being of Agency employees or patients. The School agrees to abide by and require that its faculty and students abide by all applicable state and federal laws, rules and regulations...
Responsibilities of the Agency. 1. The Agency shall provide a rape crisis hotline staffed 24 hours a day, seven (7) days per week by certified victim advocates. 2. The Agency shall provide a mailing address for Inmate victims to send correspondence. 3. The Agency shall provide a certified victim advocate to respond in-person to requests for advocacy and accompaniment during sexual abuse forensic exams and investigatory interviews between 7 a.m. and 7 p.m., daily. When the Agency receives a request after 7 p.m., an advocate shall provide a response by phone, or in-person the following day after 7 a.m., if the need still exists. The advocate’s response time shall be no more than 150 minutes (2.5 hours), and he/she will provide an estimated time of arrival and maintain communication until arrival at the institution. 4. The Agency shall provide follow-up services and crisis intervention to Inmate victims of sexual assault, including ongoing phone and in-person counseling sessions, as resources allow. The Agency shall provide sessions Monday through Friday, between 9 a.m. and 4 p.m., with a maximum of three (3) sessions per Inmate. If additional sessions are needed, the advocate will notify the institution’s PREA Compliance Manager. 5. The Agency shall provide the Department with the advocate’s name responding to a forensic exam, investigatory interview, individual counseling, advocacy, and follow-up sessions. 6. The Agency shall maintain privileged communication with Inmate victims as required by State and federal law, and the Agency’s policies. 7. The Agency shall terminate the hotline call or individual service session(s) if an Inmate’s need for services is not primarily motivated by a desire to heal from sexual violence. 8. The Agency shall provide Inmates with referrals for treatment after release or upon transfer to another facility. 9. The Agency shall provide Inmates with information about reporting sexual abuse, including the correctional institution’s responsibility to investigate each report and to protect Inmates and staff who report from retaliation. 10. The Agency shall communicate any questions or concerns to the Agreement Manager. 11. The Agency shall ensure advocates receive background screenings in accordance with Section VII., C.,

Contact the on-campus supervisor as needed. 7. Complete a midterm and final evaluation of the intern's performance on forms provided by the university. X. Responsibilities of the Agency. A. The AGENCY agrees to complete an Internship Job Description or Student Learning Contract that outlines the duties and responsibilities of the internship to determine suitability for academic credit. B. The AGENCY will employ qualified personnel to provide students with adequate orientation, training, supervision, and evaluation of their duties and responsibilities in a safe workspace. C. The AGENCY is solely responsible for the selection of the student intern/s. D. The AGENCY agrees to notify the UNIVERSITY of any changes in staffing or in its services that will affect a student’s internship experience. E. The AGENCY will provide the student and the UNIVERSITY with written notifications of any requirements for the student to complete a background investigation, drug test, or physical examination prior to the start of the internship experience. F. The UNIVERSITY does not discriminate on the basis of race, color, gender, national or ethnic origin, age, religion, disability, marital status, veteran status, sexual orientation, or pregnancy in educational programs and activities as required by Title IX. Protection from retaliation is also covered under Title IX. As outlined in the Violence Against Women Act, the UNIVERSITY prohibits the offenses of domestic violence, dating violence, sexual assault, and stalking. If a student makes a report to the AGENCY, believing he or she has experienced or witnessed discrimination including sexual harassment, domestic violence, dating violence, sexual assault or stalking, the AGENCY must report such incidents to the Title IX Coordinator (000-000-0000, xxxxxxx@xxxxxxx.xxx, xxx.xxxxxxx.xxx/xxxxxxx). G. The AGENCY agrees to provide reasonable accommodations to an individual with a disability as identified by Accessibility and Disability Services. H. The AGENCY is aware that unpaid internships in the public sector and for “non-profit” charitable organizations where the intern volunteers without expectation of compensation are generally permissible by the US Department of Labor. Unpaid internships in “for-profit” private sector employers must comply with the following six criteria outlined in the Fair Labor Standards Act:

Maintain adequate numbers of certified EMS providers to provide both basic life support and advanced life support services. b. Ensure the OMD has a current roster of all EMS providers serving with the AGENCY, including certification level and any corrective or disciplinary action in force. f. Ensure the OMD has a current roster identifying the Captain, President, Training Officer, Infection Control Officers, and other line officers. g. Abide by defined rules established for clinical and preceptor programs. h. Enforce remedial or corrective measures for EMS personnel as rendered by the OMD. i. Provide a representative to the WVEMS Peer Review Committee (or other pre-hospital committee functioning as a peer review committee) for the area in which the agency operates. j. Recognize the role of the Western Virginia EMS Council as agent for the OMD and grant full access and rights to the OMD and WVEMS when acting on behalf of the OMD. This may include, but is not limited to, investigation of complaints, developing protocols, recommending corrective or remedial measures, and/or enforcement of the policies, procedures, protocols, and guidelines as established by the OMD. k. Name the OMD as an insured party on the AGENCY’S liability insurance, and name the AGENCY’S OMD as an officer of the AGENCY to ensure that the OMD is covered on the AGENCY’S directors and officers (D&O) liability insurance
