Procedures for Use. 1. A person who desires to use the Open Facilities must conclude prior consultation with the facility manager (the "Facility Manager") of the relevant Open Facilities at AIST and submit an Open Facilities Use Application Form (Attached Form No. 1; the "Application Form") to the Tsukuba Innovation Arena Headquarters of AIST.
2. AIST shall, when the requirements listed in the following items are recognized as having been completely satisfied, notify the User of permission for use by the Open Facilities Use Response (Attached Form No. 2; the "Response"). This notification shall cause an agreement on use of Open Facilities to be established in accordance with this Agreement between AIST and the User.
(1) The use applicant desires to use the Open Facilities.
(2) There is no risk of the use harming the public welfare or the betterment of the public or national interest.
(3) There is no risk of the use interfering in the research work of AIST.
(4) The use applicant is capable of paying the Use Fees under Article 6.
(5) There is no risk of the use applicant violating the matters for observance under Article 8.
(6) The use applicant is capable of assuming the indemnification obligation prescribed in Article 12 and the product liability prescribed in Article 14, and the like.
(7) There is no other particular reason, other than as prescribed in the above respective items, why the use of the Open Facilities should be recognized as unreasonable.
3. AIST may hear the views of a third party as to whether the requirements in the respective items of the preceding paragraph have been satisfied.
4. In the event that any of the requirements in the respective items of Paragraph 2 are not satisfied, AIST may notify of non-acceptance with the Response.

To request the use of the Wex’xxxx for an event, the user must: Schedule the event/activity at least two weeks in advance with the LIBC Planning Department via telephone by calling 000-000-0000 or in person at 0000 Xxxxx Xxxx as all coordination, dates and times with the Operations and Maintenance personnel will be required. At that time a pre-inspection walk-through will take place with the proposed renter. The payment will guarantee and confirm your scheduled event, however, in the event of an unexpected funeral all booked events will be cancelled or rescheduled when possible.
B. During the pre-inspection walk-through, the pre-inspection form will be completed and checked off between the Maintenance Personnel and the User at least 48 hours prior to the scheduled event.
C. The pre-inspection form must be signed and agreed to by both parties that the premise will be maintained and returned to its pre-rental condition upon completion of the event. The following guidelines apply: • All materials used or borrowed from the Wex’Xxxx used to help with the scheduled event (such as, tables, chairs, kitchen utensil, etc.) must be returned to its original place upon completion of the event. • The Wex’xxxx items/equipment’s are to be checked out with the Maintenance personnel during the pre-inspection walk-through and the items to be checked out are to be noted on the pre-inspection form. • The facility, bleachers and grounds should be maintained during events and garbage/debris in the surrounding areas should be picked up and disposed of at the completion of event. • All identified areas on the pre-inspection form should be cleaned and maintained accordingly. • All caterers are responsible for bringing their own cooking utensils, pots, pans, etc. and are also responsible for the cleanup of the kitchen.
D. The completed and signed pre-inspection form will be forwarded to the LIBC Planning Department to hold until the post-inspection form is completed by Maintenance Personnel after the event to ensure the deposits are secured.

Procedures for Use a. In addition to a signed Internet Usage and Equipment Agreement form, student users must have permission from their instructors before using computers, the Internet, or related technology equipment or resources.
b. Users may not play games or use computer resources for non-academic activities unless given permission to do so by the teacher, superintendent, administrators, or technology staff.
c. Users may not waste or take without permission supplies (such as paper, printer cartridges, external storage devices, etc.) that are provided by OCS.
d. No user of electronic media may purchase and/or install technology hardware or software without the written approval of the technology staff.
e. Damage or vandalism of any technology resource including, but not limited to, equipment, computer networks, or programs will result in punishment as defined in the school conduct policy.
f. Any use of technology resources that reduces the efficiency of use for others can be considered a violation of this agreement.
Procedures for Use. Once a study is released, Lifestory Research permits, via a license agreement, Licensees to use the awarded claim provided that the Licensee has signed and executed a license agreement with Lifestory Research.

Procedures for Use. 1. Administrators and teachers may access the Internet or e-mail for educational or work-related purposes at any time, which is not disruptive and does not interfere with the performance of other responsibilities by the employee.
2. The District will notify parents/guardians about the District network, related safety issues, and issues governing its Internet through a general letter to all parents. Parental permission is not required for use of the Internet, but parents will be notified they have the right to file a Parent/Guardian Denial Form available from the school principal if they do not want their child(ren) to have access to Internet resources.
3. A student’s parent of guardian must sign a Student E-mail Account Agreement in order for that student to be granted an individual e-mail account. The parent/guardian may withdraw approval at any time through a written request directed to the student’s teacher or principal.
4. All computer, Internet usage and e-mail usage by District employees and students must be consistent with the Greenville County School District mission and policies.
