Illness, Injury and Medical Leave. A medical leave of absence for illness, 38 injury or medical, including pregnancy, shall be granted to EMPLOYEES upon 39 presenting acceptable verification to the EMPLOYER. The EMPLOYER may request at 40 any time as a condition of continuance of any medical leave of absence, proof of 1 continuing disability or sickness.

Illness, Injury and Medical Leave. 26 A medical leave of absence for illness, injury or medical, including pregnancy, shall be 27 granted to employees with seniority upon written request to the employee’s Department Head, 28 subject to the Employer's right to require medical proof or other verification acceptable to the 29 Employer. If paid sick time or workers' compensation benefits are either exhausted or not 30 available, an eligible employee may also request and receive other accrued paid benefit time to 31 which the employee may be entitled. The Employer may request at any time as a condition of 32 continuance of any medical leave of absence proof of continuing disability or sickness.

  • Family and Medical Leave 16.1 A. Consistent with the federal Family and Medical Leave Act of 1993 (FMLA) and any amendments thereto and the Washington State Family Leave Act of 2006 (WFLA), an employee who has worked for the state for at least twelve (12) months and for at least one thousand two hundred fifty (1,250) hours during the twelve (12) months prior to the requested leave is entitled to up to twelve (12) workweeks of family medical leave in a twelve (12) month period for one or more of the following reasons 1 - 4:

  • Extended Medical Leave A Staff Member may apply for extended medical leave for health reasons for the remainder of a current school year. The request for such leave must be accompanied by a statement from a physician indicating that such leave is necessary for the physical or emotional well-being of the Staff Member. Such leave may be granted upon receipt of said statement, upon the discretion of the Board and compliance with FMLA, USERRA, the ADA and the ADAAA of 2008.

  • Family and Medical Leave Act All employees who worked for the Employer for a minimum of twelve (12) months and worked at least 1250 hours during the past twelve (12) months are eligible for unpaid leave as set forth in the Family and Medical Leave Act of 1993. Eligible employees are entitled to up to a total of 12 weeks of unpaid leave during any twelve (12) month period for the following reasons:

  • Family and Medical Leave Act (FMLA A. The Board provides leave to eligible employees consistent with the Family and Medical Leave Act (FMLA). Eligible employees are entitled to up to 12 work weeks of unpaid family and medical leave in any 12-month period. The Board continues to pay the District’s share of the employee’s health benefits during the leave. In addition, the District restores the employee to the same or a similar position after the termination of the leave in accordance with Board policy.

  • Family Care and Medical Leave An unpaid Family Care and Medical Leave shall be granted, to the extent of and subject to the restrictions as set forth below, to an employee who has been employed for at least twelve (12) months and who has served for 130 workdays during the twelve (12) months immediately preceding the effective date of the leave. For purposes of this Section, furlough days and days worked during off-basis time shall count as "workdays". Family Care and Medical Leave absences of twenty (20) consecutive working days or less can be granted by the immediate administrator or designee. Leaves of twenty (20) or more consecutive working days can be granted only by submission of a formal leave application to the Personnel Commission.

  • Family and Medical Leave Act of 1993 (a) Leave provisions of this Agreement shall be construed consistently with the requirements of the Act. However, if the leave provisions under this Article are more generous, this Agreement shall prevail.

  • Occupational Injury Leave Occupational injury leave shall be governed by the Rules promulgated on this subject and the Ohio Revised Code 5503 as they exist on March 26, 1989, except as modified in this Article. All employees in the bargaining unit shall be entitled to occupational injury leave.
