A pay direct drug card will be issued. Employees are required to pay the dispensing fee. o Semi-private hospital. o Medical equipment and supplies o Paramedical practitioners, limited to $200 per practitioner per year, including acupuncturist, chiropractor, speech therapist, podiatrist, clinical psychologist, physiotherapist, dietician, occupational therapist, osteopath, naturopath, audiologist, social worker and massage therapist. o Orthotics, up to $200 per year. o Orthopaedic shoes, limited to a maximum of 1 pair per year, for up to $150. o Hearing Aids, up to $500 every 5 years. o Private duty nursing up to $10,000 per year. o Out of country emergency medical expenses up to a $5,000,000 lifetime maximum, including a travel assistance card. The maximum trip duration is 60 days. o Extended Healthcare benefits cease at the earlier of termination of employment, retirement or age 70.

  • Extended Health Care Plan ‌ The Employer shall pay the monthly premium for regular employees entitled to coverage under a mutually acceptable extended health care plan.

  • Extended Health Care Benefits The City will provide for all employees by contract through an insurer selected by the City an Extended Health Care Plan which will provide extended health care benefits. The City shall pay one hundred per cent (100%) of the premiums, which will include any premiums payable under The Health Insurance Act, R.S.O. 1990, as amended.

  • Extended Health Care The Hospital shall contribute on behalf of each eligible employee seventy-five percent (75%) of the billed premium under the Extended Health Care Plan (Liberty Health $15-25 deductible plan including hearing aids with a maximum of $300.00 per person and vision care with a maximum of $150.00 every 24 months per person, or its equivalent) provided the balance of the monthly premium is paid by employees through payroll deduction. Any Hospital currently paying more than 75% of the premium shall continue to do so. The drug formulary shall be as defined by Liberty Health Formulary Three.

  • Extended Health Plan An employee who makes an election under this provision must enrol in each and every of the benefit plans and shall not be entitled to except any of them.

  • HEALTH CARE PLANS ‌ Notwithstanding the references to the Pacific Blue Cross Plans in this article, the parties agree that Employers, who are not currently providing benefits under the Pacific Blue Cross Plans may continue to provide the benefits through another carrier providing that the overall level of benefits is comparable to the level of benefits under the Pacific Blue Cross Plans.
