Plan Description. During the term of this Agreement, bargaining unit employees not covered by the Police and Fire Pension Plan shall be provided retirement benefits through the City=s defined contribution retirement plan established pursuant to Art. II, Section 3 of the City Charter.
Plan Description. At the time of retirement, the retiring employee shall receive a payment equal to one-half of the regular hourly rate of pay at retirement times the number of unused hours of sick leave accrued up to a maximum of sixteen hundred (1600) hours based on the following criteria:
21.2.1 at any age after thirty (30) years of service with the District; or
21.2.2 if your age plus years of public service total at least ninety (90) (Rule of 90) for employees in the Public Employees Retirement Association (“PERA”); or
21.2.3 at least twenty-nine (29) years and a day of service in the Minneapolis Employee Retirement Fund (“MERF”); or
21.2.4 after twenty (20) years of service with the District and age 60.

B. The benefit levels of dental, vision, life and death and dismemberment programs as they exist are presently described in the pertinent Plan Documents and are incorporated in this Memorandum by reference.
Plan Description. A. THE PLAN
Plan Description. The Systems RHI Plan provides medical benefits to eligible retired employees under a defined benefit post-employment health care plan. The contribution requirements of the RHI Plan members and the System are established and may be amended by the Systems Board of Directors. Current retiree RHI Plan members who are receiving benefits do not contribute to the RHI Plan as the System covered their health insurance based on current Medicare regulations, which made the RHI Plan the secondary payer with Medicare paying as the primary payer. Employees who retire at age 65 or later with 20 years of continuous full-time service or equivalent part-time service will receive, if they elect retiree health coverage, a $2,500 check each year for the rest of their life, which will be increased in subsequent years by 2%.
Plan Description. The System sponsors a frozen, noncontributory, single-employer, defined-benefit plan retirement plan (the CCMC Plan), administered by a committee appointed by the System. Under the provisions of the CCMC Plan, the System has the authority to make amendments. There have been no new members of the CCMC Plan since the date the CCMC Plan was frozen. The CCMC Plan provides life-only annuity benefits to plan members and beneficiaries. An actuarial report is prepared each year effective June 30 and is available from the System. The funding policy of the System is to contribute an amount at least equal to the Annual Required Contribution (ARC) prescribed by GASB and determined by the actuary. For the years ended September 30, 2023 and 2022, respectively, the ARC was $1.0 and $1.2 million.
Plan Description. The Company shall pay the monthly premium cost of the Long Term Disability Benefit Plan. The Long Term Disability Benefit Plan shall be administered in accordance with the terms of an insurance policy.

Contractor must include detailed descriptions of all new rate plans approved by the lead state by fully populating Attachment G with complete details related to each plan and feature offered under this contract. Contractors should propose plans that can be easily understood without complex restrictions and terms. Scoring will reflect the degree of concise and impactful plans – from flexibility and cost perspectives.
Plan Description. Pacific's repair service program for major gas and electric household appliances and patio products (including pool and spa heaters). Unlike the Appliance Protection Plan program, under the appliance repair service program Customers are charged only when repairs are performed. The Appliance Club is an association of Customers who receive free annual safety checks of heaters and air conditioners, 10% discounts on appliance repairs, automatic reminders of the need to change filters, and free furnace filters every heating season.
Plan Description. Pacific's program to design and construct functional natural gas pipelines or similar utility infrastructures on the Customer's side of the gas meter. This service can involve replacement of an existing "house" or "yard" line and/or installation of a new line.
