Background Verifications. College shall provide Hospital with a description of its background investigation processes, shall attest to Hospital College has completed a background check for each Program Participant in the form attached hereto as Exhibit C, and disclosed to Hospital any positive findings for a Program Participant to the extent permitted by law and shall provide Hospital or its designee access to the background screening results upon reasonable notice. A background check will be considered “completed” if it includes, at a minimum, all of the following elements: (1) 7 year criminal background check in current and previous counties of residence and employment, (2) confirmation that the Program Participant is not listed as sexual offender and, if requested by Hospital, in any child abuse registry (3) evidence that the Program Participant is eligible to participate in all federal and state health programs and verification that the Program Participant is not on the OIG or GSA exclusion list and (4) any other element required by Hospital to meet state or federal law requirements. Hospital shall have the right to require the withdrawal of any Program Participant in the event that Program Participant fails to meet the standards established by Hospital for acceptable background.
Background Verifications. To inform Students, prior to coming to Agency, that they must meet Agency’s requirements for background screens. Student shall provide a copy of the completed background check to Agency upon request prior to the commencement of any practicum experience in the Agency. The Agency reserves the right to allow students to participate in the Program based on background screen results.
Background Verifications. School acknowledges each Program Participant shall be required to submit to a complete background check as a condition of participation in the Program. School shall provide a copy of the completed background check to Hospital prior to the commencement of any Program Participant’s participation in the Program. A background check will be considered “completed” if it includes, at a minimum, all of the following elements: (1) 7 year criminal background check in current and previous counties of residence and employment, (2) confirmation that the Program Participant is not listed as sexual offender and, if requested by Hospital, in any child abuse registry (3) evidence that the Program Participant is eligible to participate in all federal and state health programs and verification that the Program Participant is not on the OIG or XXX exclusion list and (4) any other element required by Hospital to meet state law requirements. The background check must be no more than 12 months old as of the first day of the clinical with Participating Institution. To the extent permitted by law, School shall provide Participating Institution with applicable documentation certifying that a Student has completed the applicable background check. Background checks will include, at a minimum, all of the following elements: (1) 7 year criminal background check in current and previous counties of residence and employment, (2) a check of the national sex offender registry and (3) evidence that the Student is not ineligible to participate in all federal and state health programs and verification that the Student is not on the OIG or GSA exclusion list.
Background Verifications. CareRev acknowledges that each CareRev Professional shall be required to submit to a complete background check. CareRev shall conduct each background check and provide Facility with a description of its background investigation processes, shall attest to Facility that CareRev has completed a background check for each CareRev Professional, and disclose to Facility any positive findings for a CareRev Professional to the extent permitted by law. A background check will be considered “completed” if it includes, at a minimum, all of the following elements: (1) 7 year criminal background check in current and previous counties of residence and employment; (2) confirmation that the CareRev Professional is not listed as sexual offender and, if requested by Facility, in any child abuse registry; (3) evidence that the CareRev Professional is eligible to participate in all federal and state health programs and verification that the CareRev Professional is not on the OIG, XXX, or GSA exclusion list, the National Practitioner Data Bank, or any similar federal or state database indicating that such individual is ineligible to perform the Services under this Agreement; (ii) excluded, debarred, or otherwise ineligible to participate in any of the federal health care programs as defined in 42 U.S.C. § 1320a-7b(f) (the “Federal Health Care Programs”); (iii) convicted of a criminal offense related to the provision of health care items or services but have not yet been excluded, debarred, or otherwise declared ineligible to participate in the Federal Health Care Programs; (iv) under investigation or otherwise aware of any circumstances which may result in Agency or any of the Agency personnel being excluded from participation Medicare, Medicaid or any other state of federal healthcare program; and (4) any other element required by Facility to meet state law requirements. Facility shall have the right to require the withdrawal of any CareRev Professional in the event that CareRev Professional fails to meet the standards established by Facility for acceptable background.

If required by applicable accreditation organizations, state or federal law, or by Site, and at Site’s request, School shall conduct a background check on each Student assigned to the Experience, prior to participation at the Site. To the extent permitted by law, School shall provide Site with applicable documentation certifying that a Participant has completed the applicable background check. Background check will include, at a minimum, all of the following elements: (1) 7 year criminal background check in current and previous counties of residence and employment, (2) a check of the national sex offender registry and (3) evidence that the Student is not ineligible to participate in all federal and state health programs and verification that the Student is not on the OIG or GSA exclusion list. The School will not release background check information to Site without a signed release from the Student that is the subject of the check.
