Practice Session. All Students will park in the sports complex parking lot. • Students will arrive dressed and ready for practice. • Buses will not be available to transfer students from the high school to the softball field. • Students will enter through the south main entrance between the softball field and city field. • Coach Xxxxxxxx will record temperatures and symptom checklist. • Students will use both dug-outs during practice to ensure social distance. • Coach Xxxxxxx Xxxxxxxx will ensure students maintain social distance in the home dug-out, Coach Xxxxx Xxxxxx will monitor the visitor dug-out while student place their bags at designated location. • Socially distance markers will be placed in dugout’s where each student will place their bag and water container. • Coach Xxxxx Xxxxxxxx will be waiting for the student in the outfield to begin warm-ups. • Students will separate along the foul line to begin warm-up. • Practice sessions will include hydration breaks. During each break students will separate and sanitize their hands. Game Day’s: • Home Games: o Coaches will have the same responsibilities as practice. o Students will socially distance in 1 dug out, and along the fence line if necessary. o Virtual students will receive their game clothes the day before, and expected to arrive dressed and ready to play. o The visiting team will be screened before arrival. Athletic trainer will perform screening if possible. If the Athletic Trainer is unable to be their Coach Xxxxxxx Xxxxxxxx will screen the visiting team. o No fans will be allowed due to seating capacity. o Referees will be screened before entering the field. • Away Games: o Face to Face Students: ▪ Will change into their game clothes in the locker room before departure. ▪ They will have their temperature rechecked before getting on the bus. o Virtual Students: ▪ Will receive their game clothes the day before the competition. ▪ They will be expected to arrive dressed and unable to enter the locker room. ▪ Students will be screened before entering the bus. ▪ Coach Xxxxxxx Xxxxxxxx, Xxxxxx Xxxxxxxx, or an Athletic Trainer will perform the screenings for the students. Hydration: • Students will need to bring their own water bottle. • Water coolers with a touchless spout will be provided for each dug-out. • Coaches will allow extra time for hydration breaks to allow each student to practice social distance, and refill if needed. Practice Session. Once the first lesson / practice session (after the 3-day window to cancel) has occurred, I may cancel this Agreement at any time, but all payments I have made to or that have been processed by Nugget Surf prior to its receipt of my written cancellation notice are nonrefundable. To cancel this Agreement, I will deliver a written cancellation request to a Nugget Surf employee or representative. I agree that I must pay for all lessons scheduled within the 30 days after Nugget Surf’s receipt of my request to cancel. WAIVER RELEASE OF LIABILITY WAIVER AND ASSUMPTION OF RISKS AGREEMENT **Caution** do not sign this document without reading it in its entirety. By signing this you give up important legal rights. Surfing is an athletic activity the participation of which involves certain risks. Please read and understand this document. Game Day’s: • Home Meets: o Students & Coaches will have the same responsibilities as practice. o Virtual students will receive their game clothes the day before, and expected to arrive dressed and ready to play. o The visiting teams will be screened before arrival. Athletic trainer will perform screening if possible. If the Athletic Trainer is unable to be Coach Xxxxxxxx Xxxxxxx will screen the visiting team. o No fans will be allowed due to seating capacity.

  • Practice Tip If the Buyer does not want to perform any inspections that are not named above, it is recommended that the Buyer initial “Waived” to make it clear that the lines were intentionally left blank. Practice Tip: There is only one line to elect and one line to waive this contingency. If more than one item is listed under other and the parties would like to remove one, the item to be removed should be crossed out and initialed. It might be clearer to simply address this change in an addendum to the Agreement. Existing Conditions There is a space to list any items that are to be excluded from any of these “blanket” inspection contingencies. The Sellers may want to consider excluding any items/systems that have already been disclosed as faulty. For example, if the Seller has disclosed that a hot water heater needs to be replaced, that defect should be considered by the Seller in setting an asking price and by the Buyer when making an offer. Including it in the list does not mean the Buyer cannot inspect for it, but it does mean that the Seller does not have to negotiate over it in a report/as part of a Written Corrective Proposal. The Seller should consider listing all defects so the Buyer does not attempt to negotiate the offer based on an inspection report detailing a defect that was already disclosed. Subparagraph (D): Notices Regarding Property & Environmental Inspections Clients should be directed to these Notices when they are deciding whether to elect or waive any certain inspection. These Notices provide a small description of some less-common issues that may arise and direct them to agencies which can give them more information. Paragraph 13: INSPECTION CONTINGENCY‌ Subparagraph (A): Contingency Period The Contingency Period that applies to all inspections elected in Paragraph 12 is established in Paragraph 13(A). The default Contingency Period is 10 days, though the parties can agree to a different length if desired.

  • Practice Guidelines 37.1. CONTRACTOR shall adopt practice guidelines (or adopt COUNTY’S practice guidelines) that meet the following requirements:

  • Medication 1. Xxxxxxx’s physician shall prescribe and monitor adequate dosage levels for each Client.

  • Contract for Professional Services of Physicians Optometrists, and Registered Nurses In accordance with Senate Bill 799, Acts 2021, 87th Leg., R.S., if Texas Government Code, Section 2254.008(a)(2) is applicable to this Contract, Contractor affirms that it possesses the necessary occupational licenses and experience.

  • Outpatient Dental Anesthesia Services This plan covers anesthesia services received in connection with a dental service when provided in a hospital or freestanding ambulatory surgical center and: • the use of this is medically necessary; and • the setting in which the service is received is determined to be appropriate. This plan also covers facility fees associated with these services.

  • Anesthesia Services This plan covers general and local anesthesia services received from an anesthesiologist when the surgical procedure is a covered healthcare service. This plan covers office visits or office consultations with an anesthesiologist when provided prior to a scheduled covered surgical procedure.

  • Chiropractic Services This plan covers chiropractic visits up to the benefit limit shown in the Summary of Medical Benefits. The benefit limit applies to any visit for the purposes of chiropractic treatment or diagnosis.

  • Training and Professional Development C. Maintain written program procedures covering these six (6) core activities. All procedures shall be consistent with the requirements of this Contract.

  • Medical Services Plan 10.1.1 Regular Full-Time and Temporary Full-Time Employees shall be entitled to be covered under the Medical Services Plan commencing the first day of the calendar month following the date of employment.

  • Medical Services We do not Cover medical services or dental services that are medical in nature, including any Hospital charges or prescription drug charges.
