BENEFITS SCHEDULE a. The plan shall provide for the payment of one hundred percent (100%) of the usual, customary, and reasonable charges for preventative and diagnostic services such as: 1) Routine oral examinations twice every twelve (12) months unless medically necessary; 2) Teeth cleaning twice every twelve (12) months; 3) Fluoride treatments and dental sealants twice every twelve (12) months; 4) Emergency treatments; space maintainers; diagnostic x- rays, tests, and laboratory examinations. b. The plan shall provide for the payment of eighty-five percent (85%) of the usual, customary, and reasonable charges for basic restorative services such as: 1) Fillings (amalgams, silicate or acrylic); 2) Root canal therapy; 3) Treatment of gum disease; 4) repair of bridgework and dentures; 5) extractions and oral surgery; and

BENEFITS SCHEDULE. This Benefits Schedule is attached to and is a part of the Settlement Agreement between Xxxxxx X. Xxxxxxx (the "Employee") and Illinova Corporation (the "Company") dated November 29, 1999, and sets forth certain rights to compensation, benefits, and payments under the Retention Agreement between the Employee and the Company dated January 18, 1999 (the "Retention Agreement"). The compensation, benefits, and payments under the Settlement Agreement (including this Benefits Schedule) shall be in lieu of any compensation, benefits, payments, or rights that would otherwise be provided under section 1 of the Retention Agreement in the absence of this Settlement Agreement.
BENEFITS SCHEDULE. On an annual basis, the availability, frequency, and amount of benefits depend on the amount of client fees ("Total Earned") generated by Psychotherapist and by CCG’s financial status. It is decided each year, based on CCG's financial state, whether each of the following benefits are offered for a calendar year and at what levels. Total Earned Per Month Category 1: $0.00 - $2,999.00 Definition: Generates $2,999.00 or less per month of revenue from client fees ("Total Earned"). Status: Psychotherapist is a W-2 employee and is classified as a non-exempt, regular, psychotherapist employee with the benefits connected to that classification as specified herein and in the Employee Handbook. Health Insurance: Psychotherapist has the option to enroll on their own in any health insurance plan, but is not required to do so. If Psychotherapist elects to enroll on their own in a health insurance plan, Psychotherapist is responsible for the premium payments to the insurance company. CCG will issue a monthly check to the Psychotherapist for the plan cost and will deduct the amount as a pre-tax deduction from Psychotherapist's monthly salary check. This benefit is not intended to reimburse the Psychotherapist for any health insurance premium that is already being paid by another organization or entity. It is only intended for any out-of-pocket health insurance premium expense paid directly by the Psychotherapist.
BENEFITS SCHEDULE. The parties have agreed that travel privileges will be by way of expense claim and reimbursement.
