Pursuant to that section an employee may authorize the Committee to deduct from his/her salary a contribution to Voice of Teachers for Education in the Amount of fifty cents ($.50) per week which the employee will specify in writing. Payroll Deduction. 16.1-1. The Treasurer shall make payroll deductions upon receipt of signed employee authorization for the following areas: Conseco Insurance Cancer Aid Program Tax Sheltered Annuities (upon completion of plan enrollment) Hospitalization/Major Medical/Life Insurance Dental Insurance Credit Union Fund for Children and Public Education (FCPE) (If at least 10 bargaining unit members sign up by October 1 of any given contract year) United Way
2. If any of these deductions as listed in this section changes, the name of the entity the new name will be recognized and honored for Article 8 Section G.
3. The School Board agrees to deduct from the salaries membership dues of its classified employees dues for the ▇▇▇▇▇▇ Educational Support Association (CESA), as classified employees individually voluntarily authorize the School Board to deduct, and to transmit the monies promptly to the CESA. Authorization will be written on forms provided by the Association., its unified affiliates and other affiliated organizations in the manner prescribed below:
16.1-2 a. The Association will certify inform each of its members and prospective members of the voluntary nature of his/her authorization for deduction(s) including the prescribed procedure utilizing said authorization and the provisions and procedures for revoking an authorization.
b. The Association agrees to distribute and collect prescribed authorization forms from members of the bargaining unit. The Association agrees to provide the prescribed authorization forms to the School teacher.
c. The Association President or Treasurer shall submit all new signed authorization forms to the Treasurer of the Board by October 10 of each year. Unless revoked in writing the current rate of membership dueskeeping with procedures contained herein, an authorization will continue from year to year. The School Board’s Treasurer shall deduct dues in ten (10) equal installments. Any member of the bargaining unit who has authorized said deductions may revoke his/her authorization by notifying the Board’s Treasurer on a form prescribed by the Board will be notified and the Association by October 10 of any change school year.
d. Any teacher employed after October 1, shall be eligible for payroll deduction in the rate of membership dues thirty (30) days prior to the effective date of such change.

g. If a member’s employment is terminated or authorization is revoked after October 10, the School Board monthly balance of any changes in said list. 4. Payroll deduction changes for insurance(s) as a result of a switch from single to family coverage may be made at any time during open enrollment periods specified by the carrier. At other times the unit member may apply to switch, but acceptance depends upon the carrier’s dues. Membership dues will then be discontinued as acceptance of the 1st pay period in Augustchange. The change shall be effective upon acceptance by the carrier.
16.1-5 Upon termination of any employee, dues for the Association will end the month of termination.
16.1-6 5. The Association CEA shall indemnify the Board and save hold it harmless the employer from against any and all claims, demands, suits and costs, incurred in connection claims that may arise from compliance with any such claim, demand and suit, resulting from any action taken by the employer for the purpose of complying with the provisions of this Article.

15.4 This authorization shall remain in effect for the ▇▇▇▇▇▇ Educational Support Association duration of my employment with the City or until my termination from the City or until it is revoked by me in writing.
15.5 Payroll deductions shall be made monthly and special assessments and penalties when assessed. However, the initial deduction for any employee shall not begin unless both (CESA), as classified employees individually voluntarily authorize 1) a properly executed “Voluntary Authorization for Deduction of Organization Dues” and (2) the School Board to deduct, and to transmit amount of the monies promptly monthly membership dues certified by the organization has been delivered to the CESA. The Association will notify the School Board monthly remittance or payment of any changes in said listsum other than that constituting actual deductions made from the pay earned by the employee. 16.1-2 The Association will certify to the School Board in writing the current rate of membership dues. The School Board will be notified of any change in the rate of membership dues thirty (30) days prior to the effective date of such change.
16.1-3 B. Deductions referred to in 16.1-1 Section A shall be made in the following manner: The total annual membership dues for these designated professional associa- tions, certified by the MTA Treasurer on the authorization forms as shown in Section A, shall be deducted in ten equal consecutive installments beginning with the first pay period in November. No later than the fifteenth day of October, the Association shall submit to the Board signed original payroll deduction authorization forms for new members and two copies of a typed, alphabetic list of all Mohonasen Teachers’ Association members who have authorized payroll deductions. The list will itemize for each employee the exact amount of the annual dues to be deducted for the organizations named in Section A or any combination thereof. The list will be signed by the Treasurer of the MTA.
C. Dues deductions will be made in equal installments semi-monthly during November, December, January, February and March. The Board of Education will transmit to the year. Deductions will be made effective treasurer of the date the membership form is signed, and the dues for the sign-up pay period shall be deducted Mohonasen Teachers’ Association within ten (10) working days from the following last day of the previous month's salary.
16.1-4 No later than October 15th of each year, the Association will provide the School Board with a list of those employees who have voluntarily check covering all authorized the School Board to deduct dues deductions for the organization that month for organizations named in Section 1 above. The Association will notify A of this Article (XIX).
D. Wherever it is indicated in Article III that the School MTA transmit forms, lists and other written items of information to the Board monthly of any changes in said list. Any classified employee desiring Education, all such communications shall be directed to have the School District discontinue deductions he/she has previously authorized must notify the Association in writing between July 1st and July 15th of each year for that school year’s dues. Membership dues will then be discontinued as Clerk of the 1st pay period in AugustBoard of Education.
16.1-5 Upon termination of any employee, dues for the Association will end the month of termination.
16.1-6 The Association shall indemnify and save harmless the employer from any and all claims, demands, suits and costs, incurred in connection with any such claim, demand and suit, resulting from any action taken by the employer for the purpose of complying with the provisions of this Article. The School Board may designate one person to act on its behalf with respect to the accounting and administrative functions required by this Article; and may direct that such person be responsible for questions and problems regarding such accounting and administrative functions.

Payroll Deduction. 16.1-1 A. The School Board District agrees to deduct from the salaries of its classified employees dues for the ▇▇▇▇▇▇ Educational Support L.T.A. and any state and/or national affiliates designated by the Association (CESA), as classified employees individually voluntarily authorize the School Board to deduct, and to transmit the monies promptly promptly. Any classified employee desiring to have the School District discontinue deductions he/she has previously authorized must notify the Association in writing between July 1st and July 15th of each year for that school year’s dues. F. An employee may withdraw his or her authorization at any time by written notice received by the District except between two (2) weeks prior to the first pay period in Augustdues are to be withheld and the last pay period.
