RECOVERY RESIDENCE HOUSING SERVICES. The Performing Agency shall: 1. Subcontract with recovery residence providers in Texas to establish at eight (8) MAT RR Recovery Residences in metropolitan areas of Texas, and three (3) MAT RR in rural, border, and/or tribal residence areas, utilizing applicable procurement processes and standards for identifying the System Agency funded RR Providers. A. Performing Agency will develop, maintain, and submit a work plan for each MAT RR Recovery Residences to System Agency. Performing Agency shall submit the developed work plan to System Agency for review within thirty (30) calendar days of contract execution. 2. Ensure each home is established as a Level II or Level III home in accordance with the (NARR) standards of care, which are located at the following site: xxxxx://xxxxxxxxxx.xxx/wp-content/uploads/2017/10/NARR_Standard_V2_rev_07-10- 2017.pdf. 3. Subcontract with “Entity” to provide Certification of Recovery Residence Homes requirements to ensure the new RR homes are certified by TROHN per NARR (National Alliance for Recovery Residences) standards and/or have applied for certification. In addition, Performing Agency shall ensure each home has a yearly on-site inspection by subcontractor and once certified, ensure the RR recertifies with TROHN, as needed. If “Entity” discovers noncompliance issues during any audits, fidelity checks, certification, and/or recertification, “Entity” shall provide a report to Performing Agency. 4. A fidelity check includes, inspection findings, corrective actions, and any other pertinent information to ensure compliance. 5. The System Agency requests a copy of the Recovery Residence Certification for each funded RR; information on the Recovery Residence Certification process is at the following site: xxxxx://xxxxxxxxxxxxxx.xxx/services/recovery-residence-certification/. 6. Develop, and submit to System Agency a report, labeled Report A, in excel format, within sixty (60) calendar days of contract execution, that provides the following information: A. Legal Name of the Home; B. Doing Business As (DBA) of the Home; C. Legal Entity Number; D. Home Owner’s Name; E. Address; F. Contact Information (Phone/e-mail); X. XXXX Standard of Care Level (Level II and/or Level III); H. Compliance with NARR Standards of Care (Yes/No); I. Proposed date for compliance (if answer is No); J. Number of Beds in the Home; X. XXXXX/XXXX certification – enter date of certification; X. XXXXX/XXXX certification expiration – date certification expires;...

  • Special Services Should the Trust have occasion to request the Adviser to perform services not herein contemplated or to request the Adviser to arrange for the services of others, the Adviser will act for the Trust on behalf of the Fund upon request to the best of its ability, with compensation for the Adviser's services to be agreed upon with respect to each such occasion as it arises.

  • Surgery Services This plan covers surgery services to treat a disease or injury when: • the operation is not experimental or investigational, or cosmetic in nature; • the operation is being performed at the appropriate place of service; and • the physician is licensed to perform the surgery. This plan covers reconstructive surgery and procedures when the services are performed to relieve pain, or to correct or improve bodily function that is impaired as a result of: • a birth defect; • an accidental injury; • a disease; or • a previous covered surgical procedure. Functional indications for surgical correction do not include psychological, psychiatric or emotional reasons. This plan covers the procedures listed below to treat functional impairments. • abdominal wall surgery including panniculectomy (other than an abdominoplasty); • blepharoplasty and ptosis repair; • gastric bypass or gastric banding; • nasal reconstruction and septorhinoplasty; • orthognathic surgery including mandibular and maxillary osteotomy; • reduction mammoplasty; • removal of breast implants; • removal or treatment of proliferative vascular lesions and hemangiomas; • treatment of varicose veins; or • gynecomastia.

  • Educational Services Any service or supply for education, training or retraining services or testing including: special education, remedial education; cognitive remediation; wilderness/outdoor treatment, therapy or adventure programs (whether or not the program is part of a Residential Treatment facility or otherwise licensed institution); job training or job hardening programs; educational services and schooling or any such related or similar program including therapeutic programs within a school setting.

  • DEPENDENT PERSONAL SERVICES 1. Subject to the provisions of Articles 16, 18 and 19, salaries, wages and other similar remuneration derived by a resident of a Contracting State in respect of an employment shall be taxable only in that State unless the employment is exercised in the other Contracting State. If the employment is so exercised, such remuneration as is derived therefrom may be taxed in that other State. 2. Notwithstanding the provisions of paragraph 1, remuneration derived by a resident of a Contracting State in respect of an employment exercised in the other Contracting State shall be taxable only in the first-mentioned State if: a) the recipient is present in the other State for a period or periods not exceeding in the aggregate 183 days in any twelve-month period commencing or ending in the tax year concerned, and b) the remuneration is paid by, or on behalf of, an employer who is not a resident of the other State, and c) the remuneration is not borne by a permanent establishment or a fixed base which the employer has in the other State. 3. Notwithstanding the preceding provisions of this Article, remuneration derived in respect of an employment exercised aboard a ship or aircraft operated in international traffic by a resident of a Contracting State, may be taxed in that State.
