Children Served. Afterschool: During the academic school year, BGCGNH offers afterschool programming at both our Columbus Clubhouse located at 000 Xxxxxxxx Xxxxxx xx Xxx Xxxxx, and on-site at partnering schools. Our clubhouse serves up to 175 youth daily, over 99% of which are New Haven Public School students. We have the capacity to provide afterschool programming at an additional 10 school sites, allowing afterschool programming to be accessible to more New Haven Public School students. Summer Camp: BGCGNH’s Summer Camp program is offered at our Columbus Clubhouse, serving up to 175 campers daily. Over 99% of those attending are New Haven Public School students. Spring Break Camp and School Closures: When schools out, we’re in! BGCGNH provides programming during most of the days that schools are closed for holidays, in addition to Spring Break.

Children Served. 2552.81 What type of children are xxx- gible to be served? § 2552.82 Under what circumstances may a Xxxxxx Grandparent continue to serve an individual beyond his or her 21st birthday? (a) Only when a Xxxxxx Grandparent has been assigned to, and has developed a relationship with, a child with a dis- ability, that assignment may continue beyond the individual’s 21st birthday, provided that: (1) Such individual was receiving such services prior to attaining the chronological age of 21, and the con- tinuation of service is in the best inter- est of the individual; and (2) The sponsor determines that it is in the best interest of both the Xxxxxx Grandparent and the individual for the assignment to continue. Such a deter- mination will be made through mutual agreement by all parties involved in the provision of services to the indi- vidual served. (c) The sponsor may terminate serv- ice to a child with a disability over age 21, if it determines that such service is no longer in the best interest of either the Xxxxxx Grandparent or the indi- vidual served. Children Served. Provider will work with each school listed above to offer individual and group therapy services to eligible students, and will respond to any crisis or behavioral issues that may arise at the school, upon request of the principal or principal’s designee. Participating children must be determined eligible for services based on assessment by a Provider qualified mental health professional staff member. A parent or guardian must give written consent and must complete all necessary documentation for their child to participate in the program. Eligibility for services will be determined by Provider, based upon payers’ eligibility criteria.
Children Served. Emergency shelter care service will be provided for children who have been referred by the Mendocino County Health and Human Services Agency (HHSA) Family & Children’s Services (FCS), and meet the criteria in a or b below, and c, d or e below: a. Have been removed from parental custody because of allegations of abuse, neglect or abandonment; or b. Are dependents of the Juvenile Court under Welfare & Institutions Code Section 300 and must be moved immediately from an existing xxxxxx or group home placement; and c. Are transitioning from a Xxxxx 00 or higher group home and require temporary shelter pending the completion of an assessment and selection of a suitable xxxxxx care placement; or d. Are at risk of placement in a Level 10 or higher group home; or e. Have displayed behavior which precludes placement in other emergency shelter care facilities. Examples of such behavior include: i. Being under the influence of alcohol or illicit drugs ii. Previous runaway from a shelter home iii. History of hurting self while in shelter care
