Outreach to Students, District Staff and Community Members. 3. The District will develop and administer an annual school climate survey to students (grades 3-12), staff, and parents. The survey will be administered separately to each of these groups and will measure (including, but not limited to) the perceptions of: relationships among the school community members (teacher-student, teacher-parent, and student relationships); school safety; clarity of rules and behavioral expectations; and fairness and equity in the administration of discipline. The District agrees to submit the climate survey prior to administration to OCR for review and approval. The District will draft a report about the survey results (to include response rates) and determine a method for sharing the report publicly, particularly with the participating groups. All LISD schools will be requested to participate in the annual climate survey. REPORTING REQUIREMENT: By January 12, 2018, January 12, 2019, and January 12, 2020, the District will submit to OCR for review and approval the climate surveys referenced in this item. During the monitoring period, if the District utilizes a climate survey previously approved by OCR, the District does not need to submit the survey again to OCR for approval. By June 30, 2018, June 30, 2019, and June 30, 2020, the District will submit to OCR documentation showing that its climate survey has been administered, a copy of the report on survey results, and evidence of how the report was shared publicly.

Student Discipline Annual Educational Session (Student Session): Each school in the District will conduct annual educational in-classroom sessions during regular school hours that specifically provides students with information regarding the District discipline policies, practices, and procedures, including definitions of terms and consequences for misbehavior. The Student Session shall take place by May 31, 2017 and by the same date annually thereafter. The sessions will also include an opportunity for students to provide suggestions for improvements in discipline policies, procedures, and practices. The District will select an appropriate format for each session based on the age and grade levels of students and the particular needs of each school, as long as each student is given the opportunity to participate meaningfully in such a session at least annually. At least one site administrator with school-wide responsibility for discipline for each school site will, on an annual basis, invite all students at the school site to provide him/her with suggestions for improvements in discipline policies, procedures, and practices at already scheduled education sessions, a school assembly, office hours, or through another appropriate means determined by the school site with the goal of ensuring students at the school are offered a meaningful opportunity to participate. Reporting Requirements: By June 30, 2017 and by the same date annually thereafter, the District will provide documentation to OCR of each Student Session, including the date, time, length, and location of each session, the number of students attending, a summary of the concerns and suggestions expressed by students at each such session, and District responses, if any.

By September 30, 2016, and by September 30 each school year thereafter, in order to raise awareness of discipline issues, each middle school, high school, charter school and alternative school in the District will conduct an annual forum during regular school hours that specifically provides an opportunity for students to discuss with faculty and administrators any matters relating to discipline, including non- discrimination, and provide their input for any improvements in the District’s discipline policies. The District will designate at least one student note-taker for each session. At least one administrator not affiliated with the building at which the forum is being held will attend each forum session or a part of each forum session. If an administrator is not a part of the entire forum session, students who have been assigned as note-takers for sessions, shall meet with the administrator to share the summaries of the sessions and all the suggestions and comments of the students. An adult staff member will be present at each session involving student note-takers. The District will select an appropriate format for each session based on the age and grade levels of students and the particular needs of each school, as long as each student is given the opportunity to participate meaningfully in such a session at least annually. REPORTING REQUIREMENTS: By December 15, 2016, December 15, 2017, and December 15, 2018, the District will provide documentation to OCR of each student forum session including the date, time, length and location of each session, the number of students attending, a summary of the concerns and suggestions expressed by students at each such session, and the District’s responses to the same.

Typically, a 3-unit course. Not required for students who transfer in 30+ units. Writing 1: The Art & Craft of Writing Any first-semester composition course. Often titled "Freshman Composition," "College Composition," or "Reading and Composition." Must include basic research skills and a research paper. Writing 2: Genre, Evidence, & Persuasion Courses titled "Critical Thinking," "Advanced Composition," etc., that follow a basic freshman level writing course. These courses involve the use of logic, critical thinking, rhetoric, and advanced composition. In addition, genre-specific writing courses will introduce students to the genres of writing, rhetorical moves, and forms of evidence in a specific discipline. Possible courses include: Writing in the Humanities, Writing in the Social Sciences, Writing in the Arts, Writing in Theology, Writing in Business, Writing in Nursing, etc. Must include a research component. Writing 3: Writing in the Disciplines This category focuses on preparing students to be professionals in a field by being independent thinkers capable of constructing their own knowledge, including producing polished writing products in the genres of writing that students are likely to use in their future professions. Most courses in this category are required for the specific APU major and are therefore not likely to be fulfilled by a student's transfer work. Oral Communication Any Public Speaking or Oral Communication course. Must contain at least 3 individual public speeches. Cannot be taken as a hybrid course. Personal Wellness Any physical activity course with a cardio component and instruction in fitness principles. The aims of Traditional Medicine cooperation will be: (a) to build on existing agreements or arrangements already in place for Traditional Medicine cooperation; and (b) to promote information exchanges on Traditional Medicine between the Parties. 2.   • Community Involvement The Grantee will facilitate and convene a Community Task Force as one means of developing collaboration among the Grantee, affected residents, and the broader community. The Grantee also will provide information to keep the Community Task Force fully apprised of the planning and implementation of revitalization efforts. The Community Task Force shall be comprised of affected public housing residents, local government officials, service providers, community groups, and others. The Community Task Force will provide advice, counsel and recommendations to the Grantee on all aspects of the HOPE VI development process, including shaping the goals and outcome of the Community and Supportive Services Plan. Community Task Force participants also will disseminate information throughout the community about the Grantee's revitalization efforts.
