Benefit Premiums. a) (i) Subject to the Resident Physician or legal designate providing written confirmation to AHS of application for long term disability (LTD) benefits, the supplementary health and dental plan premiums, inclusive of the Resident Physician’s twenty-five (25%) share, shall be waived.
