Payroll Deduction. 16.1-1 The School Board agrees to deduct from the salaries of its classified employees dues for the ▇▇▇▇▇▇ Educational Support Association (CESA), as classified employees individually voluntarily authorize the School Board to deduct, and to transmit the monies promptly to the CESA. Authorization will be written on forms provided by the Association. 16.1-2 The Association will certify to the School Board in writing the current rate of membership dues. The School Board will be notified of any change in the rate of membership dues thirty (30) days prior to the effective date of such change. 16.1-3 Deductions referred to in 16.1-1 will be made in equal installments semi-monthly during the year. Any classified employee desiring to have the School District discontinue deductions he/she has previously authorized must notify the Association in writing between July 1st and July 15th of each year for that school year’s dues. Membership dues will then be discontinued as of the 1st pay period in August. 16.1-5 Upon termination of any employee, dues for the Association will end the month of termination. 16.1-6 The Association shall indemnify and save harmless the employer from any and all claims, demands, suits and costs, incurred in connection with any such claim, demand and suit, resulting from any action taken by the employer for the purpose of complying with the provisions of this Article. The School Board may designate one person to act on its behalf with respect to the accounting and administrative functions required by this Article; and may direct that such person be responsible for questions and problems regarding such accounting and administrative functions.

Payroll Deduction. 16.1-1 A. The School Board Revere Public Schools agrees to deduct from the salaries of its classified employees dues for the ▇▇▇▇▇▇ Educational Support Revere Teachers Association/Massachusetts Teachers Association/National Education Association (CESA), as classified employees individually voluntarily authorize the School Board to deduct, and to transmit the monies promptly to the CESAsuch Association(s). Authorization Teacher authorizations will be written in writing on forms provided approved by both the AssociationAssociation and the payroll office. 16.1-2 The Association B. Teachers waive all right and claim for said monies so deducted and transmitted in accordance with the authorization, and relieve the Revere Public Schools and all of its officers from any liability therefore. C. Each of the Associations named in Section A above will certify to the School Board Revere Public Schools in writing the current rate of its membership dues. The School Board Any Association which will be notified of any change in the rate of its membership dues will give thirty (30) days written notice prior to the effective date of such change. 16.1-3 D. Deductions of dues referred to in 16.1-1 Section A above will be made in equal weekly installments semi-monthly during until the yearfull Amount of said dues has been deducted. Any classified employee teacher desiring to have the School District Revere Public Schools discontinue deductions hededuction that s/she he has previously authorized must notify the office of the superintendent and the Association concerned in writing between July 1st and July 15th by September 15 of each year for that school year’s 's dues. Membership dues will then be discontinued as . F. Within thirty (30) days of the 1st pay period in August. 16.1-5 Upon termination execution of any employeethis Agreement, dues for the Association School Committee will end the month of termination. 16.1-6 The Association shall indemnify and save harmless the employer from any and all claims, demands, suits and costs, incurred in connection with any such claim, demand and suit, resulting from any action taken by the employer for the purpose of complying with vote to accept the provisions of this ArticleM.G.L. Ch. 190, Section 17I. Pursuant to that section an employee may authorize the Committee to deduct from his/her salary a contribution to Voice of Teachers for Education in the Amount of fifty cents ($.50) per week which the employee will specify in writing. The School Board may designate one person Committee will certify on the payroll the Amount to act on its behalf with respect be deducted by the Treasurer. Such amounts will be transmitted to the accounting Massachusetts Teachers Association within thirty (30) days of said deductions, along with a list of those contributing and administrative functions required by this Article; and may direct that such person be responsible for questions and problems regarding such accounting and administrative functionsthe Amount each employee has contributed.

Payroll Deduction. 16.1-1. The Treasurer shall make payroll deductions upon receipt of signed employee authorization for the following areas: Conseco Insurance Cancer Aid Program Tax Sheltered Annuities (upon completion of plan enrollment) Hospitalization/Major Medical/Life Insurance Dental Insurance Credit Union Fund for Children and Public Education (FCPE) (If at least 10 bargaining unit members sign up by October 1 of any given contract year) United Way 2. If any of these deductions as listed in this section changes, the name of the entity the new name will be recognized and honored for Article 8 Section G. 3. The School Board agrees to deduct from the salaries membership dues of its classified employees dues for the ▇▇▇▇▇▇ Educational Support Association (CESA), as classified employees individually voluntarily authorize the School Board to deduct, and to transmit the monies promptly to the CESA. Authorization will be written on forms provided by the Association., its unified affiliates and other affiliated organizations in the manner prescribed below: 16.1-2 a. The Association will certify inform each of its members and prospective members of the voluntary nature of his/her authorization for deduction(s) including the prescribed procedure utilizing said authorization and the provisions and procedures for revoking an authorization. b. The Association agrees to distribute and collect prescribed authorization forms from members of the bargaining unit. The Association agrees to provide the prescribed authorization forms to the School teacher. c. The Association President or Treasurer shall submit all new signed authorization forms to the Treasurer of the Board by October 10 of each year. Unless revoked in writing the current rate of membership dueskeeping with procedures contained herein, an authorization will continue from year to year. The School Board’s Treasurer shall deduct dues in ten (10) equal installments. Any member of the bargaining unit who has authorized said deductions may revoke his/her authorization by notifying the Board’s Treasurer on a form prescribed by the Board will be notified and the Association by October 10 of any change school year. d. Any teacher employed after October 1, shall be eligible for payroll deduction in the rate of membership dues thirty (30) days prior to the effective date of such change. 16.1-3 manner described herein. Payroll deduction changes for insurance(s) as a result of a switch from single to family coverage may be made at any time during open enrollment periods specified by the carrier. At other times the unit member may apply to switch, but acceptance depends upon the carrier’s dues. Membership dues will then be discontinued as acceptance of the 1st pay period in Augustchange. The change shall be effective upon acceptance by the carrier. 16.1-5 Upon termination of any employee, dues for the Association will end the month of termination. 16.1-6 5. The Association CEA shall indemnify the Board and save hold it harmless the employer from against any and all claims, demands, suits and costs, incurred in connection claims that may arise from compliance with any such claim, demand and suit, resulting from any action taken by the employer for the purpose of complying with the provisions of this Article. 6. The School Board Any teacher may designate one person make changes in tax sheltered annuity deductions up to act on its behalf with respect to the accounting and administrative functions required by this Article; and may direct that such person be responsible for questions and problems regarding such accounting and administrative functionstwo times per school year.

The Union shall certify the amount of dues to be deducted from each employee’s salary for the ▇▇▇▇▇▇ Educational Support Association (CESA), as classified employees individually voluntarily current school year. Authorization Cards will be furnished by the Union and approved by the Board. A. An Employee may authorize the School Board to deduct, and to transmit the monies promptly dues deduction by presenting an Authorization Card to the CESABoard on or before September 1st. Authorization The Annual Membership dues will be written on forms provided deducted from the Employee’s monthly paycheck then (10) equal installments beginning with the September check. B. Any Employee after September 1st may authorize dues deduction by presenting an Authorization Card to the Board. One tenth (1/10th) of the Annual Membership dues will be deducted from the month of authorization through June of that fiscal year. C. All dues deducted by the AssociationBoard shall be remitted to the Union in ten (10) monthly installments within ten (10) working days after the close of the payroll period. 16.1-2 The Association will certify to the School Board in writing the current rate of membership dues. D. The School Board will be notified of any change in pay the rate of membership cost incurred to provide authorization dues thirty (30) days prior to the effective date of such changededuction. 16.1-3 Deductions referred E. The School Board may designate one person to act on its behalf with respect to the accounting and administrative functions required by this Article; and may direct that such person be responsible for questions and problems regarding such accounting and administrative functionssection.

Payroll Deduction. 16.1The Board of Education agrees that whenever duly authorized by any certified employee on a form or forms appropriate for such or required by law, the Business Office shall make the necessary deductions for the following: 1. USD 470 Group Health Insurance 2. Teacher Retirement 3. Withholding Tax - Federal 4. Withholding Tax - State 5. Social Security 6. Medicare 7. United Teaching Profession Dues (ACTA, K-1 The School NEA, NEA) After receipt of written authorization from teacher, the Board agrees to shall deduct from the salaries salary of its classified employees the employee and make appropriate remittance for Association dues (ACTA, K-NEA, NEA). Such authorization shall continue in effect for the ▇▇▇▇▇▇ Educational Support Association (CESA), as classified employees individually voluntarily authorize the School Board to deduct, and to transmit the monies promptly to the CESAentire school year. Authorization will automatically continue from year to year unless the district receives written notice from the teacher prior to October 1 that such deduction is to be terminated. A copy of the written on forms provided notice will be sent to the ACTA President. Pursuant to such authorization, the Board shall deduct one- eleventh (1/11th) or appropriate amount of such dues from the regular monthly salary of the employee beginning with the October check. Amount to be deducted shall be supplied the Board through a schedule established by the Association. 16.1-2 . The Association will certify Board shall transmit to the School Board in writing Association the current rate of membership dues. The School Board will be notified of any change in the rate of membership total monthly deduction for professional dues thirty (30) days prior to the effective date of such change. 16.1-3 Deductions referred to in 16.1-1 will be made in equal installments semi-monthly during the year. Deductions will be made effective the date the membership form is signed, and the dues for the sign-up pay period shall be deducted from the following month's salary. 16.1-4 No later than October 15th of each year, the Association will provide the School Board with a list of those employees who have voluntarily authorized the School Board to deduct dues for the organization named in Section 1 abovesalary period. The Association will notify shall hold the School Board monthly of any changes in said list. Any classified employee desiring to have the School District discontinue deductions he/she has previously authorized must notify the Association in writing between July 1st and July 15th of each year for that school year’s dues. Membership dues will then be discontinued as of the 1st pay period in August. 16.1-5 Upon termination of any employee, dues for the Association will end the month of termination. 16.1-6 The Association shall indemnify and save harmless the employer from any and all claims, demands, suits and costs, incurred in connection with any such claim, demand and suit, resulting from any action taken by the employer for Board in withholding dues and paying the purpose of complying with the provisions of this Article. The School Board may designate one person to act on its behalf with respect funds to the accounting Association. 8. ACT Credit Union 9. Voluntary TSA, 403 (b) Annuities and administrative functions required 403 (b 7) Custodial Account Mutual Funds Payroll deductions for Voluntary TSA’s or other 403(b) and 403(b7) Custodial Accounts shall be limited to only those vendors approved by this Article; the Board of Education and may direct that such person have an Investment Provider Agreement included in the district’s 403(b) Plan Document. Request for changes can be responsible for questions made anytime, and problems regarding such accounting and administrative functionswill become effective on the first payroll cycle fourteen (14) days after notification.

Payroll Deduction. 16.1-1 The School Board of Education of the Board of Cooperative Educational Services of St. ▇▇▇▇▇▇▇▇ and ▇▇▇▇▇ Counties agrees to deduct from the salaries of its classified employees unit members dues for the Board of Cooperative Educational Services Teachers' Association, the New York State United Teachers Association, the American Federation of Teachers, or any one of any combination of such Associations as said unit members, individually and voluntarily, authorize the Board to deduct. A form, attached to this Agreement as Appendix A, shall be used for the purpose of such deductions. In the event the Association changes the rate of its dues, the Board shall be given at least thirty (30) days notice before such change will become effective for the purpose of these deductions. The dues shall be deducted over the full year's pay schedule beginning with the first pay period in October. No later than two (2) weeks prior to the first scheduled paycheck in October, the Association shall provide the Board with a list and the original signed dues authorization cards of those unit members who have voluntarily authorized the Board to deduct dues. The Board of Education shall make every effort, following the final monthly pay period from which a dues deduction is made, to transmit the amount so deducted to the Treasurer of the St. ▇▇▇▇▇▇▇▇-▇▇▇▇▇ Educational Support Counties BOCES Teachers' Association (CESA), as classified employees individually voluntarily authorize the School Board to deduct, and to transmit the monies promptly to the CESA. Authorization will be written on forms provided by the Association. 16.1-2 The Association will certify to 10th of the School Board in writing the current rate of membership duesfollowing month. The School final transmittal shall be accomplished by a listing of the members for whom deductions have been made and the amount deducted for each. If deductions have been made for only a portion of the deduction period, the listing will show the date of commencement of such deductions. A unit member may withdraw his/her authorization at any time by written notice received by the Board will be notified of any change in the rate of membership dues thirty (30) days Education at least two weeks prior to the effective date of such changepay period. 16.1-3 Deductions referred to in 16.1-1 will be made in equal installments semi-monthly during the year. Payroll Deduction. 16.1-1 15.1 During the term of this Agreement and to the extent the laws of the State of California permit, and as provided herein, the CITY will, in addition to deductions required by law, make payroll deductions with the employee’s consent for U.S. Savings Bonds, Employee’s Credit Union, Employee’s Health and Accident Insurance, Life Insurance, charitable contributions, Deferred Compensation and monthly dues for membership in ASSOCIATION. 15.2 The School Board agrees deduction of dues for membership in ASSOCIATION shall be based on a dues schedule provided to CITY by ASSOCIATION and made from the pay of each employee who voluntarily executes and delivers to the CITY the following authorization form: OF MEMBERSHIP DUES Name: Department: Social Security No.: 15.3 I authorize the City to deduct from wages earned by me, monthly membership dues as certified to the salaries of its classified employees dues City by the President or other designated authorized employee organization official and to remit the same to the authorized employee organization at such time and in such manner as may be agreed upon between the City and the organization. 15.4 This authorization shall remain in effect for the ▇▇▇▇▇▇ Educational Support Association duration of my employment with the City or until my termination from the City or until it is revoked by me in writing. 15.5 Payroll deductions shall be made monthly and special assessments and penalties when assessed. However, the initial deduction for any employee shall not begin unless both (CESA), as classified employees individually voluntarily authorize 1) a properly executed “Voluntary Authorization for Deduction of Organization Dues” and (2) the School Board to deduct, and to transmit amount of the monies promptly monthly membership dues certified by the organization has been delivered to the CESA. Authorization will be written on forms provided CITY at a place designated by the Association. 16.1-2 The Association will certify CITY at least ten (10) calendar days prior to the School Board in writing last day of the current rate of membership duespay period. The Association will notify the School Board monthly remittance or payment of any changes in said listsum other than that constituting actual deductions made from the pay earned by the employee. Membership dues will then be discontinued as of the 1st pay period in August. 16.1-5 Upon termination of any employee, dues for the Association will end the month of termination. 16.1-6 The Association organization shall indemnify and save the CITY harmless the employer from any liability resulting from any and all claims, demands, suits and costsor any other action arising from compliance with this Article or in reliance on any list, incurred in connection with any such claimnotice, demand and suit, resulting from any action taken by the employer for the purpose of complying with the provisions of certification or authorization furnished under this Article. The School Board may designate one person to act on its behalf with respect to the accounting and administrative functions required by this Article; and may direct that such person be responsible for questions and problems regarding such accounting and administrative functions.

The Board of Education of Mohonasen Central School Board District agrees to deduct deduction from the salaries of its classified employees of dues for Mohonasen Teachers’ Association, the ▇▇▇▇▇▇ Educational Support Association (CESA), New York State United Teachers or any one of any combination of such associations as classified employees said teacher individually and voluntarily authorize authorizes the School Board to deduct, deduct and to transmit the monies promptly to such association or associations in the manner prescribed in Section B. I hereby waive all rights and claims for said monies so deducted and transmitted in accordance with this authorization and relieve the Board of Education and its officers from any liability therefrom. This authority shall be continuous while employed in this school system or until withdrawn by written notice. $ Mohonasen Teachers’ Association $ New York State United Teachers $ American Federation of Teachers Employee Signature Date Certification of Dues 16.1-2 The Association will certify to the School Board in writing the current rate of membership dues. The School Board will be notified of any change in the rate of membership dues thirty (30) days prior to the effective date of such change. 16.1-3 B. Deductions referred to in 16.1-1 Section A shall be made in the following manner: The total annual membership dues for these designated professional associa- tions, certified by the MTA Treasurer on the authorization forms as shown in Section A, shall be deducted in ten equal consecutive installments beginning with the first pay period in November. No later than the fifteenth day of October, the Association shall submit to the Board signed original payroll deduction authorization forms for new members and two copies of a typed, alphabetic list of all Mohonasen Teachers’ Association members who have authorized payroll deductions. The list will itemize for each employee the exact amount of the annual dues to be deducted for the organizations named in Section A or any combination thereof. The list will be signed by the Treasurer of the MTA. C. Dues deductions will be made in equal installments semi-monthly during November, December, January, February and March. The Board of Education will transmit to the year. Membership dues will then be discontinued as Clerk of the 1st pay period in AugustBoard of Education. 16.1-5 Upon termination of any employee, dues for the Association will end the month of termination. 16.1-6 The Association shall indemnify and save harmless the employer from any and all claims, demands, suits and costs, incurred in connection with any such claim, demand and suit, resulting from any action taken by the employer for the purpose of complying with the provisions of this Article. The School Board may designate one person to act on its behalf with respect to the accounting and administrative functions required by this Article; and may direct that such person be responsible for questions and problems regarding such accounting and administrative functions.

Payroll Deduction. 16.1-1 A. The School Board District agrees to deduct from the salaries of its classified employees dues for the ▇▇▇▇▇▇ Educational Support L.T.A. and any state and/or national affiliates designated by the Association (CESA), as classified employees individually voluntarily authorize the School Board to deduct, and to transmit the monies promptly promptly. A teacher's authorization shall be in writing in the form set forth below. B. Designation and payroll authorization. TO: BOARD OF EDUCATION of Lowville Academy and Central School: Pursuant to Chapter 392, Laws of 1967, I hereby request and authorize you, according to arrangements agreed upon with such associations, to deduct from my salary and transmit to the CESA. Authorization will be written on forms provided associations indicated below, the dues as certified by the Association.respective associations. I hereby waive all rights and claim for said monies so deducted and transmitted in accordance with this authorization and relieve the Board of Education and all its officers from any liability thereof.
