COOPERATIVE WORK EXPERIENCE PROGRAM. ‌ 1. Attend an orientation session and program meetings. 2. Initiate, maintain, complete and submit all appropriate forms, student records, and assigned student grade using the standard Cooperative Work Experience Syllabus to the Cooperative Work Experience Office by the designated deadline. 3. Meet with student at campus site for the first consultation session to assist in development of measurable learning objectives. 4. Conduct first job site visit and meet with student’s supervisor. Obtain agreement with supervisor and student which results in sign-off for objectives on appropriate form. 5. Meet with student for midterm consultation session to discuss progress on objective and hours. 6. Towards the end of course, conduct a second a second job site visit to obtain a completed self-evaluation from student, obtain the supervisor’s evaluation of student’s completion of objectives and assign grade(s) to student using the standard Cooperative Work Experience Syllabus.

EXPECTATIONS: FACULTY Faculty who participate in the Cooperative Work Experience program will be expected to do the following: 1. attend an orientation session (only required once) 2. maintain appropriate student records 3. initiate and complete all of the specified components of the program PROGRAM COMPONENTS: 1. Meet with student at campus site to assist in development of objectives. Such objectives must be written in a measurable format. 2. Visit job site and meet with student’s supervisor. Obtain agreement with supervisor and student which results in sign-off for objectives on appropriate form. 3. At end of course, obtain completed self-evaluation from student. Visit supervisor at job site and obtain supervisor’s evaluation of student’s completion of objectives. 4. Meet with student to finalize evaluation, discuss self-evaluation, determine if objectives were met. Assign grade to student, using appropriate format.
