Life Insurance, Accidental Death and Dismemberment. (a) The cost of premiums for the $50,000 Basic Group Life Insurance including Accidental Death and Dismemberment coverage shall be shared on the basis of 10% by the teacher and 90% by the Board for teachers working 60% or more of full-time; teachers working less than 60% of full-time will receive 45% assistance on the cost of premiums.
(b) Subject to availability from the insurance carrier and eligibility, a teacher may acquire Optional Life Insurance paid 100% by the employee in multiple units of $25,000 to a maximum of $200,000.
(c) The cost of premiums for Dependent Life Insurance of $2,000 for spouse and $1,000 for each child over fourteen (14) days of age shall be shared on the basis of 15% by the teachers and 85% by the Board for teachers working 60% or more of full-time. Teachers working less than 60% of full-time will receive 42.5% assistance on the cost of premiums.
(d) An additional Dependent Life Insurance of $10,000 for spouse and $5,000 for each child over fourteen (14) days of age provided that the participating teacher pays 100% of the premium cost.
Life Insurance, Accidental Death and Dismemberment. The employer shall offer life insurance and accidental death and dismemberment insurance at no cost to each employee.
Life Insurance, Accidental Death and Dismemberment. 15.1.1.1 Any Employee not electing dependent coverage for the Major Medical Insurance will receive term insurance in an amount equal to twice his/her annual base salary rounded to the next higher thousand dollars.
15.1.1.2 All Employees shall be given the option of receiving either the amount of term insurance specified above or the maximum amount of life insurance he/she may receive without being required to include the premiums for such insurance as taxable income.
Life Insurance, Accidental Death and Dismemberment. LIFE INSURANCE The Board will purchase from a carrier selected in accordance with A above, life insurance in the amount of $50,000 per member. Unit members shall have the option of purchasing additional life insurance at the personal expense of the teacher for self and for dependents at $10,000 increments, subject to the underwriting rules of the carrier.

Life Insurance, Accidental Death and Dismemberment. Effective January 1, 1991, an employee's amount of insurance shall be equal to an amount determined by multiplying the employee’s base rate by 2080. This base rate shall be determined with the employee’s last rate prior to date of death. Upon your retirement your amount of Life Insurance will reduce to $4,000. However, your Accidental Death and Dismemberment Insurance will terminate upon your retirement.
Life Insurance, Accidental Death and Dismemberment. Insurance will be provided for those employees and their dependents who qualify for Disability Retirement subject to the waiver of premiums by the insurance carriers. Life Insurance and Accidental Death and Dismemberment Insurance coverage will be provided for those employees receiving a disability retirement prior to age sixty-five (65) up to the age of 65.

Term life insurance, with Accidental Death and Dismemberment (AD&D), will be provided in an amount equal to two (2) times a professional employee’s annual base salary, rounded to the closest thousand dollars for the life of this Agreement. The entire life insurance and AD&D plan is available upon request or on the District’s staff portal. Please note several plan limitations and exclusions apply.
Life Insurance, Accidental Death and Dismemberment. The Board will pay the monthly premium costs for $25,000 Term Life Insurance plus $25,000 Accidental Death and Dismemberment for each eligible regular employee contracted to work at least twenty (20) hours per week.
