Credit Card Policy. I understand that TCFAP requires a credit card on file. My insurance requires payment at time of service for all deductibles, co-pays, and coinsurance. As a courtesy, TCFAP will keep my credit card on file and process payment when the EOB is received from my insurance. Otherwise all charges must be settled at the time of service. NO EXCEPTIONS to this policy. I understand it is my responsibility to checkout at the time of visit if I refuse to leave my credit card on file. If I do not settle at check-out and do not have a credit card on file, my account will be considered past due and a $25 fee will be incurred. I understand it is my responsibility to update my credit card on file when it expires or is replaced. I understand that having a credit card on file ensures payment and allows me additional time to settle my account and that credit cards are stored electronically and are encrypted. State and federally funded insured are not required to leave a credit card on file as there is no balance billing. I understand that any patient balance under $250 will be automatically collected using the credit card on file. I understand any patient balance over $250 will be collected after 14 days if the balance has not been disputed. I understand that if I am a non-TCFAP patient/visitor or using one of TCFAP additional services beyond primary care (Behavior Health, Lactation Services, Travel Medicine, Nutrition and/or any future services that might be offered) I am required to place my credit card on file. Credit Card Policy. By signing herewith, I acknowledge and agree to the charges described hereon. Credit Card Payment Terms: Payment in full is to be made when billed or in extended payments in accordance with the standard policy and Terms of Rental of Rand & London (Rental Agent) and their binding Lease Agreement. By signing herewith and / or by supplying their Credit Card information to Rand & London whether telephonically, by facsimile, Email, or by other media, Lessee / Cardholder confirms that they are the authorized signatory and legal user of the referenced Credit Card. Lessee / Cardholder further authorizes Rand & London, to debit the referenced credit card and they agree to be bound by the terms and conditions of Rand & London's Lease Agreement and Credit Card Use and Acceptance Policy. To induce Rand & London to accept Lessee / Cardholders credit card payment Lessee / Cardholder agrees and accepts that there are no refunds for cancellations within 90 days of arrival, early departure or no shows and if Lessee / Cardholder has declined travel insurance then they agree to be liable for any loss associated with delays in arrival, cancellations, early departure and no shows as stated herewith and as per Rand & London's Lease Agreement. This Credit Card Authorization and the Lease are binding contracts. By signing and / or supplying Rand and London with their credit card information, Lessee / Cardholder agrees to the terms and conditions herewith and to the terms and conditions of rental of Rand & London's Lease Agreement. Failure to separately sign either document does not ameliorate or cancel the other or invalidate the mutual obligations to each other, or negate either in any way. Additional copies of the Lease will be sent to Lessee / Cardholder upon request. No refunds are made for cancellations within 90 days of arrival date. No refunds are made for early departure or partial stay as per the Lease. Cancellations must be made in writing. However, you may not use your card to initiate any type of gambling transaction. If you wish to pay for goods or services over the Internet, you may be required to provide card number security information before you will be permitted to complete the transaction. You agree that you will not use your card for any transaction that is illegal under applicable federal, state, or local law. Funds to cover your card purchases will be deducted from your share draft account. For ATM and one-time debit card transactions, you must consent to the Credit Union’s overdraft protection plan in order for the transaction amount to be covered under the plan. Without your consent, the Credit Union may not authorize and pay an overdraft resulting from these types of transactions. Services and fees for overdrafts are shown in the document the Credit Union uses to capture the member’s opt-in choice for overdraft protection and the Schedule of Fees and Charges.

  • Special Rule for Credit Card Purchases If you have a problem with the quality of property or services that you purchased with a credit card, and you have tried in good faith to correct the problem with the merchant, you may have the right not to pay the remaining amount due on the property or services. There are two limitations on this right:

  • Credit Checks 9.1 The Reseller agrees that: (a) When the Reseller applies to Voip-Unlimited to open an account, Voip-Unlimited may check the following records about the Reseller and its business partners: (i) Voip-Unlimited’s own; (ii) Personal and business records at credit reference agencies (CRAs). When CRAs receive a search from Voip-Unlimited they will place a search footprint on the Reseller’s business credit file that may be seen by other parties including lenders. They supply to Voip-Unlimited both public (including the electoral register) and shared credit and fraud prevention information; and (iii) those at fraud prevention agencies (FPAs); (b) For directors, Voip-Unlimited may seek confirmation, from credit reference agencies, that the residential address provide is the same as that shown on the restricted register of directors' usual addresses at Companies House; (c) Voip-Unlimited may make checks such as assessing the Reseller’s application and verifying identities to prevent and detect crime and money laundering. Voip-Unlimited may also make periodic searches at CRAs and FPAs to manage the Reseller’s account with it; (d) Information on applications will be sent to CRAs and will be recorded by them. Including information on the Reseller’s business and its proprietors and CRAs may create a record of the name and address of the Reseller’s business and its proprietors if there is not one already. Where the Reseller borrows from us, we will give details of the Reseller’s accounts and how the Reseller manages it/them to CRAs; (e) If the Reseller borrows and does not repay in full and on time, CRAs will record the outstanding debt. This information may be supplied to other organisations by CRAs and FPAs to perform similar checks and to trace the Reseller’s whereabouts and recover debts that the Reseller owes. Records remain on file for 6 years after they are closed, whether settled by the Reseller or defaulted; (f) If the Reseller gives Voip-Unlimited false or inaccurate information and Voip-Unlimited suspect or identifies fraud Voip-Unlimited will record this and may also pass this information to FPAs and other organisations involved in crime and fraud prevention; (g) If the Reseller has borrowed from Voip-Unlimited and does not make payments that it owes Voip- Unlimited, Voip-Unlimited may trace the Reseller’s whereabouts and recover debts; (h) Voip-Unlimited and other organisations may access and use from other countries the information recorded by fraud prevention agencies; and (i) The Reseller’s data may also be used for other purposes for which the Reseller gives specific permission or, in very limited circumstances, when required by law or where permitted under the terms of the Data Protection Xxx 0000.
