Annual Survey. 1. Collect the MAT Programs Opioid Treatment Services (OTS) annual survey. 2. Use the System Agency approved client satisfaction OTS annual survey template for collecting information from clients who have received MAT Program services. 3. Develop and coordinate a process for collecting client survey data. 4. Submit results of client survey in an annual report to System Agency.
Annual SurveyThe Provider shall support and participate in the Annual Adult Health and Well-Being Survey in accordance with the protocols developed by the Department.
Annual Survey. Company shall complete Lender’s annual compliance survey within 30 days of receipt of such compliance survey from Lender by Company for the applicable year. For each time Company fails to submit an annual compliance survey within 30 days after the date on which it is due, Company will automatically, and without notice from Lender, be assessed a $500 service charge and each such service charge will bear interest at the rate set forth in Section 12.7 from the date such service charge arises (i.e., 30 days after delivery of the compliance survey by Lender (or its representatives) to Company).

Annual SurveyThe Provider is required to support and participate in the Annual Mental Health Data Infrastructure Consumer and Family Satisfaction Survey Project in accordance with the protocols developed by the Department’s Office of Quality Improvement. The surveys are administered directly by the Department. Provider agencies will be required to assist in notifying clients about the survey prior to administration, encouraging client participation and addressing client questions regarding surveys.
Annual Survey. All bargaining unit employees will be allowed 30 minutes of official time each year to participate in a representational survey conducted by the Union on a nationwide basis. Annual SurveyThe principal is to conduct an annual survey of the staff to determine the preference, in order of priority, of each person’s grade level and/or department assignment. This survey is to be completed by March 15 each year. The teacher is to indicate semester units and length of service in each of the designated areas of teaching preference. All preferences will be taken into consideration when filling vacant positions should a domino effect in positions occur.
