Preparation of Report. If the supervisor, based upon the criteria set forth above, determines that a test should be requested, he/she shall comply with the following procedure. First, the subordinate shall be notified they are immediately being placed on paid administrative leave. From this point until the test sample is taken, the supervisor will not leave the subordinate unattended. Once the employee is placed on administrative leave, the supervisor shall prepare a report reducing the objective facts and reasonable inferences drawn from those facts to writing and shall immediately furnish a copy to the employee at that same time. The employee shall then be given the opportunity to explain his/her behavior, action or appearance. Upon request, the employee shall have the right to Union representation.
Preparation of Report. If the shift commander, based upon the criteria set forth above, determines that a test should be requested, he shall comply with the following procedure. First, the employee shall be relieved from duty. If the employee is at the scene of an emergency, he will be delivered by a department representative to a local testing facility, which may be the emergency department at a hospital at the City’s discretion, to complete this testing procedure. Once the employee is relieved from duty, the shift commander shall prepare a signed, dated, timed, and contemporaneous report reducing the objective facts and reasonable inferences drawn from those facts to writing. The employee shall, at that same time, be given the opportunity to explain his behavior/action/appearance. Upon request, the employee shall have the right to Union representation.
Preparation of Report. After the completion or discontinuance of the Joint Research, TU and COMPANY shall prepare a report on the Research Result in cooperation.

If the firefighter is at the scene of an emergency, he/she may be directed to return, or be taken to a station (when impairment is suspected) to complete this testing procedure. Once the firefighter is relieved from duty, a command officer shall prepare a signed, dated, timed, and contemporaneous report reducing the objective facts and the reasonable inferences drawn from those facts to writing and shall immediately furnish a copy to the employee. The employee shall, at that time, be given the opportunity to explain his/her behavior, action or appearance. Upon request, the employee shall have the right to Union representation. Failure of the command officer to prepare the report in accordance with the procedure outlined in this Section shall render any subsequent test and discipline based upon that test void.

Preparation of Report. 7.1 WA will as soon as practicable cause a draft impact assessment report to be prepared in accordance with the approved Terms of Reference (Attachment C).
7.2 WA shall provide the draft impact assessment report for public comment by notice:
(a) posted on the relevant WA agency’s website; and
(b) published in major WA newspapers. The notice must advise that the draft report is available and how copies may be obtained, provide contact details for obtaining further information, invite public comments on the draft impact assessment report and set a period of at least 28 days within which comments must be received.
