Financial Assistance Mechanism. Cooperative Agreement Substantial Involvement by CDC: This is a cooperative agreement and CDC will have substantial programmatic involvement after the award is made. Substantial involvement is in addition to all post- award monitoring, technical assistance, and performance reviews undertaken in the normal course of stewardship of federal funds. CDC will provide substantial involvement beyond regular performance and financial monitoring during the project period. Substantial involvement means that applicants can expect federal programmatic partnership in carrying out the effort under the award. CDC will work in partnership with awardees to ensure the success of the cooperative agreement by:  Providing cross-site and awardee-specific surveillance technical assistance such as providing tools to identify drug poisonings using ICD-9-CM, ICD-10, text searches and ICD-10-CM, if implemented during the award period;  Providing technical assistance to revise annual work plans;  Assisting in advancing program activities to achieve project outcomes;  Providing scientific subject matter expertise and resources;  Collaborating with awardees to develop evaluation plans that align with CDC evaluation activities; Providing technical assistance on awardee’s evaluation and performance measurement plan; Providing technical assistance to define and operationalize performance measures;  Facilitating the sharing of information among grantees;  Participating in relevant meetings, committees, conference calls, and working groups related to the cooperative agreement requirements to achieve outcomes;  Coordinating communication and program linkages  Coordinating communication and program linkages with other CDC programs and Federal agencies, such as Centers for Medicare and Medicaid Services (CMS), Food and Drug Administration (FDA), the National Institutes of Health (NIH), the Substance Abuse and Mental Health Services Administration (SAMHSA), and the HHS Office of the National Coordinator for Health Information Technology (ONC)  Translating and disseminating lessons learned through publications, meetings, surveillance measures and other means on promising and best practices to expand the evidence base. CDC program staff will assist, coordinate, or participate in carrying out effort under the award, and recipients agree to the responsibilities therein, as detailed in the NOFO.

CDC program staff will assist, coordinate, or participate in carrying out effort under the award, and recipients agree to the responsibilities therein, as detailed in the NOFO. • Provide guidance, trainings, tools, and technical assistance to award recipient on program area components. • Host conference calls for sharing information from CDC and between award recipient. • Work with award recipients on planning and implementation of required and proposed activities. • Monitor award recipient vaccine spend plan and replenishment providing regular feedback and working with award recipient to address barriers. • Analyze and report results of surveys regarding national, state and selected local level vaccination coverage. • Analyze and report result of surveys of award recipient plans and activities related to preparedness. • Collaboration with external partners to promote IIS in national health IT settings. • Develop and support improvements in IIS data quality and program measures to assess IIS performance. • Use IIS submitted data to develop methods to estimate national vaccination coverage and calculate vaccination coverage estimates for routinely recommended childhood vaccines. • Use IIS submitted data to monitor seasonal or pandemic influenza vaccination among ages that are available in the IIS data. • Assist, as needed, in the development of data collection and reporting methods for school- enterer vaccination coverage surveys. • Provide VFC policy guidance via the VFC Operations Guide, periodic trainings and conference calls, including use of the CDC developed quality assurance site visit tools, to assist award recipient implement their VFC program in accordance with federal requirements. • Provide subject matter guidance in all program component areas. CDC activities are intended to ensure the success of the project and will include the following: • Provide ongoing technical assistance. • Provide SPAN implementation guidance to recipients on identifying and implementing strategies and activities. • Facilitate collaborative opportunities with other CDC funded NOFO national and state partners. • Promote information sharing between recipients:  Facilitate routine conference calls, webinars, and other informational exchange.  Develop mechanism for documenting and sharing lessons learned. • Convene recipient trainings. Additionally, CDC will: • Ensure that recipients have access to expertise found throughout NCCDPHP. • Collaborate with recipients to explore appropriate flexibilities needed to meet public health outcomes and goals. Flexibility in cooperative agreements includes recipient’s ability to propose alternative methods to achieve the outcomes and goals of the cooperative agreement that align with recipient’s opportunities for success, infrastructure, partner and stakeholder buy-in, demographics, and burden. This includes bringing together resources from multiple cooperative agreements to jointly advance the goals of each, and expanding the dialogue to bring in other CDC and recipient staff to reach a win/win solution. • Create greater efficiencies and consistency across NCCDPHP programs. For example,  Jointly developed resources and tools that focus on cross-cutting functions, settings, domains, risk factors, conditions and diseases to ensure consistent messages and to meet technical assistance needs.  Joint training and technical assistance opportunities that help recipients produce policies and programs that are more holistic and fully supportive of work in tobacco, nutrition, physical activity, chronic disease management and other strategies a...

CDC Program Support to Awardees CDC will: • Support awardees in the development/enhancement and implementation of their lead poisoning surveillance programs. • Review the use of data and information collection methods and analysis instruments specific to the use of CDC HHLPSS. • Provide guidance in implementing activities, and will identify major program issues, strategies, and priorities related to the cooperative agreement. • Provide technical assistance in assessing program effectiveness. • Promote collaboration with other federal, state, and local health; environmental; and housing agencies by initiating contacts, conference calls, and on-site visits to discuss programmatic issues. • Provide HHLPSS at no cost, support awardees in deployment of the system and migration of data from other systems to HHLPPS, and provide ongoing maintenance of the system. (Note: Many states previously established HHLPSS under CDC-funded cooperative agreements.) • Provide assistance in the evaluation of surveillance activities and reporting and disseminating reports to partners. • Provide consultation and technical assistance regarding techniques and approaches used to deliver or render services. • Review the use of data and information collected to support development, enhancement or implementation of population-based interventions and/or designating areas as lead- safe.

Related to Financial Assistance Mechanism

  • Financial Assistance Subject to the terms and conditions contained herein, the Director hereby grants to the Recipient financial assistance, as established in this section, for the sole and express purpose of paying or reimbursing the eligible costs certified to the Director under this Agreement for the completion of the Project.

  • OPWC Financial Assistance Subject to the terms and conditions contained herein, the OPWC hereby agrees to provide to the Recipient financial assistance not to exceed the amount as set forth in Appendix C of this Agreement for the sole and express purpose of paying or reimbursing the costs certified to the OPWC under this Agreement for the completion of the Project (the "Funds").

  • Calculation of Financial Assistance OHA will provide financial assistance to County at $834.61 per day, per authorized Individual. OHA is not obligated to pay County for expenditures beyond the limitation for the identified period of this Agreement. OHA will make monthly allotments from invoices, after XXX’s receipt, review, and approval of such invoices. All allotments made by OHA are subject to the limitation described herein.

  • PROVISION OF FINANCIAL ASSISTANCE As authorized by Section II of this Agreement for the sole and express purpose of financing the Project defined and described in Appendix A of this Agreement, the estimated costs of which are set forth and described in Appendix D of this Agreement, the OPWC hereby agrees to provide financial assistance, subject to the terms and conditions contained in this Agreement, from the State Capital Improvements Fund which constitutes the proceeds of the Infrastructure Bonds, in an amount not to exceed Two Hundred Sixty Thousand, Forty Dollars ($260,040). This financial assistance shall be provided in the form of a Grant. The OPWC Grant Control No. is CU06V.

  • PROVISION OF FINANCIAL ASSISTANCE GRANT As authorized by Section 2 of this Agreement for the sole and express purpose of financing the Project defined and described in Appendix A of this Agreement, the estimated costs of which are set forth and described in Appendix D of this Agreement, the OPWC hereby agrees to provide financial assistance, subject to the terms and conditions contained in this Agreement, from the State Capital Improvements Fund which constitutes the proceeds of the Infrastructure Bonds, in an amount not to exceed Two Hundred Forty-Nine Thousand, Nine Hundred Ninety-Eight Dollars ($249,998). This financial assistance shall be provided in the form of a Grant. The OPWC Grant Control No. is CG10R. Once this grant amount is fully expended, the loan amount, which is part of this Project Agreement, will be drawn on for disbursing the remaining OPWC obligations contained in this Agreement. An exception applies if the loan amount is necessitated for the local share. In which case, grant and loan assistance will be disbursed concurrently. LOAN

  • EDUCATIONAL ASSISTANCE PROGRAM The District’s Educational Assistance Program will be continued during the life of this Agreement. Employees who wish to enroll in job-related and promotion oriented courses, which will improve their job knowledge, and performance will be compensated by the District for expenses as follows:

  • Educational Assistance Plan The state shall provide up to six credit hours of tuition-free courses per term at a state university or community college to full-time employees on a space available basis as authorized by law.

  • Disbursement of Financial Assistance Unless a different disbursement method is specified in that line of Exhibit C “Financial Assistance Award,” OHA will disburse the financial assistance awarded for A&D 82 Services provided under a particular line of the Financial Assistance Award to County in substantially equal monthly allotments during the period specified in that line of the Financial Assistance Award, subject to the following:

  • Conditions to Financial Assistance and its Disbursement The Grantor's obligations hereunder, including its obligation to make financial assistance available to the Recipient pursuant to the terms of this Agreement, are contingent upon compliance by the Recipient with the following conditions:

  • Grant of Financial Assistance Subject to the terms and conditions contained herein, the OPWC hereby grants to the Recipient financial assistance not to exceed the amount, as set forth in Appendix C of this Agreement for the sole and express purpose of paying or reimbursing the costs certified to the OPWC under this Agreement for the completion of the Project.
