Participant Eligibility a. Applicants to the CIILB must satisfy the following criteria: i. An applicant must have evidence of being a donating member of the CIILB prior to the time of need. ii. Members must deplete their available sick leave days before using the CIILB. iii. Written application must be made by the participating member of the CIILB or a member of his/her family. The application shall be accompanied by a physician's certificate, signed by a licensed physician, stating the nature of the illness, estimated length of disability, and prognosis of the person's condition. iv. There must be a waiting period of five (5) contract days before days from the sick leave bank may be used. These days are unpaid. v. The member must not have income available to him/her through worker's compensation or long-term disability benefits.

Participant Eligibility. By executing this application and whenever using the Services, the undersigned hereby represents and warrants that Participant meets the eligibility requirements as set forth in Rule 3.2 of the applicable Rulebook(s), as amended from time to time, and if (i) trading through the SEF or (ii) executing block trades on the DCM is an Eligible Contract Participant (“ECP”). Further, each time such Participant or any of its Authorized Representatives enters an order, effects a transaction or otherwise uses the Services, Participant represents, warrants and reaffirms that Participant meets the eligibility requirements as set forth in Rule 3.2 of the applicable Rulebook(s), and if (i) trading through the SEF or (ii) executing block trades on the DCM, is an ECP.
Participant Eligibility. In connection with the processing of claims for benefits, HEBP will determine if a Participant is enrolled under the Plan based on eligibility and other information provided by the Plan Administrator. Participant Eligibility. All Xxxxx School District employees who earn sick leave benefits are eligible to participate in the Sick Leave Bank Program. Any employee who wishes to apply for assistance through the Sick Leave Bank Program must complete the Sick Leave Bank Program Request/Statement of Condition Form and submit it to the Superintendent of Schools. The Sick Leave Bank Program review committee consisting of the Superintendent, Business Manager, and two members of the teachers negotiating committee will meet to discuss and decide the application for assistance through the Sick Leave Bank Program. Notification of the decision, by the committee, will be in writing within seven (7) days of the application date. If the participant disagrees with the decision of the committee, he/she may appeal the decision to the Board of Education, whose decision shall be final. Decisions relating to the Sick Leave Bank Program are not subject to grievance procedures.
Participant Eligibility. A participant who meets the eligibility requirements for a Restart Agreement is referred to as a Restart Participant. The eligibility requirements for the Restart Agreement are detailed below. A Participant can only receive one wage subsidy at any given time. The Department’s IT Systems determine a Participant’s eligibility for the Restart Agreement. Providers should also check eligibility in accordance with Social Security Law, the Grant Agreement and this Guideline. If a Provider determines the Department’s IT Systems have incorrectly determined Participant eligibility, the Provider must contact their Relationship Manager to discuss the issue. A participant is eligible for Restart if they: • are 50 years of age or older • have received any Services Australia (Centrelink) or Department of Veterans’ Affairs Income Support Payment or pension (including Age Pension or Austudy) for the last six months • are registered with a DES Provider at the time the Restart Agreement is created in ESSWeb. If the Restart Participant is not already registered or commenced with a Provider, the Provider must register the Restart Participant as a Volunteer and create the Agreement within 12 weeks (84 days) from the Restart Placement. This is so the Provider can finalise the creation of the Agreement with the Employer within the 84 days required under the Guideline. Not eligible- Family Members Not eligible – Family Members Participants are not eligible to attract a Restart Wage Subsidy for a Restart Wage Subsidy Placement if they are a family member of the Restart Wage Subsidy Employer. For Restart Wage Subsidy Employers that are not constituted in an individual capacity the Department will treat the individuals behind the legal entity (e.g. directors, trustees, board members) as the ‘Restart Wage Subsidy Employer’ for the purposes of determining whether a Participant is a family member of the Restart Wage Subsidy Employer. Process Details To ensure there is no real, or perceived conflict of interest or unfair advantage compared with another member of the public, the Department considers the following to be family members: a) a spouse or former spouse, de facto partner or former de facto partner, child, parent, grandparent, grandchild or sibling, including where any of these relationships are step-relations (e.g. step-parents and step-children) as well as adopted relations, of the Employer b) a child, parent, grandparent, grandchild or sibling, including where any of these rel...
Participant Eligibility. 2.1 It is a condition of this Letter of Agreement that the Participant is registered under the NER as a Market Customer, Market Generator or Market Network Service Provider, and is a Wholesale Client. 2.2 Participant represents and warrants to AEMO that it is, and will at all times during the term of this Letter of Agreement remain, a Wholesale Client. 2.3 If Participant becomes aware that it will cease or has ceased to be a Wholesale Client, it must promptly notify AEMO in writing. 2.4 This Letter of Agreement terminates automatically on the date on which Participant ceases to be either: registered as a Market Customer, Market Generator or Market Network Service Provider; or a Wholesale Client.
