Evaluation File. Each Member of the Bargaining Unit shall be entitled, upon request, to a copy of his/her current evaluation report. Each Member of the Bargaining Unit shall have the right to challenge an evaluation report which contains or is based on erroneous information or data through the grievance procedure as herein provided. If a correction of an evaluation report is made, the Member of the Bargaining Unit shall be entitled to a statement in writing that the correction has been made and that the correction appears in his/her personnel file.
Evaluation File. Faculty members must refer to 6C13-6.008 Personnel Records and Limited-Access Records regarding access to, and disclosure of, performance evaluations and other faculty evaluative information.
Evaluation File. An evaluation file shall be maintained for each tenured, probationary, term, and full-time temporary Faculty Member. The file shall be located in the departmental office. The following materials shall be included in evaluation files:
Evaluation File. Each employee shall have the right to review evaluations contained in his/her evaluation file and to obtain materials that are placed in the employee’s personnel file and such employee responses shall become a part of said file. Any complaints directed toward an employee which are placed in his/her file shall be called to the employee’s attention in writing.
Evaluation File. (A) Supervisors may maintain active evaluation files which contain materials used for purposes of evaluating employees.
(B) Employees will have access to review their evaluation files upon reasonable notice to their supervisor.
Evaluation File a. The evaluation file shall be maintained in the office of the educator’s principal/supervisor during the academic year. The evaluation files for prior years shall be maintained in the Human Resources Office.
b. The evaluation file may contain the following written documents: Pre-evaluation Conference(s); First Observation; Second Observation; Supplemental Observation(s); Post-observation Conference and the Final Evaluation(s) of the current and previous school year.
c. The evaluation file shall not contain any other written material.
Evaluation File. 8-5-1 A file of performance reviews shall be maintained in a locked file in the building of each employee. Only one file shall be maintained for each traveling employee. The file shall include Teacher Target Appraisal, Classroom Visitation Record and Summative Evaluation Form, written appraisals by supervisors and other appropriate materials such as notes, letters and personal comments. All such information shall be brought to the attention of the employee for review and possible response. Correspondence from supervisors that is to be placed in the evaluation file shall be so noted. The Classroom Visitation Record shall be made available to the observed staff member within 15 school days for non-tenured staff and 20 school days for tenured staff from the date of the observation.
Evaluation File. Faculty members shall refer to 6C13-6.008 Personnel Records and Limited- Access Records regarding access to performance evaluations. All employees may provide a written response and/or comments regarding their evaluation and have it added to the evaluation file within sixty (60) days of the receipt of the evaluation. All written material used to produce a performance evaluation shall be included in the evaluation file. ARTICLE 9
Evaluation File. Employees shall have the right to review and copy formal evaluations and written complaints and responses contained in their official evaluation file.
Evaluation File. 1. The employee shall have the right to respond to all materials contained in the evaluation file and to any materials to be placed in said file in the future. Employee responses shall become a part of the file.
2. All evaluative materials including all reports of evaluations conducted by an employee’s supervisor shall be kept in the evaluation file.
3. Each employee shall have the right at any time to review and reproduce the contents of the evaluation file. A representative of the association, at the employee’s request, may accompany the employee in this review.
4. The Board or its administrative representatives, including principals, shall make available to employees any files containing evaluative material.
