Leave. Every Employee The following provisions are effective upon approval by Human Resources Development Canada. Pending that approval, the sick leave provisions of the Collective Agreement expiring October shall receive a credit of six (6) be deemed to continue. Leave. Every Catastrophic leave is a paid leave of absence due to life-threatening verifiable long-term illness or injury such as, but not limited to, cancer and heart attack which clearly disables the individual. The Catastrophic Leave Policy can be found on the Employee Services Web Page at: xxxx://xxxxxxx.xxxxxx.xxx/EmployeeServices/Pages/aboutleaves.aspx All permanent employees of the City of Santa Xxxx who have completed 1 year in paid status shall receive be eligible for catastrophic leave due to their own catastrophic illness or injury or catastrophic illness or injury to dependent spouse, qualified domestic partner, parent or dependent minor child. The employee must first exhaust all accrued sick leave, vacation leave, floating holiday, 20 hour benefit portion of administrative leave and compensatory time before qualifying for catastrophic leave. Once all accrued benefits have been exhausted and catastrophic leave has been granted, the employee shall not accrue any vacation or sick leave while using donated hours. If an employee returns to work on a part-time basis during their catastrophic leave, the employee shall accrue vacation and sick leave. Any such accruals will be banked. No new accruals, if any, may be used until all catastrophic leave credit has been exhausted. Catastrophic leave shall be additional paid leave available from vacation, compensatory leave, administrative leave hours, Management Benefit Plan or floating holidays donated by other CITY employees to a specific qualified employee. Employees donating vacation, compensatory or administrative leave must be done in increments of six whole hours. The donating employee must have a vacation leave balance of at least forty (640) hours after the donation of vacation time. Employees may donate all of their accrued compensatory time or administrative leave. Effective July 1, 2006, employees may also donate sick leave up to twenty four (24) hours in a fiscal year. Employees donating sick leave must have a balance of 80 hours after the donation of sick leave. An employee requesting catastrophic leave for each month must receive the recommendation of service, his or her department head and the approval of the City Manager or his/her designated committee. Such leave may initially be approved up to a maximum of seventy three hundred forty (70340) donated hours. If the catastrophic illness or injury continues, up to an additional three hundred forty (340) donated hours may be recommended for approval. If leave is granted, the CITY shall require the employee to provide medical information indicating the nature of sick leave per calendar yearthe illness or injury, the prognosis and estimated date of return. In addition, the CITY shall require, prior to the employee’s return to work, written approval from his or her doctor to resume job duties. The Employer CITY shall require an employee to sign a form authorizing release to the CITY of any and all medical information prior to granting catastrophic leave. If the Alliance agree department head determines that sick leave credits cannot be banked and accumulated from year to year. Each Employee shall be entitled to receive sick pay, at full salary, for any time lost by reason documentation of illness or injury and ability to return to work is unnecessary, the full extent requirement to provide medical information and doctor’s release may be waived. Every Employee shall receive a credit of six (6) hours of Pay for sick leave is provided for each the sole and only purpose of protecting a full-time nurse against loss of income because she was unable to work her regularly scheduled tour. When she legitimately ill, she may draw from the sick leave credits granted in this Collective Agreement, provided such credits are available. During the probation period, no sick leave benefit will be payable, but upon completion of the probationary period a full-time nurse will become eligible for the sick leave benefit. The benefit would date from the first day of the month of service, following the date probation was begun and one and a half days per month will be credited to the sick leave bank to a maximum of seventy sixty (7060) days. A nurse may be, at the option of the Employer, required to produce a doctor’s certificate for any illness. A nurse absenting herself account of personal illness must notify the Employer on the first (1st) day of that illness at least one (1) hour prior to the of the day tour, and at least two (2) hours prior to the of the evening and night tours. More advance notice will be given by the nurse whenever possible. Failure to provide such notice may result loss of sick leave benefits for that day of absence. Nurses who, after working a minimum of five (5) hours of their regularly scheduled tour are sent home for reasons of sickness will be paid their regular rate for the balance of that tour and no charge will be made against sick leave per calendar yearcredits. Where a nurse injured a compensable accident, she shall be paid for the balance of the tour. The Employer and right to sick pay shall cease upon discharge, voluntary termination, lay-off, leave of absence or suspension. Notwithstanding the Alliance agree that sixty (60) day limit Article sick leave credits cannot be banked and accumulated from year currently standing to year. If the Compensation Board does not approve the claim, the monies paid as an Employee’s advance will be applied toward the benefit to which the nurse would be entitled under the sick leave credit bank at the time of such absence, except for the Employee’s awarded compensation pursuant to the Workplace Safety and Insurance Act. Where an Employee is absent on sick leave for three (3) consecutive scheduled days or more, the Employer will require a note from the Employees physician, dentist, or other health care provider which indicates that the Employee was unable to carry out her dutiesplan. Any unused sick leave credits not used at payment under this provision will until the end of a calendar year will be forfeited. In cases where such verification has not been obtained, paid leave may be denied at the sole discretion of the School Principal or designate. If an Employee is in receipt visibly ill, the School Principal has the right to send the Employee home. Should this occur, the Employee is required to utilise time from Sick Leave Account.
