Annual Survey. 1. Collect the MAT Programs Opioid Treatment Services (OTS) annual survey.
2. Use the System Agency approved client satisfaction OTS annual survey template for collecting information from clients who have received MAT Program services.
3. Develop and coordinate a process for collecting client survey data.
4. Submit results of client survey in an annual report to System Agency.
Annual Survey. Company shall complete Lender’s annual compliance survey within 30 days of receipt of such compliance survey from Lender by Company for the applicable year. For each time Company fails to submit an annual compliance survey within 30 days after the date on which it is due, Company will automatically, and without notice from Lender, be assessed a $500 service charge and each such service charge will bear interest at the rate set forth in Section 12.7 from the date such service charge arises (i.e., 30 days after delivery of the compliance survey by Lender (or its representatives) to Company).
Annual Survey. The Provider shall support and participate in the Annual Adult Health and Well-Being Survey in accordance with the protocols developed by the Department.
Annual Survey. A year-end survey of matters of concern shall be conducted annually in each building. This shall be devised by the Evaluation Committee.
Annual Survey. The Provider is required to support and participate in the Annual Mental Health Data Infrastructure Consumer and Family Satisfaction Survey Project in accordance with the protocols developed by the Department’s Office of Quality Improvement. The surveys are administered directly by the Department. Provider agencies will be required to assist in notifying clients about the survey prior to administration, encouraging client participation and addressing client questions regarding surveys.
Annual Survey. 4.2.1. After annual survey, the following information shall be kept on file in a format which can be accessed and downloaded electronically. ▪ Confirmation that the classification certificate is retained, ▪ Confirmation that survey, inspections and endorsements have been carried out as delegated in Annex VI to the Agreement. The confirmation shall include information about level and amount of inspections and surveys that have been carried out.
4.2.2. Where deviations from the requirements established in applicable Norwegian regulations have been identified, a list containing the deviations shall be submitted to the NMA.
Annual Survey. All bargaining unit employees will be allowed 30 minutes of official time each year to participate in a representational survey conducted by the Union on a nationwide basis. The Union will consult with PPT/PMO on the content of the survey before releasing it to the employees. If Management seeks to survey bargaining unit employees, it will first consult with the Union (including discussing the content of the survey before releasing it to the employees). Any such survey will provide employees with sufficient time to complete the survey.
Annual Survey. The Department will conduct an annual survey of employees in the bargaining unit to determine their interest in part-time employment. The survey form will include information on the rights and benefits of part-time employment. The Department will consult with the Union on the content of the survey form and share the results of the completed survey with the Union.
Annual Survey. As the exclusive representative of all employees in the bargaining unit, the Union is entitled to be the sole spokesperson for this group of employees. Employees will be permitted thirty (30) minutes of official time annually to participate in a Union survey regarding representational issues. For this survey, the Union will consult with Management on the content of the survey prior to releasing it to the employees.
Annual Survey. Borrower shall complete and delivery to Lender an annual survey, in a form acceptable to Lender, by the deadline set by Lender. Borrower shall be given at least thirty (30) days notice of such deadline.
