Deliverable and Reporting Requirements. ‌ The following table contains deliverables and reports required for OASIS. Task order deliverables and reporting will be specified in the task order. The Government does not waive its right to request other deliverables or reports not specifically listed in the table below. Deliverables or reports are required until the final task order is closed-out for each Contractor. If a deliverable is due on a calendar day that falls on a weekend day or a Government holiday, the deliverable or report is due the following business day. Deliverable and Reporting Requirements. A. Grantee shall use System Agency system to submit all deliverables to Clinical Management Behavioral Health Services (CMBHS) and/or any alternative method required by System Agency. Grantee is required to maintain access to required systems or platforms for the term of this Grant Agreement. Grantees are required to submit data, reports, performance measures, and any other requested information into data systems designated by System Agency. B. Grantee shall submit annual Grant Agreement Closeout documentation, for each awarded program referenced in Section V. of the Signature Page, Budget and Indirect Cost Rate. This is required each grant year, and a final Grant Agreement closeout documentation, including but not limited to final annual reports and invoices, are due in accordance with the due dates documented in Attachment A- 4. C. Grantee will report the performance measures for the previous month’s activities in CMBHS by the 15th of the current month, for each awarded program referenced in Section V of the Signature Page, Budget and Indirect Cost Rate. D. Xxxxxxx’s duty to submit documents will survive the termination or expiration of this Grant Agreement.

Grantee shall report in CMBHS information related to the Curriculum Outcome Measures for each group cycle provided throughout the fiscal year. Grantee shall submit monthly invoices in Clinical Management for Behavioral Health Services (CMBHS) for each awarded program. 6. Grantee shall submit annual Contract Closeout documentation for each awarded program. This documentation is required each fiscal year. Final Contract Closeout is due October 15. 7. Grantee shall submit a CMBHS Security Attestation Form twice a fiscal year. 8. Grantee will report the performance measures for the previous month’s activities in CMBHS by the 15th of the current month for each awarded program. 9. Deliverable and Reporting Requirements. In the event the FAS Sales Reporting System (TDR) is not operational, deliverable and reporting requirements designated for input into the FAS Sales Reporting System (TDR) shall be provided as directed by the BMO SB CO within the stated timeframes. The BMO SB CO or an authorized representative shall have the right to examine and audit all supporting records and materials, regardless of whether such items are in written form, in the form of computer data, or in any other form, for the purpose of enforcing all deliverables and compliances herein. Acceptable Accounting Systems are mandatory for all Contractors on BMO SB. Refer to Section H.9 for information on Acceptable Accounting Systems. All Systems, Compliances, and Certifications must be maintained at the Contractors’ current level at time of award or higher throughout the period of performance of BMO SB.
