Limits of Service. It is expressly agreed that no supervision or employment relationship exists between Consultant and Consultee. Consultee – or Consultee’s legally mandated supervisor if any – remains solely responsible for services provided to Consultee’s clients. Consultant will provide information based on research, scholarly consensus, and Consultant’s experience for Consultee to consider. Consultee will rely on his or her own judgment in offering specific psychotherapy services to Consultee’s clients and will not state or imply he or she is following guidance from Consultant. EMDRIA Group Consultation hours only: This group is structured to provide only group consultation hours toward EMDRIA Certification. I will actively share my EMDR case work experience in each session. After the 6th group session, Dr. Leeds will provide documentation of group consultation hours for the time I spend in these sessions. Conflict resolution: A friendly atmosphere is advocated and fostered in the consultation process. If differences arise, both Dr. Leeds and I commit to resolving any issues in a professional and mutually beneficial manner, including, if necessary, bringing in a third party. Dr. Leeds and I each agree to abide by the code of ethics of the professional organization(s) to which we belong. Logistics: The consultation group will meet via Zoom for 6 sessions Friday Series: January 7, February 4, March 4, April 1, May 6, June 3, 2022. Call times for the Friday series: Pacific: 9:00 – 11:00 AM; Mountain: 10:00 – 12:00 Noon; Central: 11:00 – 1:00 PM; Eastern: 12:00 – 2:00 PM The consultation group includes a maximum of eight members. I will be given in advance a series of Zoom meeting invitations that allow options for access by the Zoom application or telephone. I agree to securely share case files via a free account on Xxx.xxx. Consultee affirms that he or she is independently licensed or credentialed to practice psychotherapy in his or her jurisdiction, maintains malpractice insurance and that Consultee remains solely responsible for the nature and quality of the services provided to Consultee’s clients. Consultant agrees to provide Consultee information based on available research, scholarly consensus, and Consultant’s experience for Consultee to consider. Consultee will at all times rely on his or her own judgment and discretion in offering psychotherapy services to Xxxxxxxxx’s clients. Confidentiality: Consultee will notify clients and obtain written informed consent for consultation prior to presenting any case material to Consultant. Consultee will omit or redact any and all potentially identifying information from written materials and in oral discussions with Consultant. Consultation will focus on principles and fidelity in application of EMDR treatment for kinds of cases, not for specific or identified cases. Evidence of fidelity in the use of EMDR: Meta-analysis of EMDR treatment outcome research shows that degree of fidelity to the standard EMDR protocol and the AIP model predict degree of client improvement. Therefore it is agreed that before Consultant will write a letter in support of EMDRIA certification for Consultee, Consultee will provide Consultant with behavioral work samples (by video recording or near verbatim transcript) of multiple EMDR treatment sessions that demonstrate satisfactory fidelity to the standard EMDR protocol and procedural steps and for standard EMDR treatment planning and implementation for PTSD. It is understood that more than one review is generally needed to provide evidence of satisfactory fidelity for both procedural steps and treatment planning and implementation. It is understood that Consultee may need to complete more than the minimum number of consultation hours required by EMDRIA to demonstrate satisfactory fidelity. Consultee may need to pursue additional study of the standard texts or obtain additional training in order to demonstrate satisfactory fidelity. It is recommended that Consultee begin presenting behavioral work samples of reprocessing sessions to Consultant by the third of hour of consultation. Limits of Service. There are circumstances in which I may not be the most appropriate provider of mental health services. If this is the case, I will make a referral for you to another mental health professional or community agency which may be better suited to meet the needs at hand. If for some reason treatment is not going well, I might suggest you see another therapist or another professional for an evaluation. As a responsible person and ethical therapist, I cannot continue to treat you if my treatment is not working for you. If you are currently involved with or anticipate involvement in a legal case requiring testimony regarding your mental health status and functioning or require any form of forensic work, I will not provide such services or information on your behalf. Appropriate professional referrals for psychological evaluations and forensic testimony will be provided for you. APPOINTMENTS My appointments are usually 55-60 minutes. The time is reserved specifically for you. You will be charged the full fee if you are late, and you will be charged a $100 fee if you miss the appointment or cancel with less than 24 hours’ notice. I will tell you at least a month in advance of my vacations or any other times we cannot meet. Please ask about my schedule in making your own plans. FEES My fee is $180 for a 55-60 minute session. Other time I may spend on your behalf for activities such as consultations pertaining to you, or phone calls with you that exceed 5 minutes, are calculated at a base rate of $180 per hour (60 minutes). Payment is due at the beginning of each session. If there is a difficulty or concern about payment, please discuss this with me. You -not your insurance company or any other person -are responsible for paying the fees we agree upon. If you ask me to bill a separated spouse, a relative, or an insurance company, and I do not receive payment from them on time, I will then expect this payment from you.
Limits of Service. The Service includes CD updates for the Covered Products only. The service is limited to the length of the Agreement or number of updates received, whichever comes first. The Service provided does not include assistance in training your personnel in the installation, administration or normal use of the Covered Products. Limits of Service. The purpose of the service is to assist you to answer day-to-day questions that arise in your business or practice. The service does not replace the need for specialist advice on planning or strategy matters. Nor can we advise you about the correctness of tax positions you have taken or commercial transactions. Therefore we cannot review specific documents or other detailed attachments. However, where possible, we will set out the relevant legal issues or alternatives for further consideration. CCH is a publishing company and generally our staff members are not practising lawyers, therefore legally we cannot provide the same services as law firms and we may decline to answer questions on that basis. Registration for the service and payment of the membership fee entitles you to submit questions using the question template on the CCH website. We will use our reasonable endeavours to answer all questions submitted by you which we consider to fit within the above parameters of this service. However, we reserve the right to decline to answer those questions which we consider to fall outside the scope of this service (see Sample Questions), such decision being final and not open for review. However, we will act in good faith and will communicate clearly with you in these instances. When using the standard subscription service, we aim to get a final answer to you within three working days for tax questions and straight-forward questions in other non-tax areas. However, we do not promise you that we will be able to meet these targets in all cases, and we take no responsibility, and assume no liability to you or anyone else, if we are unable to answer your questions within that time period or if we decline to answer your questions at all.

Limits of Service. It is expressly understood that no on-site supervision or employment relationship exists between Supervisor and Supervisee. Supervisee affirms that s/he is independently approved to practice in her/his jurisdiction, that s/he maintains malpractice insurance, and that Supervisee remains solely responsible for the nature and quality of the services provided to Supervisee’s clients. Supervisor agrees to provide Supervisee information based on available research, scholarly consensus, BBS standards, and Supervisor’s experience for Supervisee to consider. Supervisee will rely on her/his own judgment and discretion in offering clinical services to Supervisee’s clients. Consistent with the professional standards of Supervisee’s pre-licensure or pre-certification, jurisdiction, and professional memberships, Supervisee will consider the need to obtain separate, additional supervision from other Consultants(s) for any significant clinical, ethical, legal, or professional issues which either Supervisor or Supervisee believes merit it.
Limits of Service. Client agrees that there are limits to the service which IJASCODE, LLC can provide. Wireless Delivery Service provided by IJASCODE, LLC has a “soft limit” of monthly aggregate data usage. The level of usage is delineated in the estimate or invoice provided with the initial agreement. If the “soft limit” is not explicitly stated in the estimate or invoice, it will be defaulted to 5GB. Should the camera exceed this monthly aggregate limit IJASCODE, LLC reserves the right to charge additional monthly fees for exceeding the “soft limit”. Monthly web hosting service is also limited by the amount of server storage and amount of server bandwidth. IJASCODE, LLC reserves the right to take actions to protect its server storage space and bandwidth from any excessive use by the client including adding fees to the monthly charges.

Confidentiality: Consultee will notify clients and obtain written informed consent for consultation prior to presenting any confidential case material to Consultant. Consultee will modify, omit and/or redact all potentially identifying information from all case material. Resolution of issues: Consultant and Consultee abide by their respective professional organizationscode of ethics. If any ethical issues arise related to the consultation relationship, Consultant and Consultee will make every effort to resolve them informally and with good will. Fees: Consultee will pay Consultant a fee of $185 per hour of service for consultation. Payments may be by credit card or by check within 7 days of services being rendered.
