Responsibilities of Agency. If this Contract requires Agency to provide any Implementation resources, and Agency fails to provide the requisite quality or quantity of such resources, or fails to provide such resources in a timely manner for a period that does not exceed 15 (fifteen) Business Days, Contractor’s sole remedy is an extension of the applicable delivery dates corresponding to the delay. If Agency’s failure to provide such resources exceeds 15 (fifteen) Business Days, and Contractor can show to the reasonable satisfaction of Agency, that the Agency's failure has resulted in an unavoidable increase in the cost of the Services required for the Statement of Work then Contractor will be entitled to recover from Agency the reasonable amount of such increased costs. Contractor's right to delay applicable delivery dates or recover for increased costs may be exercised only if Contractor provides Agency with reasonable notice of Agency's failure and Contractor uses commercially reasonable efforts to perform notwithstanding Agency's failure to perform.

Responsibilities of Agency. Agency must designate a primary supervisor of the student(s) who agrees to be available for instruction during the Practical Experience. Agency will retain full responsibility for Agency’s operations and administration. Agency will provide any pre-placement information that needs to be shared with the student(s) and provide any necessary orientation to the student(s) within the first week of placement. Agency has the authority to request the withdrawal of any student from the Practical Experience for reasonable cause related to the need to maintain an acceptable level of service and/or business operations. Such request is to be in writing. Upon receipt, CMU will immediately comply with the request.
Responsibilities of Agency. The following are responsibilities of the Agency. The Agency accepts and agrees that it shall:
A. Make available suitable job(s) at its worksite(s) and in the respective occupation(s) described in the job description(s) attached and hereby made a part of this agreement, for those eligible CAREERSOURCE POLK participants who are selected and referred to the Agency by CAREERSOURCE XXXX.
X. Notify CAREERSOURCE XXXX immediately if any participant referred by CAREERSOURCE XXXX would be directly supervised by a member of that person’s immediate family for this is a violation of the terms of the agreement.
C. Assign meaningful work assignments to participants during the entire time they are at the worksite in accordance with the approved job description(s). Duties other than those in the approved job description(s) must be pre-determined and mutually agreed upon by the Agency and CAREERSOURCE XXXX.
D. Direct and supervise participants’ work activities and training in accordance with their job description(s).
X. Xxxxxx that it will have supervisory personnel who will act as worksite supervisors for each of the Agency’s worksites to provide for continuous on- site supervision and training of participants at a ratio of participant(s) to staff of no more than 1:5.
X. Xxxxxxxxx and train their worksite supervisory personnel including any alternate staff directly responsible for the supervision of participants as to the Agency's responsibilities and obligations under this Agreement.
X. Notify CAREERSOURCE XXXX immediately of any temporary and/or ongoing changes in the designation of personnel who will be supervising the participant(s).
X. Xx accountable for maintaining CAREERSOURCE XXXX participant’s timesheets through the online platform provided. Ensure that timesheets are accurately completed and approved in a timely manner in accordance with CAREERSOURCE POLK payroll procedures.
I. Require participants' conformance with the Agency's Personnel Rules of Conduct such as dress code, parking, etc.
J. Ensure that all sites where participants will be assigned are sanitary and safe environments in accordance with the CDC safety guidelines and health and safety standards established by State and Federal law.
K. Ensure that all required safety supplies and equipment are used in the proper manner of the intended use and in accordance with applicable laws, including child labor laws.

A. Secure funding from the National Park Service, as provided in the LWCF guidelines and as are made available by the Federal Government, for the Project and pay Subrecipient in accordance with Section 4 above.
B. Pay Subrecipient in accordance with Section 4 above.
C. Retain all records related to the Project as set forth in this Grant Agreement and all applicable LWCF program rules and regulations.
Responsibilities of Agency. Agency shall:
A. Provide appropriate services within its capability and capacity to clients referred by CCDPH to Agency.
B. Not refer clients to CCDPH for primary care services.
C. Only refer clients to CCDPH who are suburban Cook County residents.
D. Provide evidence to CCDPH, with the signed copy of this Agreement, of applicable accreditation and/or licensure for all Agency services provided and any other documentation requested by CCDPH (Attached hereto as Exhibit A).
