Participant Accident Insurance. If accepted as a Member, you must purchase participant accident insurance for each event to help to pay for acute injuries sustained while competing in most US PBR Events. This insurance coverage is subject to payment of a fee (that must be paid before the start of the Event), with any deductible paid by you. This insurance provides secondary (that is, your personal insurance policy shall apply first) participant medical coverage of up to US$20,000 for rides/incidents at US PBR UTB Events, and up to US$12,500 at US non-PBR UTB Events. You remain solely and fully liable for the full amount of any claims and expenses related to the injury regardless of any failure or deficiency of insurance coverage. This program is subject to change and cancellation at any time without notice. Please check with the PBR Membership Competition Office for more information about coverage amounts, terms and conditions. If accepted as a Member, I acknowledge and agree that as a condition of my membership, I consent to the medical examination protocols and rules applicable to PBR Members. I further acknowledge and agree that I will not be able to participate, and indeed will not participate, in any events unless I am cleared to participate by a qualified medical professional approved by PBR and that in the event I am not cleared to participate, I shall not be entitled to any compensation in connection therewith. I represent and warrant that I shall immediately alert the applicable PBR staff (as identified by PBR) in the event I begin to experience any symptoms associated with the COVID-19 (coronavirus) illness as identified by the Centers for Disease Control and Prevention (the CDC), including, without limitation, fever or chills, cough, shortness of breath or difficulty breathing, fatigue, muscle or body aches, headache, new loss of taste or smell, sore throat, congestion or runny nose, nausea or vomiting, or diarrhea, at any time during the Term prior to fulfilling any obligations hereunder. PBR Members are required to compete at PBR events with PBR approved Rowels. The cost of the Rowels is included in your 2021 PBR Membership Dues for the 2021 PBR Competition Season and PBR World Finals. Participant Accident Insurance. Participant Accident Insurance with limits per covered accident of not less than $25,000 covering all participants involved in SPORTS ORG or SPORTS ORG-sponsored activities on USOPC Olympic & Paralympic Training Centers (“OTC”), designated Olympic & Paralympic Training Sites and USOPC-sponsored events. Coverage may be written on a basis secondary to coverage carried by or on behalf of an individual participant. Participant Accident Insurance noted above is also required for any Sports Org given Elite Athlete Health Insurance (EAHI) for its athletes. The EAHI program contains a $25,000 sport related injury deductible; this Participant Accident Insurance required of the Sports Org is to cover the cost of this deductible so the cost is not transferred to the Athlete.

  • Travel Accident Insurance We agree to provide you with Travel Accident Insurance at no direct cost to you.   • Policy Because the volume of human genomic and phenotypic data maintained in these repositories is substantial and, in some instances, potentially sensitive (e.g., data related to the presence or risk of developing particular diseases or conditions and information regarding family relationships or ancestry), data must be shared in a manner consistent with the research participants’ informed consent, and the confidentiality of the data and the privacy of participants must be protected. Access to human genomic data will be provided to research investigators who, along with their institutions, have certified their agreement with the expectations and terms of access detailed below. NIH expects that, through Data Access Request (DAR) process, approved users of controlled-access datasets recognize any restrictions on data use established by the Submitting Institutions through the Institutional Certification, and as stated on the dbGaP study page. Definitions of the underlined terminology in this document are found in section 13. The parties to this Agreement include: the Principal Investigator (PI) requesting access to the genomic study dataset (an “Approved User”), the PI’s home institution (the “Requester”) as represented by the Institutional Signing Official designated through the eRA Commons system, and the NIH.   • Policy Grievance Where either Party disputes the general application, interpretation or alleged violation of an article of this Agreement, the dispute shall be discussed initially with the Employer or the Union, as the case may be, within thirty (30) days of the occurrence. Where no satisfactory agreement is reached, either Party may submit the dispute to arbitration, as set out in Article 10.
