The application for leave must state the: Kind of leave being requested Recommended number and type of staff required to escort a consumer on escorted leave, as indicated in the consumer’s Security Category and Leave Entitlement (SCALE). Note: the final decision will be made by the Leave Panel (see Section 1.5). Risk Assessment and Risk Management Plan (excluding External Therapeutic Leave and Special Circumstances Leave Plan where the risk assessment and management plan will form part of the attached leave plans.)   • Application for Leave Any request for a leave of absence shall be submitted in writing by the employee to the Employer or designee. The request shall state the reason the leave of absence is being requested and the length of time off the employee desires. Authorization for a leave of absence shall be furnished to the employee by the Employer or designee, and it shall be in writing.

  • Permission to Leave Work The Employer agrees that stewards shall not be hindered, coerced, restrained or interfered with in any way in the performance of their duties, while investigating disputes and presenting adjustments as provided in this Article. The Union recognizes that each xxxxxxx is employed by the Employer and that they will not leave their work during working hours except to perform their duties under this Agreement. Therefore, no xxxxxxx shall leave their work without permission of their supervisor, which will not be unreasonably withheld.
