Application of Rates and Charges. (A) A service record retrieval charge applies to each customer service record electronically delivered to the reseller.
(B) A reseller may request any number of electronic customer service records, but will only be charged for the number of electronic customer service records successfully transmitted to the reseller.

The monthly rate includes a flat rate one-party business line or key trunk with Touch tone, flat rate EAS, expanded local calling (where available), features, unlimited long distance provided by CenturyLink Communications, Inc., and the following fees and surcharges: Interstate Subscriber Line Charge, Intrastate Subscriber Line Charge (where applicable), Access Recovery Charge, , and certain additional state-specific charges as follows:
B. Simply Unlimited Business includes a rate stability plan which allows customers to pay a fixed monthly rate that is not subject to rate changes over a fixed period. Customers will pay the tariffed rate in effect as of the service establishment date for the first twelve months after service is established. On the service establishment anniversary date, the customer’s monthly rate will increase to the lesser of the then-prevailing tariffed rate or a rate that is $5.00 higher than the rate in effect on the service establishment date. Thereafter, on each subsequent anniversary date, the monthly rate may increase by no more than $5.00, not to exceed the then-prevailing tariffed rate.
C. Simply Unlimited Business lines subsequently added to the customer’s account will be charged the monthly tariffed rate in effect as of the date the new Simply Unlimited Business line(s) is/are added to the account. The same rate stability provisions apply to subsequently added lines, based on the service establishment date for those lines.
D. Customers will be notified at least thirty days prior to any service anniversary date rate increase. Such notice will include the actual amount by which customer’s Simply Unlimited Business will increase.
E. Monthly rates for Simply Unlimited Business are located in the the applicable Tariff, guidebook, schedule and/or Local Terms of Service which may be found at xxx.XxxxxxxXxxx.xxx/xxxxxxx for each CenturyLink company listed in the table on Page

Application of Rates and Charges. 16.3.1 Beginning on and after the Effective Date of this agreement, the Parties agree that the rates and charges for Collocation shall be as set forth in this Appendix and in the Pricing Schedule applicable to collocation (“Collocation Rates”). The Parties agree that the Collocation Rates shall apply, on a prospective basis only, beginning on the Effective Date of this agreement, to all existing CLEC collocation arrangements, including those established before the Effective Date of this agreement. Because the Collocation Rates will apply on a prospective basis only, neither Party shall have a right to retroactive application of the Collocation Rates to any time period before the Effective Date, and there shall be no retroactive right of true-up for any time period before the Effective Date.

1. When a tape or cartridge is provided in place of data transmission, data transmission charges are not applicable and a per tape or cartridge charge will apply. If a tape or cartridge is requested in addition to data transmission, both the per tape or cartridge charge and the per data transmission charge applies.
Application of Rates and Charges. Petroleum accepted for transportation shall be subject to the rates and charges in effect on the date of receipt of such Petroleum by Carrier. Transportation and all other lawful charges will be collected on the basis of net quantities of Petroleum delivered. All net quantities will be determined in the manner provided in Item No. 40.

Rates and service quotations are estimates and will be based upon the information provided by you, but final rates and service may vary based upon the shipment actually tendered. Charges will be assessed at the rates effective the day the shipment is accepted by us. Due to potential delays beyond the control of SFC in the discovery of errors SFC reserves the right to retroactively assess or adjust fees.

(5) Telecommunications Relay Service (TRS) and

4. Access Line Discount - Applies to: - IntelliLinQ - PRI Channel Arrangement - Enhanced FlexGro w (Trunk Capacity) Customers will receive a fixed dollar discount, as set forth in C. Rates following, on eligible access lines. Access lines included in customer specific pricing agreements are ineligible for the access line discount. However, cus tomers with customer specific pricing agreements are eligible to subscribe to and receive the benefits of all the other Corporate Rewards components. Customers who exceed Tier 4 maximum monthly aggregated usage volume will not receive access line discounts . Customers who do not exceed Tier 1 maximum monthly usage volume will also not receive access line discounts. For customers subscribing to a two or three - year Corporate Rewards service Agreement, access line discounts will apply only to those access lin es rated on the shortest term period available. Customers must convert their existing access lines to the shortest - term period available in order to be eligible for the access line discounts.
