b) An employee, while on temporary layoff or unpaid leave of absence of up to six (6) months may choose to continue to maintain their coverage (where a third party benefits provider agreement permits) in the Medical, Dental, Extended Health, Group Life Insurance, Accidental Death and Dismemberment and Long Term Disability benefit plans by paying one hundred percent (100%) of the costs of the premiums the first day of the month following that in which the layoff or leave occurs.
